هذا الفرق Composers is a poly-acid modified resin-based composite material. It’s a mix between composite and resin modified glass ionomer cement ولكن خصائصه أقل من ال composite وكمان خاصية fluoride release أقل. فما لها ميزة إضافية فما سارت تستعمل لأن composite لوحده خصائصه أفضل، أو RMGIC لوحده أفضل بالنسبة ل SDR هو bulk-fill composite إحدى منتجات شركة Dentsply

@@drafnanoalzain يعني دكتورة بقدر استخدم ال sdr في class 1 او class 2 عادي مافي اي مشاكل؟ ولا عندو contraindications في الكافيتي الكبيرة؟

@@baselyahya8963 عادي ممكن يستخدم في أي كلاس ولكن يحتاج عناية وحرص وقت ال application لأنه flowable فممكن يصير فيه voids حسب الدراسات البحثية

Thanks for the explanation but I have two questions, please which technique most used in clinic and witch is the best technique for application

You are welcome. The answer is variable and depends on the case. 1. The most used technique in our school is the selective etching technique because a lot of cases have a mix substrates between enamel and dentin. However, other schools may prefer a different technique in application. 2. Which technique is best: the gold standard is the 3-step specifically bonding to enamel. The 2-step etch and rinse was also reposted to have effective bonding to enamel specifically. The 2-step self-etch was reported to be better for bonding to dentin. Some schools prefer the one-step when bonding on dentin. When enamel is involved they use the selective etching technique. There are a lot of studies that reported the effectiveness of the different bonding systems. So as I mentioned, it really depends on the substrate you are treating and the case.

الفيديو حلو شكرا دكتوره .بس في ملاحظات .😉 أولا. كيفية تطبيق الحمض وهذا ضروري هذا اجاني سؤال من برفيسر بجامعه اوربيه انه ماهي مدة تطبيق الحمض بالضبط وكيف تبدأ ؟ الجواب مدة تطبيق الحمض ١٥ ثانيه على العاج وبعد هذه المده يطبق على الدنتين ايضا ١٥ ثانيه ويكون المده الكامله ٣٠ ثانيه.بعدها غسيل لمدة ٢٠ ثانيه. بالنسبة لل2step المحتوى في العبوة بحاجه ليتطبق على مرحلتين الاولى بالفرشاة ثم تنشيف خفيف انتظار ٢٠ ثانيه للتفاعل ثم تطبيق الطبقه الثانيه تنشيف خفيف وتجميد بالضوء. سؤال كمان مهم .متى نتفادى تطبيق الحمض على الدنتين وماهي البدائل؟ منتجنب تطبيق 37%H3Po4 على الدنتين بالصناديق العميقه لما مابيظل في دنتين باسفل الكافيتي يعني صار الحفر قريب للحجره اللبيه ,هون اذا طبقت حمض ممكن يتأذى اللب السني و ندخل ب التهاب اللب السني. لذلك بالصناديق العميقه يكفي تخريش العاج و استخدام self etch primer . اليوم صار في الجيل السابع الي هوي All in One ذاتي التخريش يحتوي برايمر بوندنك يتصلب بالضوء .كذلك يحتاج لتطبيق على مرحلتين اول طبقه تهويه خفيفه انتظار ٢٠ ثانيه ثم الطبقه الثانيه تهويه خفيفه تجميد بالضوء. صلابة هذا الجيل عاليه مقارنة بالإيجاب السابقه بحيث تصل إلى 25Mpa. ايجابياته يختصر الوقت والخطوات و المريض لا يشعر بطعم الحمض . ممكن أيضا مع هذا الجيل تخريش بالحمض مع انه لديه القدرة على التخريش الذاتي.

صحيح.. المدة الزمنية تختلف من منتج لمنتج لذلك يجب دائما اتباع الخطوات الموجودة في منشور المنتج واتباعها بالضبط حتى يتنج عنها قوة عالية

@@drafnanoalzain صحيح يادكتوره ... من فتره زاد تأييد فكرة dopll application نظام التطبيق الثنائي للbonding وفي توجه حديث لتجميد الطبقه الاولى بالضوء ومن ثم تجميد الطبقة الثانيه أيضا بالضوء بدل التنشيف الخفيف الطبقه الاولى بالهواء. وانا بشوف تجميد الطبقتين بالضوء أفضل. المهم أيضا إعطاء البوندنغ فتره لحتى ينضج قبل مانطبق عليه الكموزيت او flowable komposit . وبالمناسبة أنا بشوف تغليف ال Bonding ب بطبقة ولو نصف مم اي ٥٠٠ميكرون مفيده جدا ولازم تطبق على اي صندوق و كذلك في طريق رفع الجدار المتهدم في approxmal Area الي تسمى deep margin elevation بس كمان هون في ملاحظه مهمه 😉 انه بعد وضع الماتركس وضبطها بالشكل الصحيح منطبق طبقة flowable komposit و مامنجمدها و مباشرتا عليها منطلع بالكومبوزيت والطبقتين مع بعض بيتجمدوا بالضوء. طبعا الماتركس و wood wedge مهمين يركبوا و يشكلون ضغط لتجنب تشكل اي overhang in the approxmal Area.

@@nabilalhmaidi1887 حسب الدراسات الحديثة لا نضع طبقة من البوندنج فوق سطح الكمبوزيت لأنها طبقة جدا خفيفة وضعيفة تذهب مع الوقت ويترك كمبوزيت غير ناعم ويأخذ التصبغات. فالأفضل تلميع الكمبوزيت بدرجة عالية. والنقطة الأخرى التي أيضا أثبتت الدراسات عدم استخدامها هي وضع طبقة من ال flowable في قاع الحفرة حتى تصل للأماكن الصعبة ، اكتشفوا أنها تضعف الحشوة لأن قوة المضغ عالية وهذه الطبقة في القاع وتحتوي على نسبة قليلة من ال filler فيسهل ذوبانها مما يؤدي إلى فراغ وتسوس في المستقبل، وكما قد يسهل كسرها مع ضغط المضغ. فالأفضل بدل من وضع هذه الطبقة من ال flowable هو تسخين الكمبوزيت بجهاز خاص composite heater حيث تكون شبه ال flowable في سيولته ولكن بقوة الحشوات العادية فعند تجميدها تكون الحشوة أفضل. هذا الآن منطقة أبحاث كثيرة والمجال أكثر للاستكشاف والتطوير.
