Benign Prostatic Hyperplasia (BPH), or simply enlargement of the prostate gland, is a condition that is common in men as they age and can oftentimes cause confusion when it comes to the diagnosis and staging of prostate cancer. For example, BPH can cause a higher PSA which is often mistaken as a sign of more serious cancer if a physician uses older staging guidelines. Here, Alex asks medical oncologist Mark Scholz, MD, about BPH, its potential side effects, and what patients need to know about how it can potentially confuse the diagnosis or staging of prostate cancer.
0:05 What is BPH? How does it cause confusion in the world of prostate cancer?
1:42 What is a "normal-sized" prostate, and what is considered enlarged?
2:42 What causes prostate enlargement?
3:24 Are the symptoms that are often associated with prostate cancer (urinary issues, etc.) actually BPH symptoms?
4:51 What happens to the urethra when a prostate is enlarged?
7:06 Can BPH cause sexual dysfunction?
7:45 Is blood in the urine a potential sign of BPH?

My prostate was measured at 62cc during my biopsy. My PSA counts are consistently between 4.5 and 5.5. Unfortunately, I’m starting to experience urinary issues, primarily during evening trips to the bathroom. Thanks for sharing this valuable information for those of us worried that a high PSA is a sign of cancer.

For the past years, I have had to go to the bathrooms numerous times in one. Yesterday, I flew from Bangkok to LA and had to use the bathroom 5 times of the first three hour flight and then 6 times on the long haul. In addition, prior to boarding I had to go to the bathroom 5 times. Then when I land two more times. Trying to sleep tonight and I had to go 5 times already. So, this is 21 trips to the bathroom. My fluid intake is not abnormal.

Excellent discussion. I wish I would have seen before my biopsy. I had a biopsy that was recommended by my doc because my psa was 5. My MRI showed a 47cc prostate size prior to my biopsy. And based on the comments here a 5 would be normal for a 47cc prostate size. Doesn’t seem like I really needed the biopsy.

My enlarged prostate may have saved my life. I went in for urinary symptoms and ended up also having an aggressive prostate cancer, 4-5, 5-4. My lady medical practioner hadnt been ordering psa tests or digital inspections in my exams, so it went undetected. Luckily, the cancer was confined to the prostate.

First, thank you for all your informative videos, much appreciated. I have prostate cancer and have been on active surveillance for 3 years. Can you simultaneously have prostatitis and/or BPH at the same time as prostate cancer?

PCRI is the one source that every adult male should seek out and learn from. My first TURP was in 2009. My “ peach size” prostate PSA went from 2.0 to 6.3 over 12 years. After years of enduring all the signs, symptoms, diagnosis procedures and TURP, the HoLEP procedure finally brought long term relief. In my case Medicare covered.

I'm terrified of getting older. I'm just now realizing how humiliating and painful this is. Urology offers nothing but pain and humiliation. Desperately need more women in urology to make it the tiniest but better. This is so terrifying.

@@jimbrew4529 I had my surgery 15 mths ago. Urinary problems gone although I have minor leakage and wear 1 light pad per day. The trade-off was worth it. I also have ED which is unlikely to improve. Radiation was not recommended as it would further impact urinary symptoms.

I was initially diagnosed with BPH , after tests and a MRI which didn’t show anything sinister, but PSA kept creeping up and the biopsy confirmed PC 3+4 Gleason 7 and graded at T3a , thank god for the PSA , still don’t know how a MRI scan missed it

@@wormsnake1 I proceeded with a prostatectomy robotic keyhole surgery by Da Vinci robot, the was delayed by 4 months during the height of pandemic lockdown here in the Uk. Was out after 2 days and I am being tested for PSA annually , so far reading 0.1 PSA .I am well but really hit me mentally and although free of cancer their some permanent side effects following surgery, that you do eventually learn to live with, please feel free to ask any more questions
