Serum testing is useless. I’m sorry, but you need to go off symptoms. My results were “normal” but I still had hot flashes, depression, low energy and I hated my husband and my life. Increased my estradiol and now feel great (with zero “high estradiol” side effects), but I’m “out of range”. Thank god I have a menopause specialist who didn’t try to convince me that “normal” was the end goal.

Unfortunately I live in the UK and we don't have anything like this at all, the only thing that the doctors do is take your blood pressure and then prescribe you the HRT

Your oestrogen levels can fluctuate from night to day, so a blood test is pointless, that's why we don't do it in UK

Great Video, love your channel. There is so much contradictory info as not only yourself, also other menopause info providers support in the test hormone blood levels. Can you please explain how having blood levels tested for hormone levels is a reliable method in also determining hormone dosage(s) or if you have made a video explaining this . As on another channel it was said that the aforementioned was not a reliable method to use, as hormones fluctuate day to day hour to hour possibly due to diet/stress, etc and to only increase/decrease HRT dosage in relation to menopause symptoms??? if a client says she suffers from insomnia, anxiety with no bloodwork done, I would think and theorize her symptoms could be a result of too high/low of either hormones of which those symptoms can fall into either category and either hormone which could result in a long trial and error guessing game to ease the clients' symptoms and finding the balance of hormones......I speak from personal experience and a dr who refuses to acknowledge the hormone blood testing.

I just had mine tested last week. 83 estradiol, 3 progesterone, FSH 15.5. This was after taking off the patch and stopping progesterone because I felt very PMS-y on them. I feel ok right now but we’ll see. I’m 49 and have ovaries but not a uterus. I emailed you to find a specialist in my area, but haven’t heard back

Thank you very much for this information! I’ve been on 0.05 mg transdermal estradiol and 100 mg micronized progesterone pill for 3 years and have been feeling well - no more menopausal symptoms. However, I have beginning osteoporosis per a DXA scan and it turns out my estradiol level on the above dose is only 26-30 pg/ml. The endocrinologist I see for the osteoporosis says this level is good enough, but after doing some research and viewing your video, I worry that my estradiol level is not high enough to help with the osteoporosis and the prevention of the other diseases you mentioned. I am 57 y.o. Please, do you happen to have a scientific source that I can show to my doctor or some documentation that talks about the minimum estradiol blood levels for prevention of the diseases associated with menopause, and specifically osteoporosis? I have tried to find this on my own, but have found it difficult to get the information. I would be very grateful.

@@danskdna8550 It’s now a year later, and I’ve learned a lot! The the supposedly “minimum dose” of estradiol (patch) to prevent diseases associated with menopause was not enough in my case to help prevent further bone loss. I went up a dose and one of my symptoms - hypercalciuria (too much calcium in urine) that had been detected consistently over a one year period, disappeared! I did a lot of research and found a lecture on the Bone Health and Osteoporosis Foundation website about exactly this topic - the necessary estradiol blood level range for prevention of bone loss and excess urinary calcium loss. The frustrating thing was that my endocrinologist had absolutely no knowledge of this. In terms of your point of why increasing the dose when being symptom free, it turned out that I was totally surprised about how much better I ended up feeling on the higher estradiol dose! More energy, faster brain/thinking … What I’ve learned is that we can get used to how we’re feeling, because the menopause transition stretches over many years… My advice based on my personal experience is that if in doubt, women should not hesitate to experiment with different dosages, whether higher or lower, to find the one that’s optimal for them. Everyone is different and the doctor has no way of knowing how much an individual woman needs to feel her best and reduce her risk for the diseases associated with menopause. Sorry for the long reply, but I hope it might help/encourage some women on their journey. 😊

Great information Steve. Glad I found you. Trying to get to the optimal estradiol range (75-100) to relieve my low estrogen symptoms. When should estradiol blood level be checked after increasing Estradiol patch dose? And if blood estradiol levels are below optimal how long before safely increasing the patch dose? Thank you.

It usually takes a few weeks for your estradiol to settle out after you've increased the dose. I'd suggest not testing until after 4-6 weeks on the new dose. Your provider may have another ecommendation.

Hi Dr. Great video. I’m 54 years old. Started estradiol injections 2 months ago. Supposed to go for 3rd tomorrow. My levels were just tested this week & level is 22. Started out at basically 0 in Feb in spite of doing Bioidentical troches. I’m still so tired even though on antidepressants. Do you think trying to get above 22 will help. Also how do you feel about IM being injections vs patches & pills. Thanks so much!!!

I appreciate you going over how hormone levels change with age naturally in different videos, but I have this lingering question. How does being on hormonal birth control affect all these things? Does it suspend your aging? Does your body stop producing its own hormones in rhythm? Do women on birth control still go through perimenopause? Does long term birth control usage delay menopause or speed it up? Thank you for your concise, helpful explanations!

Inga - Those are very good questions. Hormonal birth control has complicated effects both while you're taking it and afterwards. I haven't done that much research about it. I'm not so convinced that it's the best approach for all women.

@user-xq3kt9mb2x That's a great question. The levels of several biomarkers are in different UNITS in the UK vs. the US. Estradiol is commonly referred to in "picomoles per liter" (pmol/L) in the UK. In the US, we refer to levels of estradiol in "picograms per millileter" (pg/mL). There are several online calculators that help convert one to the other. Here's a link to one I used unitslab.com/node/113 The bottom line: I'm saying a level of 100pg/ml (US) is "optimal" for estradiol. That translates to about 368pmol/L (UK) or right in the middle of that 300-400pmol/L range you mentioned. So those "optimal estradiol" ranges are very consistent with each other. It's just a matter of looking carefully at which units are mentioned.

Now, my question is, why does a post-menopausal woman taking testosterone injections increase body fat, hunger, and increased size in breasts. Which aromatase inhibitor should we take. Someone told me about aromatase inhibitors and where do we buy them?

@jbstepke3 Thanks for watching and for the compliments. Your questions have me thinking there must be several things going on. Yours sounds like a good case to try and find help with. If you can find a hormone specialist who understands all the nuances, that would be the best approach. As for aromatase inhibitors, they have their place but often create more problems than they solve. I would rarely recommend them for men or women.

I experienced those same symptoms and many others when my estrogen had tanked. I started on a .075mg estradiol patch and am now taking .1mg. I am 45 yrs young😊

Hello, I thought blood levels only showed what is circulating in the blood and not what is available to the tissues and that saliva tests where more accurate.

I dont understand why gynos dont test estradiol levels in ages 20s, 30s, 40s etc. Mine were never checked til AFTER my total hysterectomy and its been hell trying to figure out how to remedy all the herendous problems its cauaed.

Most hormone providers Will recommend testing about 5 hours after the last dose. If you normally take estradiol at night, you might take it in the morning the day of your blood draw instead. Of course, if your doctor has specific recommendations, go with those.

I’m in post menopause and got on the E patch two years ago. I’m now on the .075 E patch and my level is 69. I’m really trying to get it closer to 100. Hopefully by going up to the highest dosage patch I will get close to the 100 level or at least 75.

@crh251 That's a great level to shoot for. From my wife's experience and from my survey of providers on my email list, it's often difficult to get to 100pg/ml without getting breast tenderness or other side effects. Even 69-75pg/ml would be pretty close to an "optimal" estradiol level though.

Hi Steve, I started estradiol 1mg for menopause. Ever since I’ve had a low grade annoying headache for the last month. It was enough to make me stop taking it for a few days just to be sure it was actually the estradiol causing my headache. It went away after about 36 hours. Any advice? I want to take it for the health benefits for menopause. Also my hair started shedding shortly after starting it.

About 10 years ago when I went through menopause I didn't have any hot flashes or night sweats no discernible mood swings or anything I really couldn't tell any difference except for my whole genital area went dry and no sex drive then continued to shrivel up now I have vaginal atrophy bummer. I couldn't even have sex with my new boyfriend my vagina was too small I told him that we could work on this together that I needed some estrogen cream and dilators but he wasn't interested in showing me how to order things from the internet so I never did get the estrogen or the dilators and he just quit seeing me. he wasn't for marriage anyways. Oh I forgot my question why didn't I have any of the normal symptoms I didn't have any symptoms at all no night sweats no hot flashes no mood swings nothing but I do know that I was in menopause. Now I'm 66 so it's been more than 10 years probably 12 or 13 and my vagina continues to shrink and shrivel and get dry this is really a bummer and no one would give me any testosterone bulshit. Bulshit.

Watch the whole video and she will talk about "Yoni Stones" which she says will bring blood back to the vaginally area which will plump up.the cells and hence get rid of the dryness and atrophy. My estradiol was very low. 1mg of gel was not working for me. The doctor started giving me 1mg patches witch seem to be too strong. My hot flashes stopped, I still have brain fog. My doctor is new to this and doesn’t have experience. From Doing research I’m hearing that patches are stronger than cream and gel, therefore you don’t need as much. Is this true?

1. Most doctors will recommend using estrogen at bedtime. That makes your level as high as it can be, early in the morning when hot flashes can wake you up. 2. Most of the providers I work with avoid using estrogen in perimenopausal patients, mainly because estradiol levels can be sky high one day and rock bottom the next. It's very possible to overshoot estradiol levels in a perimenopausal woman.

HRT won't help you in the long run. And once you stop it the symtoms will return and you'll be a lot older dealing with them.

Hi there. Thanks so much for this. I can’t get beyond 50 without terrible breast tenderness. It so uncomfortable I have just ripped my patch off of 75 after using these for a month I can’t cope with it any longer. The breast tenderness will just not go. Is this due to estrogen or progesterone sensitivity? Many thanks

@@SimpleHormones Thankyou. I’m already taking 100 mg of utrogestan daily. Breast tenderness is now gone at 50 estradot patch. I’m also using testim, vagifem and the cream….it’s a LOT! Poor women it never ends 🙏🏼

Should I lower mine? I'm on hrt and my serum level is 122 pg/mL. But I have a lot of belly fat. Doesn't make sense. Seems I should be losing. Im paleo and exercise 4 times a week with a combo weights (anaerobic)/aerobic routine.

That's actually a pretty good estradiol level. There are lots of reasons for weight gain or not being able to lose weight, including low estradiol. You need to do intermittent fasting and completely change up your diet. It's the adrenals producing more stress hormones such as cortisol.

Hi Dr. I have been taking 17beta-estradiol 0.06% gel one pump and 100mg prometrium everyday, but lately I am having a little hot flashes etc, can I increase my dose to 2 pumps, if so do i have to increase the prometrium as well, or would I be OK with 100mg? Please help Thank you

@marystahl 2.8pg/ml is a "normal" estradiol for a woman in menopause. That's not 0 but pretty close. Not to throw your doctor under the bus but that's a less-than-optimal estradiol level. It's nowhere near 100pg/ml.

So what the heck do women in perimenopause do?! We get very low estradiol and have all of those symptoms - we need sine kind of continuing estradiol to keep it even - and so what if it gets too high - I’d rather that than suffer with the symptoms - no one will give me estradiol and I think I need it - I’m so frustrated

Good observation! It IS a conundrum. Giving perimenopausal women estradiol can have some major drawbacks, especially sky high estradiol levels that you're adding MORE estradiol to. I've talked with several providers and asked them this questions specifically, "Do you give estradiol to women in perimenopause?" The answer is often whispered, "Well . . . I know it's tricky but I DO give perimenopausal women estradiol. I tell them to use it on days when they have the most hot flashes and back off if the hot flashes do." Estradiol levels in perimenopause are essentially useless, since they'll be entirely different tomorrow.

@Jean-bw7qn Thanks for watching. You mentioned the DOSE of estradiol you're taking - 1.0MG. That's certainly a starting place. When I talk about "estradiol level" I'm referring to the amount of estradiol in your blood (technically your "serum" but basically blood). A 1MG dose of estradiol will produce varying blood levels in different women. The only way to find out your blood estradiol level is by having what's called a "hormone panel" drawn by a laboratory. Most hormone providers will order a comprehensive lab panel that checks all your hormone levels but also looks at blood lipid (cholesterol and fats in your blood), blood glucose and insulin, and other "biomarkers." You get some vials of blood taken out by the lab and 10 days or so later, the lab sends the results to you, your doctor, or both. Then you can see how your levels compare to optimal hormone ranges.

Hi. Great video. I'm from the UK and I had a hysterectomy bilateral oophorectomy 18 months ago and im really struggling on HRT, I use patches (changed from the gel due to migraines) and I use testosterone & progesterone, but my symptoms are just a rollercoaster. I seem to get a constant hot face, all day everyday. My levels were 598 last time I had bloods. Please help

Leanne I’m 69 and have been on a small bioidentical dose for 8 years and asked PCP to check levels, estrogen was 557, she said it was “high…….but you’re supplementing!” Said keep taking. Got second opinion and that nurse practitioner said she’d never seen a level so high at my age and wants it 60. I’m only on it for bones as had no symptoms of menopause but seems there’s no clear cut answers to what levels should be. Everyone has a different opinion.

BIG difference! Premarin® comes from "Pre(gnant)-Mare(horse)-(u)rine." The Premarin® tablet is sugar-coated so that you can't smell it. I have cut a Premarin® tablet in half and sniffed it. Smells EXACTLY like horse pee! Premarin® contains at least 10-12 but maybe as many as 60 different estrogen hormones, the vast majority of which are ONLY found in horses. It's a DIRTY, MESSY, CONTAMINATED hormone that I wouldn't recommend for any woman.

Whether that oestradiol level is "normal" depends on a number of factors, including: 1) whether you're still having regular periods 2) when the blood was drawn in the menstrual cycle

I haven't seen Mylan estradiol patches up close for many years, since I worked in a pharmacy. But that's most likely possible. The main question is if it's "dry" meaning there's no liquid that is going to ooze out when you cut it. If so, then you should be able to. As long as the adhesive is evenly distributed. You could also ask the pharmacist at the pharmacy where you got that prescription filled. for their recommendation

I live in Canada./and my recent estradiol lab was 164 pmol/l. I am 51, no uterus so cannot track by cycle if in menopause. Does that number indicate menopause? I see your measurement in pg/ml. So i am confused?

That calculates to around 44pg/ml. It’s somewhat low. Estradiol alone can’t be definitively used to diagnose menopause. Most providers use FSH to make a diagnosis. Over about 50 is considered menopausal.

@@jh9912 Pretty sure it’s 50mIU/ml. There are different values floating around. The key is if your FSH is consistently above something like 30 on the low side and 50 on the high side, it’s probable you’re in menopause. One FSH test may not be enough because everyone’s levels fluctuate. Yours sounds like it’s kind of borderline and a bit inconclusive, even though it is over 30.

@natasha09179 The answer to that question is a bit unclear. I did a brief search in the PubMed database of studies. Some of them indicate inositol increases estradiol. Some indicate it decreases estradiol. In either case, the impact of inositol on estradiol was relatively small.

My platelets have been dropping since my Covid vaccine but I have also not been taking my estradiol regularly at all due care-giver stress. Could this cause low platelets and it not be the vaccine?

I'm not aware of platelet issues with vaccines. There are ALWAYS complicating factors. I'd suggest the best thing you can do is to take your estradiol as directed by your provider.

my worst symptom ever of having too high of estradiol was a period that started super late (day 37, I normally start around day 28) with extreme PMS worst I've ever had in my life, and a very long drawn out period with a week of light or spotting, a week of extremely heavy with one day of flooding which was scary, and another week of just spotting, worst experience of my life, had a bunch of tests done to rule out anything abnormal, and even thought my doctor didn't check my hormone levels, I think here in France they think it's pointless if you aren't fully in menopause, he only went off of my age, how my organs looked, and that there was nothing abnormal on the tests, I was started right away on an extremely low dose of dienogest, 2mg (it's an extremely common progestin here in Europe, yes, it's synthetic, but I've read that it can mimic certain things of bioidentical progesterone), was given tranexamic acid for a week to slow down the bleeding, then a single injection of decapeptyl, a GnRH agonist, to cause temporary menopause for about 2-3 months to see if it would help. I have to admit all those drugs in the first month of treatment was absolute hellish, it wasn't bad enough to go back into the doctor or to the ER since the side effects were within normal range, but I only had a week off from bleeding before a long drawn out period started again because the decapeptyl stimulated my ovaries first before shutting down and it was like my symptoms were almost worst before it got better.

Hello Steve, I’m in Perimenopause. Yesterday, I was informed that my Estradiol is 500 pg but it should be around 50 pg. Is that dangerously high and require balancing out with medication or does Estradiol fluctuate a lot like Progesterone? My Estrogen is 951 so is that too high? My Progesterone is only 0.2 and I was told it should be somewhere in the range of 15 to 30? Unfortunately, I can’t seem to lose weight even though most of my adult life I was slim with a lot of running and avoiding bread, rice and pasta. I still avoid those foods and I don’t eat sugar but due to injuries I can no longer run. If I take medication to lower my Estradiol and increase my Progesterone, will I finally be able to lose weight? When during my cycle should I be having these hormonal blood tests for Estradiol, Estrogen, and Progesterone? Thank you.

Hello, thank you for your videos. Do you know if transdermal estradiol 2 pumps would effect a benign thyroid nodule 1cm. I understand it doesn’t go through the liver it goes straight into bloodstream. My concern is the nodule will grow with estradiol but my symptoms are not good without it. Thank you

I haven't read anything or heard anything at a conference about estradiol and thyroid nodules. Interestingly, there is an ongoing study that's apparently looking at just that question. I am 57 years old and on HRT for six months now and my estradiol rised from 15 to 277 pg/ml, on one pump a day topical bio identical cream . I am feeling so much better now than 6 months ago, should I need to reduce my dose? Even if feeling great now?

That sounds like a pretty good estradiol level to me. Of course, it may go up or down, based on when you use the cream. As long as you're not having side effects (breast tenderness is most common, followed by bloating, headaches) there's no need to change. It's best if your doctor checks your level once a year or so. As I mention in several other videos, it's VERY important to be taking an ORAL progesterone along with estradiol to prevent endometrial overgrowth. (Even if you've had a hysterectomy and don't have a uterus, I still recommend taking progesterone to help with insomnia, depression, anxiety.)

Should I take progesterone with my estradiol patch? I had a hysterectomy where they removed uterus and ovaries as well. I had horrible endometriosis and don’t want it to come back. I’m miserable and my levels are low on estrodial at 47ml my fsh is 50 and sex hormone is 34. The strange thing is that my testosterone is 40 and the free test is 7ml/ph. That seems high to me. So not sure what to do.

Can I ask Are Estradiol patch as good for reestablishing those levels. I understand that HRT treatment always works with Progesterone But it can also interact with the absorption of estradiol treatment. Would love to hear from you please

Thanks for watching. As I’ve mentioned in some of my other videos, there are advantages and disadvantages to different dosage forms of hormones. Estradiol in a transdermal patch is equal in effectiveness with estradiol oral capsules or tablets as far as getting optimal levels and relieving symptoms. Oral estradiol has distinct advantages in reducing heart disease risk, provided it’s given early in menopause. Progesterone is a different story. Progesterone given in a transdermal cream is ineffective for sleep, depression, and anxiety. Transdermal progesterone, more importantly, doesn’t protect the uterus from overgrowth. I always recommend oral progesterone.

Hi, I suffered through perimenopause severe symptoms) now in menopause started with estradiol patch 25, did nothing really, 50 was wonderful, I could sleep again and feeling myself, but after a few months, I started getting hot flashes and brain fog again and very tender breasts, moved to 75, tenderness gone for the most part but after a few months I am getting random hot flashes, brain fog (like memory blanks) and other symptoms again… should I move to 100?

You'll definitely need to work with your provider to try and find doses that 1) work to relieve symptoms and 2) don't produce unbearable side effects. An experienced provider knows how to find that balance. Estradiol is one hormone that is the most difficult to pinpoint an exact dose. My wife takes an oral capsule and a tiny bit too much causes her breast tenderness, too little and she gets breakthrough hot flashes. She now is alternating between a compounded 1.5MG capsule and a compounded 1.75MG capsule, every other day. You won't be able to get that kind of precision dosing with a manufactured patch. Only a compounding pharmacy can provide precision doses to that level.

Estrogen levels can fluctuate wildly from one day to the next. So what happens more often that "too much estrogen feel[ing] like low estrogen" is "way too much estrogen one day (the day it got measured) and way too little the next (a day when it did NOT get measured)."

Estradiol tends to fluctuate a lot over the course of a month. It's hard to tell what that level means without understanding the overall context and what all your hormones are. Low estradiol may be one factor that can contribute to infertility. It requires a trained hormone specialist to figure that out.
