Mr. Sharma, having severe Left Ventricular Dysfunction with an ejection fraction~15-20 percent and Left Bundle Branch Block who despite optimal medical therapy, complained of progressive breathlessness on exertion for the past six months. Owing to technical reasons conventional CRT-D could not be performed and hence the cardiologist had to resort to LBB-CRT-D (left bundle branch CRT-D) implantation which is a kind of physiological pacing and has shown to have a good clinical outcome.
