The runner up is not the one that gets a +1 spot to their region at msi, it's the second best performing region.

if a single region is both winner and runner up (ex SKT wins msi, and GenG are runner up) does that mean Kr gets 2 extra slots for a total of 5? winner is guaranteed a spot a worlds (if they make their summer playoffs) and runner up adds an extra slot so that would theoretically make 5 slots for Kr (in this specific scenario).

No. If the were to be 1st place T1 2nd geng 3rd blg. T1 would secure their spot at world with 3 other seed from lck. Lpl would then get the 4th seed as they would be the second best region at msi. Its not runner up that secure extra spot. Its the second best region of the tournament
