Dr. Parsa's signature Trifecta Lift procedure, that leaves patients with natural-looking results, has been developed over the course of many, many years. Originally, the purpose of the lower eyelid blepharoplasty was to remove fat from the under-eye via a huge incision made on the outside of the eyelid. As years passed, we learned we can actually hide the incision on the inside of the eyelid. Years later, we learned that removing too much fat from the under-eye causes hollowness in the long run. ⁣

The Trifecta Lift is a surgical procedure that Dr. Parsa created to target the three aspects of lower eyelid aging. His technique addresses the following 3 aspects of lower eyelid rejuvenation.⁣
• Lower eyelid bags: a customized lower eyelid blepharoplasty is performed through the inside of the eyelids with no visible scars outside. The herniated fat pads are then repositioned to a different degree as needed to create a smooth transition of the tear troughs.⁣
• Skin laxity: a resurfacing or a tightening procedure is performed as needed to reduce fine lines. It must be noted that dynamic wrinkles which appear when we smile giving rise to crow's feet will still be there and treatments with neurotoxins are one of the best and simplest ways to address those issues.⁣
• Volume loss: I find this is one of the most important aspects of lower eyelid rejuvenation that usually gets neglected because we’re so focused on the “eye bags”. For me, the best surgical way to revolumize the mid-face is to use the patient’s own fat.⁣

This is a highly technical procedure, and the goal is to create a natural feel to the mid-face without making someone look overinflated. Over the past 20 years, Dr. Parsa performed thousands of these procedures. Me too 😢 I have had major fat bags under my eyes my entire life and its devastating on my self esteem. I have had men i like make comments on it. I am a single mom and only 40 years old, I need to have this done to feel better about myself and feel confident dating. Did you end up doing the surgery? I always felt the same way. Wouldn't even want to make eye contact with myself sometimes when getging ready in front of the mirror. I'm 2 weeks post surgery and I forgot how good it feels to be able to make eye contact with strangers and not feel self conscious. Hopefully you did it. You won't regret it

My wife is 57 and has had circles under her eyes from a baby! She is losing a lot of weight and will need to have excess skin removed once she has lost all her weight. She was told dark circles under eyes run in the genes! Would a surgeon do the surgery you suggest once she gets the excess skin removed? Could this surgery be done at the same time? Hello! Thanks for your support. As long as she's at her goal weight before surgery, it doesn't matter which surgery comes first. However, we wouldn't be able to do it at the same time. Trifecta Lift takes about 2-3 hours. However, she would need a consultation before to determine if this is the best treatment for her dark circles. Hope this helps!

We both say thanks! We live in the UK and she is presently under 18 stones and needs to hit 9 stones, so much weight still to lose! Her biggest weight though was 22 stones and 7 pounds so already lost a total of over 4 stones! So has a further nearly 9 stones left so you add it all up to 13 stones and 7 pounds weight loss of fat! So you can imagine the amount of excess skin to be removed? Esther says it would be wise to have her eyes done AFTER the excess skin removal just in case there is some excess skin needed to be removed on her face as you never know! By the way as a surgeon have you had clients who like my wife have been in a similar situation and then found excess skin on their face? Just curious as that would mean having a face lift then and would include the eyes being done? Just asking! Thanks for your time and patience! Shame you don't do your surgery in the UK!👍

Doctor, can you please talk about Accutite (non surgical solution to remove fat and tighten skin, and less expensive!) which works similar to Blepharoplasty. Its supposed to be good for 3-5 years. Do you think it is a good option for people who don't want to have a surgery?

Would this fix tear troughs and dark hollow under eyes? I’ve tried filler I like it, it plump up the loss of volume under my eye but didn’t last long so I’m looking into something permanent either this or a fat transfer or a mid face lift which do you think will be more effective in the long run and moneys worth?

Yes, you can have a virtual consultation with our PA Justine. If she deems you are surgical candidate, the next step would be to consult with Dr. Parsa. Surgery is usually booked 4-6 weeks after surgery. Patient's must be willing to stay in the area for 6-10 days and fly back for their follow-up appointments so Dr. Parsa can monitor your healing after surgery.

Great video! Question for you Kami, I've seen another video stating fat graft to the under-eye area shouldn't be recommended as over time, micronized fat can become lumpy and hardened which would be visible under the already thin skin in this delicate area. Would you say its something to be concerned about if someone is considering a lower bleph?

I was reading online that suturing the fat pad downwards over the orbital rim would pinch the fat pad's blood supply and over time, the fat pad would die due to this. How can a skilled surgeon mitigate this ?@@oculoplasticsurgery

I don't ever want a doctor trying to take the fat from under my eye bags and push it/reposition it into my cheek area. I watched a documentary of a surgeon doing the so-called new procedure and I am positive they will be saying in 5 or 10 years that it was a bad idea too.
