As a Michigan OSHA Construction Senior Safety Officer, I arrived at an excavation site to investigate an injury accident. The excavation was 32' deep and 10' wide. When I looked at the excavation I noted there were shore boxes (each 10'L x 20'W) stacked being used. Due to the depth, there was an engineered shor8ng design for the site, which called for steel plates (road plates) to be used with the boxes, and to be installed to within 2' of the trench bottom.the upper portion (above the top edge of the shoring was to be cut back (2' below the top edge of the shoring.) With the company safety person and the site superintendent with me, I looked at the work being done and was stunned. The boxes were 8' above the bottom of the trench with three men working in the bottom. I asked the safety guy if I was actually seeing this. He replied that the shoring was 8' above the bottom, but that was per the design. I first asked the men to be brought out of the trench and pronounced the job "shut down." When I asked to see the design I was told it wasn't on site. The next morning, job still shut down, I reviewed the design with the safety guy and quickly learned he didn't know how to read prints. Someone had convinced him that what they were doing was per the prints. I quickly educated him to the error of their ways. Their excuse was due to the fluidity of the black clay soil the excavator didn't have enough power to drive the shore plates to the proper depth. As the company, supervisors, operators and top person had all previously been on sites I inspected and had issued violations (for unsafe excavations) , I issued Willful Violations.

Marko came to my jobsite Today to get our Team Members' heads around the upcoming work we are about to do which involves trenching and excavating in a busy city roadway. We haven't been involved in this type of work in quite some time because we have been working on a rail system. Marko informed us of this video. In the many trainings I've had on trench and excavation safety over the years, this video was by far the best at explaining how to implement the OSHA standard in the field. I'm in my first year in the apprenticeship program of learning how to be a plumber. I've only dug trenches on working with water lines near the water meter. I haven't worked past 2 feet deep but it's still useful information in case I'm working on a sewer lines that's deeper.

Hello Marko: I just want to share something that all Mature Americans should commit to Whatever their lifestyle or job/career. Each person after high school will reach Intellectual maturity only if they Are mentored by older wiser adults In all areas of life. When we learn Something we should share it with others, or show them the danger ahead of time if they don’t do something a particular way. So every employer and mature fellow workers should take it upon himself To teach younger teens and adults Why things have to be done a safe way. Whether I was volunteering my work or being paid , after I had done a few things wrong in the workplace in the 80s , I became wiser the more skills i was trained on. Thus I was willing to share with younger folks what to avoid so as not to make the same mistakes I did, and to always do your best for your employer but not put yourself in danger. I think OSHA is doing this.

The guy using the jack hammer at 4:42 could use respiratory protection from silicon dust generated from breaking up concrete.

More emphasis needs to be directed to smaller contractors (non-union for the most part). Ask 95% of these small contractors what a competent person is, or how to determine soil classification and they look at you as if you've got 3 heads - they have no idea what you're tslking about. OSHA really needs to watch KZfaq/Instagram and start fining these so-called grading contractors for some outrageous (and fully documented) safety lapses. Vertical/near-vertical unsupported deep trenches are common. Spoils piles placed too close to excavations are very common. This video missed key excavation issues by saying spoils piles need to be farther than 2' away from the trench - yes, but that presumes the trenches are properly supported/excavated.

Great video and Marko knows his stuff, which is always a good thing. A few observations from me (not everything); home-made ladders - I never allow them on site, never mind let someone use them, Most of my work is in SE Asia and home-made ladder are a real problem. Also, so much lifting with the diggers' bucket - for me this is another serious problem and always have the hook removed when the vehicle is inspected for site use. 1, the hook is NEVER engineered, it is an add-on so the lifting capacity cannot be ascertained, and 2, a digger does not have a SWL so clearly cannot be used for lifting of any description. That said, I think its a good video and would use it as a training aid.

Had a cave in at work that pinned two men against a trench box. Problem is, they were OUTSIDE of the trench box....

Anyone else think this dude looks like Adam Sandler?
