The basis for this shtick is adults don't take the things their kids say seriously, so they tend to overlook or outright ignore practically everything Stewie says in spite of the fact he is speaking clearly, concisely, and is perfectly understandable The only times when Stewie is actually understood by anyone other than Brian, Chris, or his baby/toddler-aged peers is when it's to make a joke or for the sake of the plot, such as how Stewie is often understood perfectly by characters outside the family or the main and/or regular cast

For some reason, I expected stewie to reveal that he is an Orange robot named Socket. Peter and Lois confess that they are robots too named Rivet and Widgit. Socket makes the bleep sound again and Brian turns into a tan dog and his collar turns green with a golden diamond. His name is now Sam. Chris and Meg get red hair and are now Robin and Daisy. Quagmire gets a black shirt with red shorts and is now named earl. Socket makes the bleep sound again and explodes turning the church into jesus shit.

For people saying that Seth said that everyone can understand Stewie needs to understand that this is a comic running for over 2 decades and there will be inconsistencies. There is one time where Brain and Stewie discuss whether Loretta is closely related to the family to understand Stewie and she ends up understanding very little. Only Brain and Chris understand Stewie all the time. Also there is a scene where Carter dozes off while driving and Stewie shouts at Carter and Barbara but they end up crashing because Stewie was not understandable. Similarly there will be many cases where many understand him. But the general consensus is only Brain and Chris in the close family understand him.
