I have a question I also got the feeding one and I missed the amount intake I had two cups given to me to give the actor one had 240 ml and other was half so 120ml the actor took one sip of each how much would the total intake be ! Also for the food the actor took three bites in total of applesauce would it be 25%?

Hello, I have a question about measuring the liquid intake. How can I know how much liquid is in the cup? Since the tray would already be prepared upon testing, would the dietary card contain how much fluid is in the cup? and then after the actor is finished eating, you pour the left over fluid into a graduate and see how much is left? Thanks

Okay so I am a psw student, my major issue when feeding is I can’t tell when they have enough water/liquid in their mouth and want to stop, I always spill it after. (I just want to say with food I have no issues, and I never leave liquid just sitting on their face, I wipe it off and apologize instantly)

No, you do not wash the tray (in a clinical setting, the tray will be returned to the dietary department for cleaning. The spoon for the exam is disposable (as is the cup and food container) so it will be thrown away. Best of luck!

how else could we explain what we're doing without humiliating them? im having so much trouble with how to talk to patients while protecting their dignity. not that im doing it on purpose but im just very awkward when it comes to taking to people.

At the beginning of the skill, during the opening, I ask the patient to sit up straight. This addresses his inappropriate position and ensures that I can feed him safely. Thanks for the comment and best of luck!

New York tests through Prometric...your test is the same as the Florida test! All of our resources will get you ready for the state exam!

For most documentation skills, you have a margin of error for documentation. However, with feeding, you are estimating based on a percentage of what the patient started with. The percentages that you have to choose from are: 0% (refused), 25%, 50%, 75% and 100% (all). You will check the box for the one closest to what the patient actually ate. The margin of error on this skill is within 25% of the nurse's calculation. Hope this helps!

As soon as Im done feeding the pt for test will I throw away the food or snack fed and document or the evaluator would see first how much it was fed then I discard of the food?

Do not address the evaluator directly. Trust me, the evaluator knows exactly how much the patient ate (because the "patient" is directed to only take 4 bites, then refuse). They are watching you feed the patient, they know the amount eaten. Simply perform the skill as you would normally. Throw the food and drink containers away after feeding. Hope this helps!

Don’t you need gloves whenever there’s potential contact with bodily fluids? I feel like we used them in lab.

You should not use gloves when feeding someone, unless the care plan specifies glove use. Most gloves are made of latex or vinyl - these substances have an odor which can affect appetite, so when you feed someone with gloves on, those gloves are below the nose of the patient, which means they will smell the gloves, not the food, they won't eat well. In most cases, you are holding a feeding utensil, and not placing food directly into a patient's mouth, so there is very little risk of body fluid contact. Think of your own eating habits...do you get saliva on your spoon HANDLE when eating soup? Probably not. Any saliva stays on the portion of the spoon that was actually in contact with the mouth - and we don't have any reason to touch that. Most people that need help with feeding won't have body fluid issues when eating - and if they do, it will be on the care plan. Hope this helps! :-)

For the exam, everything is disposable. The pudding cup, the cup holding water, the spoon and the napkin can all be thrown away at the end of the skill. Thank you!

If the patient is able to wash his own hands, then why can't he feed himself? Is this just to record the amount of intake?

The care plan states the patient is unable to feed themselves and must be fed. As a CNA, we follow the care plan. But the care plan does NOT say that the patient cannot wipe their hands. Possibilities: Maybe the patient has Parkinson's and shakes too badly to feed themselves. Maybe they have shoulder injuries that prevent them from raising their arms. Or the patient may have suffered a stroke that affected their dominant side. We don't need to know why they can't feed themselves because the care plan gives us the task of feeding the patient. Yes, you will document the amount of intake at the end of the skill, but they are also grading your ability to safely feed a patient. Hope this helps!

Just curious why you choose reusable towels for clothing protectors, but use (and toss) all of those Chux pad barriers when emptying the Foley? Couldn't you use a towel instead and wash it rather than wasting the disposable pads??

If you refer to the Clinical Skills Checklist for Empty and Measure Contents of Urinary Drainage Bag, Checkpoint 4 states: Set graduate or bedpan on barrier placed on floor and empty full contents of drainage bag into the graduate or bedpan. Since we are working with body fluids that may spill, it is best to use a waterproof, absorbent barrier, such as a chux. This prevents any urine that may spill or drip as we empty the bag from passing through onto the floor, as it would with a towel. This is the preferred method for the exam. A clothing protector is designed to prevent food or toothpaste from staining clothing, so a towel is more appropriate, since we are not as concerned with the waterproof properties. Yes, it could, because measuring something without putting it in the patient's chart does not benefit the patient or the care team. This is a VERY important step, so that decisions regarding the patient's care can be made quickly and efficiently. Best of luck!

No, that is not necessary for the exam. The evaluator is there to "set the scene" based on the care plan. So there will be a chair available for you to sit in and you will leave it where you found it at the end of the skill. Hope this helps!

For the exam, there is no diet card to check. This is not a graded checkpoint because the care plan instructs you to feed the patient a snack. Knowledge of specific diets and the importance of maintaining a prescribed diet will be covered on the written test, but this is not a graded checkpoint on the clinical exam, so it is not demonstrated in the video. This is not a graded checkpoint on the exam. This patient does not have a medical condition that requires that they remain upright for 30 minutes after feeding. Thank you and good luck.

The care plan states that the patient is in an inappropriate position for eating. If you notice, the patient was asked to sip up straight before I washed my hands. You do not need to wipe the patient's mouth between bites and drinks, unless the patient needs it - which this patient did not (and most won't). Thank you for your comment.

Hi - thanks for your question! Feeding in bed is done the same way, just raise the head of the bed to the full upright sitting position and make sure the entire bed is in the low position so you can reach the patient when you are sitting at the bedside (you should sit to feed someone so they don't feel rushed). Curtains are not closed for feeding because eating is a social event. Most people gather in groups and socialize while eating. Closing the curtain will isolate the patient and diminish the appetite. You do not need to wipe the resident's hands after feeding them, as they did not touch the food. Washing their hands before feeding stimulates the appetite, as handwashing is generally part of the eating ritual - most people wash their hands before eating. Thank you for your comment.
