Dr. Ebraheim animated educational video describing current concepts of foot and ankle injuries.
I.Ankle fractures
a.Supination-external rotation
i.Vertical fractures of medial malleolus is bad
ii.Differentiate between supination/external rotation and pronation/external rotation by lateral x-ray to see the direction of the fracture
b.Isolated lateral malleolar fracture
i.Nondisplaced with no talar shift can be treated conservatively.
ii.Displaced fracture with talar shift will require surgery.
c.Weber C fractures
i.Syndesmotic injuries are common
II.Ankle fractures & diabetes
a.Surgery is better in diabetics
III.Pilon fracture (tibial plafond)
a.Axial load, high energy injury. Can occur due to a fall or from a car accident.
i.medial malleolar fragment
ii.Anterolateral fragment: chaput fracture
iii.Posterolateral fragment: Volkman fracture (posterior inferior tibiofibular ligament is attached to this fragment)
IV.Treatment of displaced fractures:
a.Delay open reduction internal fixation until the skin condition improves.
b.Soft tissue complications: you need to get skin wrinkles before you do internal fixation.
c.Joint fusion surgery (arthrodesis) for arthritis: not commonly used or needed.
V.Navicular fractures
a.Stress fracture of the navicular is the important one and usually occurs in the central third.
b.Treatment: Cast and non-weight bearing. Do ORIF in athletes, if there is a nonunion, or failure of conservative treatment and also in displaced fractures of the navicular.
VI.5th metatarsal base fractures
a.Zone I: proximal tubercle avulsion fracture. Treatment of zone I proximal tubercle fracture Is usually nonoperative (use a boot or a fracture shoe).
b.Zone II (jones fracture) at the 4th and 5th metatarsal articulation which is the junction between the metaphysis and the diaphysis.
c.Zone III proximal diaphysis fracture
VII. Open calcaneal fractures
a.Open fractures of the calcaneus may lead to amputation. There are two basic fragments:
i.anteromedial (sustentacular) fragment
ii.posterolateral (tuberosity) fragment
VIII.Talus fractures (Hawkin’s classification)
a.Type I: non-displaced. 10% AVN
b.Type II: fracture with subtalar dislocation or subluxation. 50% AVN.
c.Type III: fracture with subtalar and tibiotalar subluxation or dislocation. 90% AVN.
d.Type IV: fracture with subtalar and tibiotalar dislocation and talonavicular subluxation. 90-100% AVN.

Idk, but i slid down a hill with my foot folding to the outside. Now I am in some serious pain. Got no medical.

Probably tore some tendons or strained some ligaments. A sprained ankle. Should be healed by now

No way. It's been like 4 or 5 months and there is pain there off and on like fracture pain. Can't jog. I can't walk right sometimes. I really have to take care of it now. I think it was 100% broken and healed wrong. I did put all my weight on it as it turned backward and I heard popping and cracking.

@@wolverinex4243 yeah sorry to hear that. You probably should have a doctor check that out. Even though it's been a while they may still be able to do something to help you. Probably they will do some x-rays firstly to get a better picture of the situation. I understand the insurance problem, but you may need some sort of medical intervention to keep it from getting worse in the future. Good luck. Also trekking poles come in handy if you're hiking again someday and especially moreso when going down the trail than up it.

@@StAnger561to770 I know. I should. Just don't want to get on gov't insurance. You know. As for the staffs, I should have. I did have to make one out of a branch when I got injured and had one foot left. I was holding my very expensive $700 fishing rod so I didn't want to throw it. Had I threw it, and brace myself maybe it wouldn't be so bad. I really thought I was gonna die in the middle of nowhere. The pain was so brutal that I almost threw up from the shock and adrenaline. I had to walk about a mile with a giant branch, then climb up a vertical bluff. I had a little infected cut last time I went to the hospital and they charge me about $3k. It took me 2 years to pay it off with one lump payment of half of that as my last payment. Hate the hospitals. They're a racket. The pain after my fall almost made me go to the ER a few times as it was frightening for one month. Couldn't get out of bed. The issue now is if I get on gov't insurance, then I don't know where to go for X-Rays as they are only in the ER and we all know ERs over-charge. I really have to get help. You know we take out health for granted too much. Can't achieve wealth if you ain't got health.

@@wolverinex4243 well I'm not sure what country you're in but I would recommend to find out some more about it the government insurance and then just go because they should be charging the government not you. I had a friend in United States who went to the emergency room and then afterwards he was talking to someone in the building department and told them that he has no job and no income and they signed him up for Medicare or Medicaid. It was a huge hospital bill of course but it was all covered. Of course each country has different procedures but I think generally speaking the hospital emergency room unless it's a private hospital they should accept the government insurance. I know how it is though I have a foot injury at the moment I've been putting off going for the last few weeks partially well mostly because I don't have a car and it's gotten better it was really bad the first week or so but I probably should have it x-rayed. If you qualify for the government insurance I wouldn't worry too much about what they're going to charge you for the cost of the visit because it should be covered mostly but you know each country's different you have to check with the local people there about it. Definitely a public hospital though the private ones probably do not accept the government insurance generally that's the same in all countries

I\'m not sure but ,if anyone else wants to learn about tendon swelling in ankle try Fezervin Ankle Fixer Coach (do a search on google ) ? Ive heard some great things about it and my brother in law got great success with it.
