I wonder if Kenshiro gave him a conspicuously less-gruesome death than the rest of Shin's minions because he at least had the balls to go down fighting even when he was clearly outmatched. Heart also got a pretty quick death, too.

It really says something when you can say that a guy's brains exploding out through his mouth and eye sockets is "less brutal" than most of the deaths a particular warrior inflicts on his enemies

This seems different to the other time Ken used this attack. In the other version, he poked his index fingers into the guy's temples before kneeing him in the face. In this one you see Ken using all eight fingers to the sides of the guy's neck before the knee to the face.

Hokuto doesn’t cause external injuries, or well, it doesn’t need to. Unless Ken’s seriously angry, deathblows have the capability of being invisible to the naked eye until after the effect is activated.
