GALS Screen - Gaits, Arms, Legs and Spine - Musculoskeletal Screen - Dr Gill
The GALS screen is a core part of the MSK examinations, for when you are trying to identify an underlying limb problem
It's crucial to note that the GALS screen is technically this, a screening tool, rather than an examination, so the "GALS examination" is thus a misnomer.
A GALS screen would be indicated if a patient answered "Yes" to any of the initial three screening questions:
- Do they have pain or stiffness in the limbs, joints, or back
- Do they have difficulty going up and down stairs due to pain
- Do they have difficulty dressing due to pain
The GALS screen systematically looks over a patient's gait, arms, legs, and spine.
It should be noted that there are several variations to what is contained within the GALS screen. Personally, I prefer to assess the spine after having reviewed a patient's gait.
Remember the core of the examination is finding areas of pain, or discomfort which would then trigger the clinician to do a full in-depth examination of that particular joint or body part

Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical evaluation - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognised standard textbook for clinical skills.

You can’t see behind the camera. These rooms are basically store rooms at the moment. Takes up to an hour to clear before we film!

Serious note, I started watching your videos for the asmr but it was a spinal video from a couple of years ago that made me realise there was something wrong with my daughter's back. We are now 16 months post spinal fusion and with the rate the curvature accelerated, I dread to think how bad it may have got. So came for the asmr and ended up saving my daughter. Thank you :)

Sorry if you feel strange about it but you're the only reliable source of asmr for me. Quite a few medical illustration videos do too, but not the silly ones where people whisper and run their nails over things. I think the cause of tingles is medically interesting, it sure feels strange. Like a blanket of warmth covering you and it comes in waves, inviting you to close your eyes and often times fall asleep. Just because, in my case, someone explains something in a particular way and I get to observe it. It happens in real life too, it takes you over and you look like a weirdo just staring. My mom must have passed it down on me because she experiences it too, just from different triggers I guess you'd call it

Great description; I almost thought I was the only one. I get the tingles from explanations, not from the soft p*rn that is most of asmr videos. I can get it from reading books, since I was a kid. I think it's specifically from getting an explanation about mechanisms people believe in, even if I don't believe in it, so reflexology videos work too.

I find these videos interesting. I fell on my butt on concrete about 8 years ago. My legs went numb for a few seconds but felt normal again. About 3 years later, my feet and legs up to my knees went almost numb. I can still feel pain and temperature in my legs and feet but I have a constant heaviness in them. Some days are better than others. My doctor can't seem to figure it out. I had an MRI done on my lower back down to my feet but it came back fine. I also had a nerve and muscle study done and it came back normal. I just want to get back to normal.

Hi Doctor, you recently answered my question on one of your live streams regarding how you feel about being the patient and putting your trust in other Doctors hands. I found your reply very interesting, you said when you treat other Doctors they give you the information in a different order. In my line of work I have a similar experience and never really noticed before but people definitely jump the gun when they know what you are about to ask and it definitely knocks you off your flow!! Anyway just thought I'd mention it - not sure why! Cheers

Hi Dr.James, I wanted to ask you one question about draught (draft) and its influence on neck and back pain and other areas of body, all of us at least once in our life have been under air condition and such, but does that mean when it attacks our neck muscles for example then you get chronic pain from it or you do some massage or put some heat and you're healed ? In terms of UK norms (or just your own) ... in a scenario where you detected a possible underlying issue (say scoliosis/irregular spinal movement) would you halt the screening and focus or continue before making referrals?

I think while assessing elbow extension and flexion, it is not the best way to do (3:33). And while assessing ankle dorsiflexion-plantarflexion of ankle and ehl strength, one should precipate the shin throughout examination Btw while assessing scoliosis, you should ask the patient to bend forward, i can say, but not so much, this patient has mild scoliosis (you can understand from difference between shoulders of both sides).

Great demonstration, as always At 0:57, patient should walk swinging his arms, but I understand he was withholding them because of lack of space available for a "free walk" when he started, and/or he felt shy (just my opinion)

Yes, it's a slight issue with the space. I explain to the students have the patient "walk at least five paces and turn" as that should be sufficient for the patient engage with their usual gait (Although I cheat and go to collect my patients from the waiting room, so I see them walk unbiased 😉 )

I saw in the comments, you would never recommend a chiropractor, can I ask why and what would you do to fix bad posture in your early 20’s?! Is there anyone I should see for help? I bought a back brace thing off Amazon but not sure if I notice the difference

I would speak to a trained qualified physiotherapist, or your GP (As a GP if there was an issue, depending on the issue, I’d likely refer to physio or Ortho)

You don’t need to have straight A’s to do medicine. I’m proof of that. But it does need a reasonable working memory. If you there are lots of various people in medicine. It’s a huge group. If you think you can. Apply and see 😊

You don’t specifically do an arthritis examination. But you do examine what is needed. So we’ve already got spine, hip, knee and hands recorded so that’s pretty much all covered

Doctor is there a cure for buffalo hump? Would constant appropriate exercises fix it? A part from that I’m quite flexible and fit but my upper back and neck are a mess. This video is great.

Well it depends what the actual issue is. A “buffalo hump” clinically used refer to fat from a specific condition. If you are concerned, have a chat to your GP 😊

What type of Dr is going to do this examination? I mean specialty. Because as a programmer and graduate student who's also very much depressed I spent 100% of my time sitting behind a desk or on my bed watching KZfaq videos. I have severe upper back and neck pains and a bad posture. I don't know if I should see a chiropractor, an orthopedist or maybe a GP to tell me who to see... Like I can't even bring my arms up pass a certain point and I'm 27

I would never recommend a chiropractor. However if you are having joint pains at that age, speak to your GP. It might need investigating
