Everything had to be directly approved by Lucas, and many ideas, including many Ryan has directly blamed Filoni for, came directly from Lucas. And some of those ideas Filoni disagreed with but tried to make work. There is a reason Lucas considers only his movies and Clone Wars to be his "canon."
