If you die during hardmode, do I have to reset the instance? Or can I keep going and get the same ilvl loot.

As far as I can tell, the first boss has another change you didn't mention. It seems that when you drop the puddles, you immediately take a tick of damage (which also hurts a lot more). In regular Mythic+ you can completely avoid the puddle damage if you walk out of it the second it drops.
