Dr Subhankar : massive hemoptysis with normal CT parenchyma..and normal CT angiogram.. How to approach? Pt had normal echo also..

Hi Dr Subhankar! I assume coagulopathy, liver renal ds has been ruled out, sld rule out any vasculitic syndrome/drugs. In a normal looking CT parenchyma - still a role for bronchoscopic evaluation, in an apparent normal CT angio- ask to trace the bronchial Arteries and document their caliber and nature- tortuous/dilated.. review for causes of pseudo haemopytsis. Request the radiologist to advise if repeat CT angio is required with any revised contrast protocol

Role of tranexamic acid in hemoptysis?

Hi! Tranexa has a role as basic supportive care only.. we can't rely on it to control haemopytsis. We must look for the primary cause and manage accordingly
