HMS 'Formidable' launches herself in Belfast, Northern Ireland.
M/S Lady Kingsley Wood arriving with her husband. Various shots, workmen under ship at work on launching gear. Various shots of Formidable breaking away and racing down slipway out of control, smashing all the wooden chocks and throwing wooden platform into the air. Lady Wood smashes bottle as it moves off. Various shots of men climbing from debris. C/U Sir Kingsley Wood watching. L/S Kingsley and Lady Wood enter the car, pan across scene showing debris and people watching departure. L/S Formidable lying offshore surrounded by tugs. The commentator says that one was killed with a few workers injured but not seriously.

Because it couldve been SOOOOO much worse. A whole aircraft carrier lauches herself with around 30 shipyard personnel under her and only 1 person dies, absolute miracle, not a great one as someone died but still a miracle

Bit of a late reply, but whatever. The reason is that the company that built her, Harland & Wolff, had never built an aircraft carrier before, so they had little experience in building the necessary drydock support, as seen in this video. It is essentially 'new guy underperforms due to lack of experience'.
