How are antibiotics chosen for chest infections?

How do doctors choose an antibiotic to treat a chest infection? Normally a chest infection needs to be treated empirically (without waiting for the results of bacterial sensitivity testing - which is often not available). Therefore the choice of antibiotic is made based on how the infection is progressing, how severe it is, if the patient has other chest conditions, if there is a risk of bacterial resistance and infection with antibiotic-resistant bacteria etc.
Hopefully this video clarifies the clinical thinking your doctor may have when prescribing an antibiotic for someone suffering from a chest infection (likely bacterial in nature).
** Not mentioned in the video - allergies are often also considered, as some patients may be allergic to the most common antibiotics used for chest infections.

Thank you, this is very informative. You mentioned amoxicillin and doxycycline as being pretty much the same in first-line of defence. Is that the case? I was given Amoxicillin for my chest infection at first, this not only did nothing but also allowed the infection to spread to my ear. Then I was iven doxycycline as a 'stronger' antibiotic (is that true?). The infection does seem to be getting better, however I had a reaction to doxycycline (rash), so now I've been switched again to clarithromycin. Just wondering - are all three of these basically the same 'strength', and is there a stronger antibiotic that is within the UK protocol to be prescribed for community-acquired conditions?

My dad has pneumonia currently. He didn’t require oxygen and doesn’t have a cough or congestion but had mental confusion and difficulty completing sentences. He was diagnosed by X-ray and elevated blood cell counts. He’s been given 5 days of amoxicillin and azithromycin. After 6 days he still gets occasional fever of 100.6. Is that normal? Can we expect it to get better over the next 3-4 days now that his pills have ended. Does he need another round of pills?

@@DrStefanCristianStanelhi, I was prescribed with a 5 day course of amoxicillin antibiotics around a month ago due to there being rattling in my lower right lobe during stereoscope auscultation (I had been coughing and was constantly bringing up green phlegm). They seemed to clear my symptoms up completely. Last week, I seemed to pick up another cold and have been coughing and bringing up green phlegm again. I went back to the doctor’s today, I was asked if I had been offered a chest X-Ray to which I said I had not. During stethoscope auscultation this time, my chest was clear, but I was advised the stethoscope was not working well and it could only pick up very clear findings. I had the choice of choosing between going for a chest X-Ray or a 7 day course of clarithromycin antibiotics. I went for the latter option, but the doctor also said I should contact them again 2 weeks after finishing the antibiotics this time to request a chest X-Ray if my symptoms have not cleared or if they clear and then come back. I would be interested to read your stance on this approach doctor.

There has to be a more natural alternative for treating pneumonia, right? My 84 year old mom has been rushed to hospital twice so far within the past couple of months, with pneumonia and a UTI. She was treated for sepsis and also had to have a stent put in due to a large kidney stone. Currently, she is being given antibiotics for pneumonia (yet again) and a UTI!

CT Chest A plain high resolution and multiplanar CT of the chest was performed, on a 128 Slice Dual Energy Siemens Definition Edge Scanner. Focal mild right pleural thickening is seen in the lower hemithorax with mild passive and subsegmental atelectasis in the subjacent lung. No other focal or diffuse pulmonary or tracheobronchial abnormality is seen. The lung volumes are R-1.8L/L-1.7L. No hilar or mediastinal masses are seen. The axillary and supraclavicular regions are normal. The rest of the pleural spaces and bony cage are normal. The visualized upper abdominal structures are normal. Remarks: Focal mild right pleural thickening is seen in the lower hemithorax with mild passive and subsegmental atelectasis in the subjacent lung. No fibrosis is seen in the left upper lobe.

Well, all right I’m not sure about this statement you made but I absolutely believe that there is toxins in chemicals and other things in her ear that hasn’t been like this before and I have other clients and other people I know in the last four months that I’ve been getting basically pneumonia bronchitis upper respiratory horribly bad allergies that they have not seen before. They played a record about in the Minneapolis St. Paul area Minnesota. Where are you located? I am just shocked at this. I haven’t had anything like this by 14 years very healthy 40 years old don’t eat sugar I drink water I exercise I use a Air purifier in my House.

@@Timetravel1111 I would love to follow your outcomes? Are you a physician? Indeed we are experiencing a similar situation in our local area. Personally I am having my second bout of lung issues since march 2020. First was in Mar 2020. No CVD testing available to me and I had labored breathing for months with a dry cough and low grade fever. Once tested for CVD it was negative. Tested many times since and negative. However my seasonal allergy to ragweed is virtually non existent now. But the labored breathing/breathlessness and occasional yellow mucus plug is back along with higher than normal CO2 levels in serum. I am on Doxycycline and was given a shot of steroid and a 50mg a day steroid for 4 days (without tapper??) Also inhaler of Albuterol as needed. I was told it is a possible bronchitis flare. Opinions?
