Dr. Cliff Olson, Audiologist and Founder of Applied Hearing Solutions in Anthem Arizona, discusses how Bone Conduction Hearing Aids work.
Website: www.DrCliffAuD.com
Clinic Website: www.AppliedHearingAZ.com
Over 90% of hearing losses require the use of traditional hearing aids. For the other 10%, a different treatment method is needed. If you have Single Sided Deafness (SSD), a Conductive Hearing Loss, or Mixed hearing loss, you may have the option of a Bone Conduction Hearing Aid. Video: 3 types of Hearing Loss: • 3 Types of Hearing Los...
A Bone Conduction Hearing Aid works by stimulating the Cochlea directly with vibration through the skull. This direct vibration bypasses the outer ear and middle ear. In the case of Single Sided Deafness, this signal passes around the skull to the opposite side ear that works properly. Video: Single Sided Hearing Loss Options: • Single Sided Deafness ...
For a Conductive hearing loss, the Bone Conduction Hearing Aid will transmit sound to the cochlea through the skull which bypasses the conductive hearing loss.
For a Mixed hearing loss, the Bone Conduction Hearing Aid will bypass the Conductive Hearing Loss and increase the stimulation to the Cochlea to overcome the Sensorineural portion of the Mixed hearing loss.
There are 4 different methods for using bone conduction devices.
1. Abutment - When a titanium post is surgically implanted into your skull. You then attach the Bone Conduction Hearing Aid or Bone Anchored Hearing Aid (BAHA)
2. Magnet - When a magnet is surgically implanted into your skull. You then attach the Bone Conduction Aid on a magnet that attaches through the skull.
3. Headband - Generally reserved for children, but the band will keep the Bone Conduction device firmly in place. This can also be relatively uncomfortable.
4. Adhesive Pad - This pad attaches via behind your ear on your mastoid bone. You then attach a Bone Conduction Device to the sticky pad. Not available in the USA at the time of this recording.

I have a question I got a potential diagnosis of cogan’s syndrome went deaf in my right ear horrible tinnitus and heard it can be reduced with a cochlear implant. can a bone anchored hearing aid help with tinnitus as well? and what’s the better option for Sensorineural hearing loss with tinnitus? My doctor didn’t seem to be interested in offering a cochlear implant.

Another great informative video! Thank you Dr. Cliff. I was recently diagnosed with hearing loss in the 90 Decibel levels (its not good). I digress. during my visit, the audiologist did a sound test using a bone conduction device and I was amazed how clear the sounds were. I am currently using my Sony MDR-7506 headphones and I cant help wonder what sound quality I am missing. I have grown somewhat accustom to the severe loss. As I mentioned to you in another great video you provided, I found a great hearing treatment (based on your recommendation) and soon I will be fitted for hearing aids. Thanks again Dr. Cliff. I think I can speak for many people, your informative videos have had a very positive impact on the lives of so many people (I see that in the comments).

Based on your experience which of the two hearing aids is recommended to treat microtia. Thank you very much Daniel Quito, Ecuador.

Hi Dr. Cliff! I’m so excited to have found your channel. I have bilateral atresia and have been using a Baha 4 system for the last 5 years. I originally had two sides but the left side failed to osseointegrate so I only have the right side. My surgeon recommended that we wait a few years to try again for the bone to heal. Do you recommend trying the Baha again or should I try a different bone conduction hearing aid that uses a magnet?

Hello! I’ve been watching pretty much every video you have and I have a question when it comes to bone conduction hearing aids. I’m SSD since birth and have had hearing aids my entire life. I’m about to get Osia Bone Conduction hearing aid done in one month. I do have my concerns. Sound quality-wise what gives you the best possible or most natural-sounding sound? Osia Bone Conduction or normal hearing aids? I love the idea of Bone mainly since I don’t have to put anything in my ear and deal with the itch or pressure of hearing aids. I’d also like to keep my good ear to hear normally compared to everything coming from a hearing aid. Which is another big win for bone conduction. But my main concern is the sound. Do bone conduction hearing aids have the ability to sound just as good if not better as traditional hearing aids? Looking forward to hearing you soon. Thanks!

He lost all his hearing on one side and he lost 70% on the other side. Fast forward to today he is now 11 and has been using a traditional hearing aid for 6 years, his hearing just stopped his hearing aid still works but he can't hear anything. Would a bone conduction hearing aid work for him?

I am struggling to understand why the 10% do not benefit from the traditional hearing aid? I have severe to profound conductive hearing loss in one ear from prior cholesteatoma's and after several surgeries it was somewhat repaired but now the prothesis has failed and I do not desire to repeat surgery to try and fix it. Anyways, today I tested the BAHA via headband and a Phonoak Audeo in the office and couldn't tell much difference between the two. I am struggling committing to having the BAHA done when I couldn't see much difference.

My 3 year old has profound hearing loss in one ear and perfect hearing in the other and is not eligible for a cochlear implant. I believe the only other option at this time is a BAHA device (correct me if I'm wrong). If sound is just transmitted to the good hear, does a Baha help with sound localization at all and is it possible to over stimulate your good ear and create long term issues?

Thanks for the video. I have a BAHA since 2002, 3 years ago I changed to a Ponto. I have SSD and wear the Ponto more or less all the time. But since around 2 years I experience occasionally pain in the ear after exposure to loud noise for a longer period of time. It is only on the implanted side and feels like a spasm in the ear. It can be really painful for some hours and then its fine again sometimes even for several weeks. There is no infection, everything looks and works fine otherwise. Did you hear something like that before? Is it something I should be worried about?

I've used Cros to a so-so effect, but now I'm set to get surgery for the BAHA Connect, and I was curious as to why you changed to the Ponto, and what that change entailed? Did it require another surgery, or were you able to utilize the original abutment? Also, which provided better sound? Thanks in advance!

Hi Dr Cliff. I’ve been watching your videos and find them very informative. I’m atttempting to decide if I should do a Baha. I have mixed hearing loss in both ears, With left ear severe to profound in higher frequencies. Been wearing 7 year old Starkey. Had tried OPN 1 with 60db, which I liked. ENT’s AUD recommending BAHA S Superpower, and supplement HA in right ear, which I don’t like. Biggest complaint with Starkey is background noise and speech clarity. I’m having difficulty getting an unbiased opinion as to which is best way to go as everyone seems to have a vested interest.
@DrCliffAuD
@DrCliffAuD 5 жыл бұрын
Depending on the severity of the sensorineural portion of the hearing loss, BAHA may not be adequate for you. However, if the audiologist is recommending the S Superpower, they are probably taking that into consideration. You don't have to go with a Starkey. There are a number of hearing aid brands out there and you should be able to try other brands. Does that clinic only offer Starkey?
@roninglis2711
@roninglis2711 5 жыл бұрын
The rationale for the BAHA is that my speech understanding is near 100%. Left ear drops from 65db at 2000 to 85db at 3000 and 105db at 4000. I am considering the OPN S and have found a provider that offers it unbundled. I did a trial period with the OPN 1 and was waiting on the OPN S to come out when the BAHA option came up. I have also tried Resound Quattro and Siemens, but not particularly impressed with either. Just unsure if HAs or BAHA is best way to go and really have not been able to find an unbiased opinion. I do not care for the size of the Superpower and I'm not keen on punching a hole in my head, so my leaning is HAs.

Hey Dr. Cliff! I had a tympanoplasty with mastoidectomy due to Cholesteotoma a few years ago in my left year. It left me with conductive hearing loss in that ear, but I’ve been able to rely on my right ear and have gotten by just fine. Well now the Cholesteotoma has found it’s way into my right ear and have to have the same surgery. I’ve already had the BAHA evaluation and they said I’m a candidate for it. The doctors think it makes the most sense for the BAHA to be placed on my left ear since we already know how that side is post surgery. So my question is does it matter which side it goes on? And is this still a good option not knowing how the recovery from surgery on my right side will be?

This seems it would be really unconformable over a few hours. Having your skull vibrate would probably get annoying after awhile I am thinking about changing my bones out for titanium but I have a 1 out of three chance for three things. One, I gain all my hearing back, two keep the same amount of hearing, or three I lose all my hearing (in that one ear). I have a disease on my ear bones that creates "plat" and deteriorates the bone under the plat. This hearing loss is progressive and the titanium bones are the only permanent solutions at the time of writing this. I have worn hearing aids in the past but due to natural disasters, I have had them damaged multiple times. As a university student, it is difficult to get new hearing aids at this time. I am thinking about making a device that could be a surgery place where the eardrum and bones are. The problem is power making sure to provide power when the battery or fuel cell has run out. Obviously, this would be a device I would design when I graduate and had enough money to create. What do you think of this and or if I could get a scholarship for a hearing aid? If you think I could get a scholarship for a hearing aid do you know where? This field is full of older people and makes me kind of wish I had some peers to talk to about this. Though it is hard to find peers in this field. I hope you are staying safe and have a good day.

In my case I didn't feel any vibrations as such but I found attaching and removing the BAHA to be very uncomfortable, to the point that I was relieved when the titanium abutment fell out of my skull leaving a hole in my head, obviously spelling the end of my BAHA adventure. That was over a decade ago and the general abutment design seems to be the same concept. I am still deaf in that ear. I asked my ENT consultant about maybe having another BAHA aid, though to he doesn't recommend them!

Pros are that it can't be seen if you take the processor off, less risk of infection vs an abutment, and you can use it quicker after implantation. Cons are that while wearing it stands out more, can't fit as severe of hearing losses, could get knocked off easier, could create pressure sores depending on how long you wear it.

Why use zinc air battery in magnet implant ??
@DrCliffAuD

A magnet implant has no impact on its power source. However, a lithium battery would likely be heavier and not suitable for a magnet implant.

Hello kind sir, I have a question: Can a pair of bone-conducting sports headphones like the one from Langsdom offer sound to a completely deaf person? I am tempted to try it out for my mom because I know she told me once that by adding a speaker on the back of her ear she could hear the sound (I'm not sure if by vibrations she means sound), my doubt is how effective are these pairs if the speaker is placed in front of the ear.

What are your thoughts on Sound Arc?

Hi Autumn. I would put this in the same category as a headband. It still requires pressure to work, and has similar benefits.

Doctor Cliff, AuD Thank you, I have a 17 year old with atresia. He is reluctant to have the surgery for the implant. Just looking into all options.

The ADHEAR was just approved by Medel yesterday. That might be a good option for him. I have a video coming out this weekend on that product. Check out the Medel website.

Dear Sir, Please advise me of the suitable solution to my wife , 69 y, hearing loss problem which is briefly - 1.She has mixed hearing loss for both ears. 2.Weakness of the hearing nerves for both ears ,the loss is in the range of 90% . 3.Has had two operations for the right& left mastoid bones due to infections. 4.We understand that the bone conducting solution is most suitable because of the repeated infections and the need for proper ear ventilation as doctors advise. i.e. she suffers from both hearing loss and on- off infections. 5.Here in Baghdad, doctors have advised to use the BAHA ,but such operation is not available in Iraq , while the Cochlear implant is an alterative solution, but not good enough due to on-off infection. 6.Preasently ,she is using the normal air conducting ( behind the ear ) hearing aids for both ears alternatively in order to allow for good ventilation due to usage of ear moulds , to avoid and/or decrease possible infection. I,thank you in advance and hope to help for suitble solution. With Regards Anwar A. Dawood Baghdad - Iraq 5.

Hi May. It's been about 15 years since I've been in Baghdad. Hope things are well there. I'm afraid that a BAHA would likely be her best option based on the limited info you provided.

Doctor Cliff, AuD Thank you for your response and kind advise. For proper assessment for my case, please inform me of the information needed from me to send it to you for this purpose. In the meantime, I would be thankful to you if you inform me of the followings regarded BAHA... ..Is it available in Iraq ? if not, which is the nearest place to Iraq. ..Is there any side effect during and after its implantation? ..Does it need any follow up after the implementation. ..A rough idea about the cost ( covering BAHA and the implant operation ) . ..Any other info for us which you consider its vital to know and/or to be aware of. Thanking you for your time and care. Yours May .

I am a babyboomer that has been recommended for Phonak Paradise hearing aid for first time. I have deep voice and most appreciate that I speak slowly and articulate; I find it hard to understand soft spoken people. Having sensitive skin and ears, I prefer NOT to have any hearing aids in the ear canal; Paradise delicate hearing aids with a little wire and ear bud top does not impress me. My cats cats could easily break this flimsy hearing aid. IMHO, I would prefer bone conduction options (love design of Shockz openswim)...but local provider seems to not replying to my questions. Can't I choose a non ear option for hearing aids? I am about ready to voice my concerns with Medicare.

Medicare will not approve a bone conduction device unless you have a conductive/mixed loss or single sided deafness. You could purchase the Medel Adhear, but Paradise Hearing aids would outperform them significantly.
