To fix a rotator cuff tear we need to strengthen the muscles. People think they need rotator cuff stretches but that is incorrect.
Stretching a torn muscle will aggravate the tear and prevent it repairing.
If you have a rotator cuff injury or just want to fix shoulder pain this is for you. This is the basic strategy that can be applied to most rotator cuff tears. The subscapularis is a little different as it is an internal rotator.
But usually it's the infraspinatus, teres minor or the supraspinatus that is torn. All of these muscles are external rotators and this strategy works well to rehabilitate the shoulder injury and fix your shoulder pain.

How long did it take you to recover? I believe I just got a tear a couple of days ago and it’s killing my workouts in the gym 😢

@@JoePlaya87 6 weeks with good rest and a sling on my shoulder and cold water being applied, the swell decreased even though it's still shows a little .........I was back to my home workout routine :)

@@JoePlaya87 muscle tears will heal within 6 weeks if you do the correct rehab and as long as it's not a full thickness tear.

@@UnityGymOnline Hi bud, looking for help. The MRT said frayed supraspinatus tendon, so its not torn. Got it since i did heavy lateral raises in the gym with dumbbells... THis was in october 2023. I stopped with shoulderworkout but did a bit rowing for back. The doctor said that my shoulder is turned slightly forward and created a impingment syndrome, U got any tips? Can i use the tips in the video anyways? Thank u!

Thanks for your advice, I tore my rotator cuff back in June , up until yesterday It felt better but I tweaked it yesterday at work, gonna start doing band exercises to start strengthening the torn muscles. Thank you.

Thank you. Momma and Deddy both had issues with theirs in the past, now I’m dealing with it and WANT TO KEEP WORKING but don’t wanna get hurt. This is just what I needed. You’re healing it without “sitting out for weeks.” Cause the stretching DID HURT just as bad even after it felt better a day later from rest. That saved me from making it worse. Gonna try these.

@@wndrwmn72a44 you do both, always. But it's not a straight forward answer. Everyone want's to know the "do this and it will fix it" solution. But rehabbing any injury is about progressively overloading the injured joint. That means by increasing range of motion and load on the muscle. This process should happen pretty quickly. If you're following an effective rehab program, what ever you're doing now, you'll only be doing for 2-4 weeks before you need more challenging exercises. This is why we write programs :)

@@UnityGymOnline to be honest I’m not quite sure? unlike my last injury I haven’t got a diagnosis to go off of so I’m still researching things. All I know is the front of my shoulder pops, cracks, and grinds, and sometimes hurts on bench, and almost always hurts during shoulder work, so I’m looking to take a step back and fix it

Ive been dealing wuth a shouder problem since December and it has only gotten worse. I think it is because i stretch the muscle and i try to use it like nothing is wrong. I'm going to just rest it and follow this. Question, should i stop doing these exercises if i feel pain while doing them?

Well, the idea is that there is always something you can do. So if an exercise causes pain, you shouldn't just rest. We want to find an alternative exercise that doesn't cause pain. Pulled my right cuff. Was thinking external rotation rehab. Sitting down and everyday motions already internally rotate the shoulders.

Rather than doing rotator isolation movements, aren't normal compound shoulder exercises such as pullups, bent over rows, face pulls, etc sufficient to strengthen the rotator cuff muscles.

No, none of those strengthen the external rotators of the rotator cuff. You need to train instability

Not really. These exercises are to train the inner unit of shoukder stabilisers. The superficial outer unit of muscles are what make big shoulders

@swift_3392 it's just not how you train. If you want to strengthen the rotator cuff, use these exercises. Want to increase muscle mass in the shoulders, do lateral raises, shoulder press, pull ups, rows etc.

Sooo I've taken time off from the gym cause the first day I go back in 4 years I had pain in my rotator cuff. Been doing legs and core workouts since. Are you telling me I still need to work out but just lighter weights? Basically I can raise my arm over my head fine enough but it's very stiff and slightly painful to put my arm into an 90° L shape.

Yes, you should definitely still be training your upper body, but the key is to manage the load effectively. Using lighter weights is one way to do this, but you can also manipulate your range of motion and choose specific exercises that won't exacerbate your issue. This is where a well-thought-out, progressive program comes into play. Progressive programming is crucial for rehabbing an injury like yours. So don't shy away from training; just make smarter choices about how you train. 👌

Hey man, I’ve been dealing with this rotator cuff for the last six months. I’m able to do all those movements practically fine I’m trying to figure out if I should just start strengthening it more like should I just start doing more chest press and up in the way, I don’t know if my shoulders weak, but I can do all those movements perfectly fine with a good range of motion, but I still have pain.

I have a bunch of clicking/ crunching in my shoulder when i rotate it, but i have a habit of constantly clicking it. Is this bad for the healing process? Should I stop clicking it and leave it alone

Obviously, it's a hard question to answer. But how long should I wait before loading that arm until it heals? My pain is pretty bad, but with some of these exercises, it's reduced. I hope I'm not out of the gym for months! *edit. I just read creators' replies to another thread. 6 weeks!!! 😢 Hopefully mine isn't as bad but I'm thinking no arm work for at least 3-4weeks...

you should start loading it straight away. If the thought of a strength workout makes you cringe in pain, remember, there are many loading variables we coaches use to effectively align the workout difficulty with your starting and current capabilities. Then it is our job to guide you through progressive overload, where we increase the challenge as your body adapts and evolves. Such variables include, but are not limited to: Weight: How heavy the load is that you use to stimulate muscles. Volume: The total sets and reps in a workout, and the amount of workouts in a week. Tempo: How fast you move the weight up and down, or pause between directions. Range of Movement: The depth of range that you move the weight. Contraction Type: Static holds, eccentric and concentric muscle contractions. Recovery: Both between exercise sets and between workouts.

I tried the movements you recommend here but with super low weight (1-2kg). I felt like a bit of a goose, but I'd prefer to heal and get back to 100% Thank you for your advice and great video!!! 👍

Great question! Actually, you shouldn't stretch before strength training, especially when dealing with a rotator cuff tear. I do street lifting/street workout at high level and I just injured my rotator cuff on a grind of dumbells chest press. My shoulder gave out and cracked. I have no pain it just feels weird. I don't know if i can keep training or no

I fixed a labrum tear in both my shoulders and these exercises are part of the process. But there's more to it than this. We nees to strengthen the prime movers, scap stabilsers and rotator cuff in thr pain free range, then increase range as sympoms improve.

Not all rotator cuff tears are the same - make sure you know where your tear is. A subscapularis tear requires internal rotation strengthening not external.

@arthurhowe9902 OK, there are four rotator cuff muscles. But muscle tears don't re open. It's more likely you're dealing with pain memory. The nervous system remembers the injury and creates pain sensation even when the injury is healed. We need to train a range if motion that only just pushes into where you feel pain. Then we gradually increase that range until you regain full range, then we increase weight. This is a basic explanation of progressive overload when returning to weights after a rotator cuff tear

Could be a lot of things, torn rotator cuff shoulder impingement, labrum tear ... but the treatment is the same. I would focus on what to do rather than what it is. Yes absolutely it WILL heal with the right approach. Passive hanging is not what I would put in the early phases of rehab, maybe phase 2 or 3

GOD NO you should NOT go to a surgeon for a 40% muscle tear. Surgery is an absolute last resort. Muscle tears heal very well without surgery. Have you tried a progressive strength training program? P.S. don't take anything I say as medical advice, I hold a certificate 4 and have 25 years industry experience, thats my credentials. and we're communicating via messages.

After 6 months of following a progressive periodised program under guidance, you would be back at close to 100% full function. What are you doing to get better? well muscle tears heal completely within 6 weeks, so we're not talking about a long time at all. And you keep ROM by doing full ROM strength training, which is after all one of the best ways to increase ROM :)

Rotator cuff don't heal itself even with exercises maybe it can help to strengthen other muscles to support your shoulders but the torn tendon is no way to heal itself it will be forever if you don't undergo surgery.

I appreciate that you told a few people in responses that you do not hold medical certificates. I think this video is slightly uninformed. People should do light band-work for several weeks, not weights and especially not presses of chest and shoulders. Give training advice… not medical advice that physical therapists/orthos should give regarding true rotator-cuff tears.

@@UnityGymOnline30.5 years in military, 5 years outside as firefighter/medic, had multiple shoulder injuries and was only commenting because people seemed to be asking you questions like you were a medical professional. To your credit you led responses that you weren’t. Not all rotator cuff injuries are alike and should utilize different rehab given by licensed PTs or Orthopedists. That’s all. No response necessary.

@c17mark thanks for clarifying. Allow me to clarify my education and experience. I am a licenced PT, though that is rather easy for anyone to become. I've been a coach for 25 years and attended more courses with industry leaders on injury rehab than I care to list here. When I had a SLAP tear in both shoulders, I had two world class sports physios working in my gym. We would workout together Monday to Friday, and I was able to have my rehab programs closely monitored by them. That experience gave me a level of knowledge in shoulder rehab that very few coaches in the world have. Not only have I completely fixed two traumatic shoulder injuries in myself, and I am now stronger and more capable than before the injuries. But I've also achieved the same results with hundreds of clients around the world. With all due respect, the advice I've given in this video and my comments is sound. However, you're advice is not. Saying people should only use Banded work and avoid weights and pressing is ignorant to effective shoulder rehab strategies. And to be clear, I only say that I'm not a medical professional to clarify that I am not qualified to diagnose injuries. However, when it comes to exercise prescription and writing effective rehab programs. I'm far more experienced than most medical professionals.

PT as in Personal Trainer, correct? Not Physical Therapist. I’m not saying avoid weights. I’m merely stating that your video is a hodge-podge of pushing/pulling/weights/rings etc and there is a definitive graded process to rehabilitation. It takes a bit of time to get back to weights and exercises you showed. Perhaps rework the video.

It also has an embedded link to a video showing 7 rotator cuff exercises going from least demanding to most demanding using progressive overload. Anyone willing to learn from my channel and videos gets a tonne of value and well grounded knowledge from someone with skin in the game. To fix a rotator cuff tear, the first thing you need after confirming that it is torn with an MRI, is to get it repaired surgically... All of these strengthening exercises are great, and are an essential part of your physical therapy after your surgery is done but the surgery is pretty much the first step none of this is going to do anything for your rotator cuff in the absence of surgery, I'm sure this is what the narrator must have mad but he seems to have neglected the surgery part.

incorrect. Surgery is a last resort and usually only necessary in the case of a full thickness tear. Saying you should get surgery for any rotator cuff tear is terrible advice. A lot of sports physios won't even send you for an MRI unless you don't respond well to exercises like these.

Because physio is too expensive for most people to go more than a few times. The guidance you get from a good program will fix an issue like that easily.
