I had to deal with a Manifold Exhaust Leak on a 2014 Majestic 23a Ford E-350 5.4 Triton V8. I was able to tell by just hearing a the ticking noise upon starting the engine, however upon further basic visual inspection I could see a missing nut and bolt. This surely would be the cause. There have been times I have seen manifold exhaust cracks that caused an exhaust leak - like in the Nissan Armada's and Nissan Titan Trucks (I know from experience of owning both).
I am mentioning my Nissan Titan and Armada to bring up an alternative fix - change to headers. Because the Nissan Titan and Armada are prone to manifold cracks, I swapped the manifolds to headers and the problem was forever gone. No more manifolds issues. BUT in this case, The 2014 23a Majestic Ford E-350 V8 5.4 Triton the manifolds are NOT prone to cracking, just some bolts snapped over time which is decently normal wear and tear. So I didn't resort to swapping to headers in this case.
Since Manifold bolt extraction is difficult, especially on this RV with 240,000KMS, I brought it to a Heavy Duty Auto Shop near by and they charged me $1500 parts and labor in the end. They mentioned the bolts were annoying to get out and took them 4 hours. I replied with 'I know' and paid up. In the end there were 2 bolts that needed extraction and fixing before the new one can be installed.
I want to note that the RV was driving fine, just a quite a bit more on the louder side. The noise does get a lot lower when the manifolds heat up after a drive, but still noisy. ALSO, the important piece, not all will experience this. I have seen the same RVs with over 300,000Kms that experienced zero manifold issues.
Something to be aware and to get fixed when it appears. Nothing to over think or stress about. I hope you enjoyed the video.
