Great question. I believe you can set the priority within the infotainment but I am not 100% on that. Most vehicles you can so I would assume.

I had exact same question. Just got a new 2024 RX 450H+. Actually, The "primary" Bluetooth device is linked to the Profile page. No matter how many times you forget and re-add the device from the “Manage BT Devices” screen, it will always re-add as "primary" until you disassociate the wrong phone on the user Profile. In case anyone wondering, You have to (1) forget the entire Bluetooth device on that Manage BT Device page. Then (2) navigate out to the Profile Page and delete the device there and link to the other device that you want to make the “Primary” Phone. IF you have multiple profiles (I.e., different Lexus accounts on Lexus App), each one can be associated with a different phone device, different Face ID, different Digital Key. Hope that helps.

Make sure the volume is up on your phone. The audio level can be controlled separately via your phone for different bluetooth devices.
