Hack: in lieu of 4-2-1 rule, IF the patient weighs MORE than 20kg, just add 40 to their weight in kg to get their maintenance fluids in mL/hr! i.e. 80kg patient gets 80+40=120 mL/hr

4-2-1 rule Calculate pt kg 4cc first 10kg---> 2cc per 10kg there after---1cc per hour per 60kg=120 80kg pt.

4-2-1 rule shortcut: maintenance fluids = 60 + (wt. in kg - 20). If a patient is 100kg that would simply be 60 + 80. That way you don’t have to do (4 x 10) + (2 x 10) + (1 x (wt-20))

what kind of note are you referring to. A progress note?

A progress note should be in SOAP format (Subjective, Objective, Assessment, Plan). You do HPI, then ROS, then Vitals, then Physical, then Assessment, then Plan
