You can add them literally anywhere you want to draw attention to. An interesting neckline, the top of a slit, the wrist on a one sleeve dress, a shoulder, the waist, on a headband, over a bit of gathering, etc. are all good options. For guys, I'm really liking adding more linear pieces to the breast pocket on jackets. That's a bit fresher than brooch as substitute for boutonniere.
