Great on how to handle when pt refuses E.D. despite possible AKI, like that well-explained approach for a competent patient. When would you have him follow up?

Absolutely! It's so tricky and it's really patient dependent, but in his case I recommended a few days up to a week, just to make sure he wasn't tipping in the other direction.

Hi, I love your videos. I have a question regarding your video on hyperglycemia management for George. Could you elaborate on how would you manage HPT and what meds would you switch him from Lisinopril? Maybe potassium-non-sparing diuretics?

Do you mean hypertension? I'm planning to review hypertension guidelines, it's actually a bit tricky in his case. I avoided ace-i and arb because of the relative hyperkalemia, but in that case I'd probably follow the JNC8 guidelines and collaborate with nephrology. Nephrology is really helpful with blood pressure management in renal disease, but they tend to love chlorthalidone (a thiazide) which can help with both hyperkalemia and edema.

can you describe what does the Diabetes Medication Workshop covers? do you go over doses? prices? what is more fitable for certain patients who are not able to afford some of the meds? thanks for the clarification great videos

It's a recorded, live 2-hour workshop, covering the algorithm of management for non-insulin medications according to the AACE 2020 Diabetes guidelines. Things like: What comes first, second, third, etc.; the profiles of each med choice, when to choose them; risks, benefits, side effects of each class; example patient cases to apply your learning; and, medication cheat sheets and downloads. It covers the foundations for medication management for patients with diabetes, and medication recommendations according to AACE Comprehensive Diabetes Management guidelines. Hey! For legal and safety reasons I can't comment on specific cases, but I always investigate elevated blood sugars following the algorithm I mentioned in the video!
