Our patient presents with a loose 4-unit bridge that is replacing his missing front two teeth. A month later his bridge breaks off, leaving him with 4 total missing front teeth. How can you fix your smile so that you don't keep losing more and more teeth? All on 4 still get food stuck between the fake gum and real gum, patient will require to visit dentist for cleaning removal of all on 4 basically patient can't take care of it themselves. Also some ppl doesn't like that fake gum, other times dentist will saw away your actual bones to make room for all on 4 gum. Then other times eventually your jaw bone changes, all on 4 will start showing space between your gum and the fake gum of all on 4.

Hi again. You make a great point. All-on-four sometimes trap very little food, sometimes trap a ton. If really varies. But anything dentists do traps food right? Dentures trap food, but they are easier to clean because they can be removed. I do have some all-on-4 patients who trap little food and some who trap a lot. However, none of them would have wanted dentures because of the food trap alone. Having permanent teeth is worth it even if there is some food trapped here or there. Also, the food trap isn't that big a deal since the fake prosthesis and implants do not really get cavities like natural teeth do. I usually recommend a Waterpik to clean around the all-on-4. The patients who have a harder time cleaning their teeth come to see me for cleanings a few times a year. The ones who do well, they never come back!

Thank you, this is the best explanation ever, I have the same front teeth missing, it was just one tooth but my bridge failed and it was suggested to get the 6 bridge but thank goodness I have discussed with my help and watching your video, I am opting for the partial denture flexible one.

Hi there. Glad you found my video helpful. You're correct, you need to be your own advocate when it comes to your teeth. Putting a 6-unit bridge on your teeth can be very risky. Unless you already have crowns on most of the front teeth, this is not a very attractive option. You have to shave all of these teeth down. If the bridge fails, you will have had lost all of your front teeth! I would say that dental implant is the best option, followed by partial dentures. Good luck with your dental treatment! I have a question about the front bridge, why is the first example you feel is best and the last the worst, my dentist whats to put large zirconia screws in the center of two front teeth he feels it's more solid. If so Do you feel I can separate these 2 & 2 ? for better flossing ... can they just lean on each other, and be separated in the center, what do you think?

Hi Aloha. It's always best to place two screws to support two crowns. However, there are a few occasions where you can use one implant screw to support two crowns. Here are the requirements for replacing two teeth with a single implant post: 1. Best reserved for replacing front teeth: Back teeth take lots of chewing pressure and forces. I always prefer to place as many screws as possible to support the bite forces in the back. In contrast, front teeth are used for cutting food. This means there is far less pressure on the front teeth. therefore, splinting two crowns on one implant is a viable option for front teeth. 2. Avoid the canines: Canine teeth are right at the curvature point of your jaws. You should never connect these teeth to any other teeth when possible. 3. Exclude heavy grinders and clenchers: If you're a heavy grinder, you can break your crown, or even destroy your dental implant. Therefore, it's always best to place individual implants to support each crown for these type of patients. This also applies to patients with a poor bite relationship, such as those suffering from a crossbite, sever underbite etc. So in summary, you can place two crowns on a single implant post for the front top teeth and front back teeth (aka, teeth #7, 8, 9, 10, 23, 24, 25 or 26) IF you are not a heavy grinder/clencher and don't have a severe orthodontic malocclusion.

Is it possible to have one implant for two adjacent teeth? Or is it only possible for front teeth that are not chewing surfaces?

You can place two crowns on a single implant post for missing front teeth. You can NOT do this for the molar teeth. The reason comes down to the strength of the implant compared to the size of the tooth crown itself. Front teeth are not that wide, plus they don't receive as much forces during chewing. As a result, you can place two crowns on one implant screw to save time and money. On the other hand, molar teeth are very large and they are under far more stress since these are your primary chewing teeth. As a result, you almost always have to place one screw to support one crown when it comes to the molar teeth.

I heard crown and root canal for front is okay cause it's a spot that doesn't face much force, but root canal for molars are just waiting for eventual failure. Hence for molars it's good to just invest in implant
