In his 2nd form, blood was leaking from his gills. In his 3rd form, he was vomiting blood, my guess is that his trying to use his atomic breath but it isn't evolved enough for the breath attack. Everytime he roars he is in pain, because he's a literal nuclear reactor. It's painful for him.

Its Because the 5th Form is able to counter whatever its opponent is. For Example you see Shin Godzillas 5th Form as a small human hybrid Its Because these small human hybrid forms can combat humans more effectively + they are an army of shin godzilla 5th form

5th form wasnt complete in the movie, and that was a part of his tail, it was basically the 4th form but with that on his tail but due to its death 5th form wasnt complete, so all thats left was that thing on its tail, but here its just the creature instead of it on his tail
