Going by the comments it appears majority of people completely missed the point of this video/experiment. I AM HAPPY with my donor. I KNOW I can do SMP, but I DO NOT want to. I'm not convinced it's a good idea to do permanent SMP in the long term, and skin fade doesn't suit my face shape anyways. I was trying to microneedline on my scars to see whether it will change anything. Call me a gunie pig, so YOU know what to expect without doing it yourself... I will add - It actually DID improve my scars, for the record. The camera doesn't catch it well, but scars do look a bit smaller due to eliminating curvature that's been there ever since my 1st transplant. My barber told me and showed me an example (you can even see it on the video). And to be honest even sitting in the chair, it was much harder to notice them in the mirror, than it was 6 months ago. That being said, it's a very minor improvement and likely longer needle needed for it to actually work properly. There's more than enough evidence on microneedling and scar tissue regeneration suggesting this is possible. However I will NOT risk damaging my donor with a longer needle, hence I sticked with 1.5mm.

Thank you for the video! Why don't you think SMP is a good idea for the donor in the long term? How would you face the scars issue if you have to shave off everything in the future (for progressive hair loss - hopefully it will not happen!)

This is pretty much the only reason (with my current mindset) I would consider SMP. We font know how smp will look long term after 5-10 years. There's already evidence it can change color, dots can expand, or simply dissappear (which isn't that bad tbh). I wouldn't be the one running around with blue/grey skin.

@@MrRolandas totally agree with your skepticism about SMP. It's also difficult to find reliable experiences for the long term. I can imagine, however, in order to just cover those small scars to not making them evident (in the case of having a shaved head), not much ink is required and therefore no big damage will be done. I hope you will keep that fancy hair life long tho! Thanks a lot for answering

You are right to be skeptical on SMP. 8 years after SMP mine turned blue and merged into one block instead of dots. I expect it would be even more unpredictable in scar tissue.

Scars are pretty visible but you went down to the skin so its expected. In my opinion the micro-needling did not do much if anything and is not worth the effort when you can just grow your hair in a bit and completely hide them. You have one of the best results of a hair transplant I have seen so my advice would be to just enjoy it at this point and do the basics to maintain such as the finasteride/minoxidil pill you are taking. I'm a long-time listener of TBT and have been watching you for years, so I want you to know I don't mean this in a bad way but it doesn't look any different. It's very noticeable unfortunately and you should go for SMP if it bothers you. Your transplant looks great, natural and you're a prime example of someone who fixed their hair the right way. Hope you can enjoy the benefits of the surgery for a long time. But the scars are here and you can cover them up by longer cuts, good news all in all.

I think you'd need to get some numbing cream and a single needle and stab away at those scars, drawing blood whilst doing so, to get rid of them. I have done this to scars on my body and 1 week later, after healing, the scars are gone completely. I am no doctor so don't take my advice to do this, but if you are ok with doing that - you could just test it on a 1 cm squared patch of scarring wait 1 or 2 weeks and I think you will see an obvious improvement once it has healed.

Yeah I'm sure this will work, BUT, the needle should be 3-4mm in length. This could potentially damage surrounding hair if not done carefully, so no, I wouldn't be trying this and hence I sticked with 1.5mm to see what's possible to achieve. Theoretically I'm sure it's possible though. I wonder if there are any doctors trying such thing in a surgical setting..

Not much to complain about when it comes to the results of your hair transplant because that is an 11 or out 10! As with any kind of hair transplant there will be scars whether it's a linear scar from an FUT or the dots from an FUE and just my opinion, your scars really don't look any different from the micro-needling than they looked before you started the micro-needling. That being said, when you do have your hair longer, even with a slight fade you cannot even notice anything was done, it's only when you shave the sides that you do notice

@@MrRolandas exactly, that helps a ton. With the micro-needling, if there is anything it did well, it was that it seems to have flattened the scars.

Scaring takes alot longer to improve . You need to try coconut oil and other scar treatments help soften the scars. Mederma works well with scars and stretch marks. I think personally of I was you keep a decent length you have a darker hair color and good thickness so It would cover well. Thanks for sharing

My suggestion after your 2 great hairtransplants would be try SMP in a premium clinic with ink that fades away after 4-6 years, so it won't be permanent in case you don't like it.

Hey Rolandas, great experiment thanks for this! What was your microneedling depth? I believe the extractions go around 5mm deep and if you had used only around 1.5mm needle depth, it may not be sufficient to break down the fibrotic tissue in these spots. Seeing from the video, it also looks like you have needled over your long hair too. So in fact, your practical needle length could actually be around 0.75-1mm which may explain these results.

Yes, you're right. I was thinking it might not be enough length. I was using 1 & 1.5mm length. Fue scars are in about 3-4mm depth (at least mine), so yeah. Bht if I'd use 3mm needle depth, even with a numbing cream, I could do more harm than good as it's too deep. This is why I decided to limit to 1.5mm

@@MrRolandas I agree, it’s not worth risking that deep especially in the home setting. Also, when they use microneedling for scars, the general protocol is using it somewhere around once every 2-3 months for the skin to complete the entire healing process which ends with new collagen production. The reason for the weekly protocol for mpb is that the secreted factors in the early stages of wound healing intersect with the hair growth factors. So we don’t need to wait for the collagen production stage to see the hair stimulation effect. There are upcoming treatments in the future that induce these deep scars with things like 3-4mm microneedles but apply a dissolvable polymer afterwards where the cells can grow on and fill the entire wound opening without scars. I still salute your effort. Just keep your hair slightly longer until those new treatments arrive. Cheers!

1st - I don't want to wear my hair skin faded, and just with #2 it looks excellent 2nd - it can/will change colour in the future to blue/grey which I don't believe will look good 3nd - individual dota can expand to the point all area will be covered with ink. I've seen such case

@@MrRolandas Sounds like you've seen results from a horrible artist lol. It seems a lot of artists associate SMP with tattooing which results in these god awful results. A proper SMP artist will use ink that fades rather than turning colour.

Yes, but as far as I know that's called trichopigmentation where ink fades as opposed to smp. And initially that guy was happy with his smp, but over the years dots expanded dramatically.

Try coco butter lotion, I know women use it to reduce stretch marks from pregnancy. Also you could go for a day in the hot Sun with some tanning lotion, give yourself a nice tan while enjoying yourself at the beach.

If I'd give myself a nice tan, this would be a total disaster for my scars, as they would be even more visible. Scar tissue doesn't get pigment through sun as it can't produce melanin. However the skin around the scars could and would become much darker, resulting in more obvious scars due to more contrast in between them. I doubt coco butter ot whatever would do anything to FUE scars as they're very deep (3-5mm), nothing like any regular scars and stretch marks.

Have you looked into silica gel? It's a medically-accepted treatment for surgical scars even though a lot of surgeons don't seem to use it for whatever reason.

@@sowaveysuyat bcz the doc was an asshole who didn't harvest my donor area symmetrically...he just went berserk to save time and it looks like shit when my hair is short. I've seen other donors done properly which look very symmetrical and can be used for further harvesting as well.

Same as i was thinking, dont think microneedling will ever do much to diffuse the white space, because they will not replace the folicle that was there, a tattoo to cover the scars partially seems to make sense here, only problem is down the road if your hair starts to grey (check your dad\mom) and you want to continue to shave it, the tat will still be black.

I just want to say, past days I've watched some of your older videos. Like from 2 years ago etc. Its helped me understand your journey better, the effort and time you put into KZfaq when it was hard, when you were bald/pre op, probably lower on confidence, waiting for the results. I don't know about you, but if I did yt, on a transplant journey I know people would be talking crap about me, and when you have low subs/views it can be hard, ofc when you build a community, then they got your back, and a nice head of hair helps. The change is dramatic to now, you are so different to then! In a good way, not saying old rolandos wasn't good haha. Do you feel your life has gone in a direction different to how it was going/would have potentially gone if you didn't get a transplant? Mentally, emotionally, physically etc

I'm definitely in a better mental state now vs before. Keeping comb over in one place was nightmare. Of course it changed me in some ways, gave me more confidence etc, but I wouldn't say I had a "turnaround" in life as I wasn't really "down" because of hair.

To be honest I don't see any differense but also I don't see any scars in the first place. It's just that you don't have hair in those areas just like your childhood scar. The difference in color I think is just the contrast between the color of your scalp and the color of the hairs. Only SMP will correct this but I see no reason since it's barely noticeable. In general great hair man and thanks for being a guinea pig :p

Your donor doesn’t look bad bro. Not ideal, but not bad by any means. I’ve always thought that Acell should be used post-operatively, as it’s been FDA approved for wound healing. Not sure why surgeons don’t implement this post-operatively.

A-cell wouldn't help in my case as I have hypertrophic scars. In order to prevent then from forming I'd need to use silicon for wound healing for MONTHS non stop after the surgery. Safe to say it's impossible to do it. I think microneedling helped a bit to flatten them, but not much. Nonetheless I'm happy with my overall outcome.

You've responded well to microneedling. I doubt you have a keloid scar as resurfacing over them risks making them worse: it has responded by flattening after treatment. Deeper microneedling helps with surgical scars, but will need more numbing cream. You look experienced enough to go slightly deeper.

Yeah I don't have keloid scarring, it's "something like". KEloids would be a game over for me hehe. I won't be personally riskin donor health with longer needle. Not worth it in my opinion.

@@MrRolandas The longer the needle depth, the more you'll need numbing cream and the more it bleeds; it can increase the risk of infection (just telling it how it is). It then obviously means you can't do it as often. Skin clinics are likely to not put you off: no, it "doesn't" hurt! Skin is thinner on the head, so you'll know not to go too deep and rely completely on the instructions. It's surprising how much the head can take as I've had a PRP on the scalp twice without numbing cream!

Roland In order to improve your scalp scaring go for scalp micropigmentation . That will resolve the entire situation for sure. Good luck.

You could do micro pigmentation in the donor area to cover the tiny scars.

Sun exposure would maybe also explain why it's whiter? Comparison with your face and neck, having less sun exposure due to hair covering would lead to whiter skin. Just a theory. Or if I remember well scars doesn't absorb sun rays? Also, haircut like this is far better that a shaved head, I always wondering myself if I would rather prefer to transplant everything on top in case one day I would be too far gone.

actually whiter skin on the donor would help, as there would be less difference between scar tissue and the skin. And yes, fibrotic tissue doesn't get pigment, so direct sun exposure would make the things even worse.

thank you for doing this test, ive heard that scars can get darker with tanning which can increase melanin in the scar tissue so thats another possible strategy but no ones probably tried for fue scars

actually scar fibrotic tissue can't produce melanin, so it wouldn't work. Actually it would probably get worse, as skin around the scars will get darker, thus making scars even more obvious.

@@hairtransplantchannelpeople with more melanin heal from scars better. I’ve seen a lot of black Americans with zero to very little scars from FUE. It’s amazing what melanin can do. As for the fair skinned people I’m sorry🙏🏽

@@ricoa8926 this is factually incorrect. Melanin has nothing to do with scar formation, physiologically speaking, and is merely what gives skin it's colour. Nothing more. If you've seen cases where scarring is not visible, either the hair was not cut short enough, or the clinic used the right tool and the patient was a good healer. Statistically, those with more melanin actually have a higher propensity for overactive collagen formation which leads to hypertrophic scarring and even keloid scarring. This is medical fact.

Theres no difference in the pics. Looks like the micro needling didn't do jack unfortunately. However, your results from the transplant are well worth the scarring. In your case it made a night and day difference.

They look the same the same. As time goes on scars diminish anyway. Microneedling traumatized the area and could make the scars thicker.. I saw on KZfaq, about a year ago, about a medicine that could be injected into the donor area before FUE. Sounds too good to be true, but I don't know They said that it not only reduced scaring, but a new hair grew in the extraction spot. I don't know if it is true.

Your doner area looks decent to me. Yes the scars are visible, but they are evenly distributed. Looks fine in my opinon.

What was the needles' depth? It is advised to use 1-2mm for scars but may damage the follicles, who knows.

I did with 1.5mm. Follicle roots are as deep as 3-4mm, not not enough length to damage them

You don’t need to do anything from here on out. All you need to do I just keep a low-fade haircut. The one I you currently have is a high-fade haircut. All problems resolved by just a low fade. 👍🏼

Kun 93 are you on oral Minoxidil or topical solution? I'm seriously considering oral Minoxidil instead of the liquid though ( never tried both). I have been on oral finasteride for 2 months, also thinking to switch to duta but probably not now....

@@benchabanehocine7809 No , i ve been on foam minox for years ( maybe 3 years pretty constantly with it) and i ve tried oral minox from some sketchy farms from india ,but i stopped using them because they didnt do shit to me ,and im also on dut for close to a year ,no side effects at all ,and im thinking to stop using minox ,because i got 4k grafts transplanted to my top area without having any native hair left ,and i dont think i ll need to use minox again ..i ll see in the future

It generally should last lifetime, unless donor miniaturisation will start to happen at some point in the future which can happen to anyone (although rare). My donor looks really strong as of right now + I am on finasteride, so I should be fine.

A good follow up would be to show just how short or how long the side hair needs to be to hide these scars.I reckon you are already going to do this as your haircut grows back.In my opinion the scars ,they look nasty.I do however think your transplanted areas are fantastic.

Im going to turkey in September for my first transplant, i have diffuse thinning and my folicle units are mainly 1,2’s and some 3’s unfortunately but the actual individual strands are quite thick, i will be asking the surgeon to maximise the extraction amount as i have decided that after i will micropigmentation tattoo the scars as i like to wear my hair short, have you ever considered tattooing the scars to blend it more? Thanks

Of course. This hair will behave in exact same way it was behaving when was growing in your donor. You can cut it, pluck it, shave it, colour it, whatever you want. It will keep growing as normal.

No change or something so small that it is not worth the hassle of the microneedling. It was worth a try though. I love Dr. Ferreira's work but I just can't help but think his harvesting technique has something to do with the appearance of the scars. At Eugenix for example I've never ever seen a case that has scars left like yours. That all said, your actual result is one of the best I've ever seen in my life.

You're basing this from an only case you seen. Not a good idea. Check other patients from Dr. Ferreira. You will notice I'm the only unlucky here (just like Dr. Ferreira said) lol 0.8mm punch size is used by pretty much every doctor in the industry. Very rare if anyone is going any lower than that.

I have heard that iodine is good for healing scarring. perhaps give it a try? Also you can try to use Tretinoin. That is used to regenerate wrinkly and discoloured skin. Maybe also worth a try?

Shouldn't a reputable surgeon do a test procedure where they extract 1 or 2 punches and let them heal for a couple of months to see if the patient has abnormal scarring response so an informed decision can be made before doing a full session?

I think only 0.0001% of HT customers would even want to do such thing. Firstly because good doctors have very long waiting lists. To get actual surgery done would take ages + many "check ups" by the doctor would simply make such business model a disaster.

It seems more ore less before and after is very similar, even if probably by live the situation is different. I don't realize why micropigmentation should be done so close to the surgery, would be not dangerous for donor area? Regarding your new look, I presume you got 0 mm in total area, so just if you dedcided to cut your hair by 2 mm the white doots would be invisible. In first line You seem very good, how was the density in cm2 there? I honestly I don't see a see trough line even you wear gel on it

Yes with just a bit of length I can hide scars no problem. It is indeed 0 mm skin fade in here. I had about 50-60 grafts/cm2 in the frontal 1/3 and moving to thr mid scalp its something like 40 grafts/cm2

yes, just a possibility of smp dots expanding and becoming one big piece of ink is stopping me. Besides, my scars are invisible with just guard #2 (or even less), so i don't even need it. Why risk it?
