Thanks, Justin, for another terrific episode. Having been diagnosed in the pre-internet, pre-CGM, pre-lots of things age, I really enjoy and appreciate your content. 😊 I started using iAPS at the same time you did, and my experience has been somewhat similar. Like you and Pete, I've found this system to be transformative for nighttimes. During the day, I'm still quite hands-on - for my own mental wellbeing, I try to stay within a range of 4.5 - 7.5 mmol - but I find that iAPS in combination with other things I was already doing (eating low-carb, announcing carbs, and pre-bolusing) has improved my daytime control noticeably. Re. the recent "issues" in the iAPS world, those are indeed unfortunate. Under the guidance of some folks with far more tech expertise than I have, I've upgraded to the latest version with no problems. Like Pete, I feel reasonably well-prepared to accept whatever risks there might be - as he points out, all DIY systems carry inherent risk - but I hope that the peer-review process will improve.

Hi Justin, just found your channel a few days ago and your content is amazing! I was diagnosed two years ago as a type 1 at 54! I am looking at going to my first pump and I am trying to decide between Loop and iAPS. Thank you for the detailed information. Keep up the great work!

Justin, here is an FYI, Pete used the name 640G when mentioning a previous Medtronic pump. He being in London the UK calls it 640G where as here in the US it’s called 630G don’t know why that is but as a nerd about T1D I wanted to give you a heads up on the terminology.

I have been so looking forward to this episode, and it did not disappoint! You did a terrific job of sharing the positive and negatives to iAPS to make an informed decision about switching to iAPS, and great tips for current iAPS users. This awesome video deserves a million views!

One feature we have in Dev version is to be able to use iOS shortcuts. When you exercise it can start a profile automatic, when you get to a place it can start/cancel a profile. When your alarm clock rings it can start/cancel a profile. With iOS automation there is a hole new world of opportunities. For now it is in the Dev version but will be available in the soon future for main (normal users).

I’m conflicted about all this. I used IAPs for 4 months. To say ive lost confidence is an understatement. 2 groups of people that can’t agree for multiple reasons. I will say unless you prepared to do a lot of work and fine tune IAPs may not be right. For example if you’re a beginner and you accept some of the wild bolus recommendations IAPs gives you, well let’s just say it won’t end well. I love tech and I love DIY and part of the reason I used loop and IAPs was to stick it to the main commercial companies I think that’s an element in all of us as we are not waiting. But at the same time I know if my tslim tells me you need 4 units I won’t and up passed out due to a hypo. That bolus calculator in IAPs I know it’s under work but some of the wild recommendations it gives you is crazy, for example oh it says 13 units next loop cycle it’s telling you 0. So if you did that 13 which Many newbies would do you will end up in big trouble. Commercial systems aren’t great and I’d love to see more rapid movement but it’s the testing and rigour that slows this up, but it also keeps us safe, I’d rather run a bit higher with no hypos as opposed to running on the low side to brag about my a1c. All these things have pros and cons but for most people I think it’s safer to stick to a known quantity. I’m back to my tandem and while I hate it also for many reasons I know it’s safe it’s got good support and backing and my time in range is also good for me. Great video though and good for us all to know what options. Great channel Justin

I agree! The commercial pumps are designed for those that eat a lot of fast acting carbs and for those that take a lot of insulin! I tried Omnipod 5 but only lasted 3 weeks. It wasn’t giving me any basal and I would end up over 200 which doesn’t work for me.

I was using iAPS for several weeks and have moved onto a fork of iAPS with autoISF, which has the capability to close the loop almost entirely it seems, however, it has a steep learning curve. I've forked it, and am looking into either contributing back to the iAPS autoISF branch (if they are on board with the idea) or maintaining my own fork that aims to greatly improve upon the ease of use for new users by creating a single unified dial in the settings to tweak the aggressiveness of the automated dosing algorithms up or down relative to an individual's needs, and hiding a lot of the in app dials behind a toggle that the users can choose to unlock, or not.

Thanks for your videos! Always educational. I've been T1 going on 51 years and finally started Loop today. It’s a little scary because I'm so in control and have been for 8 yrs now. iAPS seems interesting but I think I prefer to wait until all the bugs are tested and solved before I jump. My A1C's have been 5.5 for tge last 6 years and letting AI control everything is challenging. My time in range is normally 85% . Fingers crossed automation will finally work for me even tho I don’t eat carbs. It took 3 months to really learn how to bolus. I wonder if iAPS will solve this issue? Thanks!

I’ve been diabetic since 8 I’m now 47… I love my Tandem and Dexcom G7 but I always interested in good tech … I’m not sure I’d like the DIY TYPE

This sounds impossible if you are over 50. I wish there was a place to get assistance with this process--in person preferably. Online stuff is a slow process and more confusion is introduced.

Justin, based on your experience, Would you recommend iAPS (using a Dexcom G6 and Omnipod 5) instead of using just the Omnipod 5 algorithm with the Dexcom CGM? (what would YOU do for better control) I'm T1D for 27 years.

You can’t use Omnipod 5 and a diy system. You’d need the dash

Just a note - iAPS and Loop use Omnipod Dash pods. Omnipod 5 is already a closed system and not able to be used. We just switched our daughter from Loop to iAPS last night. We started Looping in 2019. I would probably suggest using Loop before going to iAPS since it's more complex. You have to be comfortable building the app as well as knowing how to test and change your settings. However, I don't know you nor your experience so it may be something to consider.

As Jason said these IAPS and the like systems have not been tested by any health agency. Guessing for IAPS to get certified it would be "playing catch up" as well in the certifying process.

I have no issues with commercial pumps. They are life savers compared to when I was dx at age 8 to no being 47 my A1c is a 6.1 … not sure why you’re bashing good diabetes management. The only issue I have is when endocrinologist who instead of having newly dx patients with type 1 be taught how to control their sugars with injections and diet and not eating just eating everything they throw a lump at them … and that in the end leaves those kids with no education or knowledge on hold to handle it when they have highs or lows or the pump isn’t working…..
