That's air in the stomach. Horses with ulcers develop air pockets in their stomachs. It's a last ditch defense against their stomach acid eating threw the lining of their stomach. Horses constantly produce stomach acid, they don't stop, unlike us humans who only produce stomach acid when we eat. Really Horses can only go 3-4 hours without having some sort of forage going threw them before they are at increased risk of ulcers. Many of these farms put hay in a feeder or on the ground and let the horse gorge until they have no hay left. This can lead not only to ulcers but metabolic issues as well. The sudden insulin spike from gorge feeding isn't good. Hay nets, slow feed hay nets or just slow hay feeders in general help with slowing down the horse and extending how long their hay lasts.

@@phyllisboone8006 really you could say this horse is tense from pain. "Sheath" noises are where the stomach is, and there is no air pocket potential in the urinary track.
