I agree with everything that you said and always listen to your advice. I've had cataract surgery and had multi focal lens implanted. I wish that I had been subscribed to your content prior to my surgery as I have learned a lot that I did not know prior to my surgery.

Dr. Lee, I watched many of your videos preparing for my cataract surgery and they helped immensely. Your last point about expectations is important. While one eye they nailed it to 20/20 distance, the other was 20/25. My optometrist said some people would be incensed. But coming from poor vision my entire adulthood I am happy so far with the outcome (I went with Vivity lens in both eyes). I easily see distance and computer screen and about 12-18" away but need readers which is exactly what I was hoping for. The only other issue are dry eyes but my optometrist hopes that goes away after the eye drop regimen is completed. The only other recommendations I would make is to research the doctors peforming the surgery, the surgery location and also the lens choices. I would not recommend watching the KZfaq surgery videos as this unnecessarily freaked me out prior to my first eye, when my surgery went very smooth.

Recently had surgery with LAL implants. Knew about 1-3 adjustments per lens, but didn't know 2 lock-ins afterwards required. First adjustment 4 weeks after surgery, each one week apart, then lock-ins a week apart. In hindsight, I wish I had selected multi-focal lens, and be done with it. Still have to wear reading glasses for small print. My surgeon was great and the laser surgery was flawless! For me, I just made the wrong selection of lenses.

Hi Dr Lee, firstly, I appreciate your time in making these videos, I have watched them all and they are so informative and helpful. I need to have cataract surgery soon. My question is would you still have chosen the LAL if you had not had LASIK? Thanks Dr Lee!

I really appreicate your kind words! If I didn't have LASIK, then I would probably still get the light-adjustable lens since I do like the idea of a near-perfect outcome. I'd have to think about it some more though because the light-adjustable lens does require many visits and a lot of time for the adjustments. Hope that helps!

Dr. Lee, thank you for the video. I do have cataracts and am in the process of finding a surgeon. I have a question I hope you help provide some insight on. At my last consultation with a surgeon, he strongly recommended getting lipiflow done before surgery. I have not been able to find anything indicating that this procedure results in a better outcome, but this is what they have said. What are your thoughts on whether or not getting lipiflow done before surgery helps. Thank you.

Thank you for your videos, they are very informative. I am 48 years old and from what my doctor says, I have astigmatisms in both eyes "that aren't bad at all" , and my progressive glasses RX is mild. I have issues wearing my glasses as I end up getting severe headaches, so I end up not wearing them. I can function fine without them but looking for a solution to my blurry vision. I also am unable to sit through lasik or prk that was suggested. I would require to be put to sleep for any surgery with my eyes because I have extreme anxiety with anything to do with my eyes. Hence contacts are a no go as well. I have no cataracts, but am planning on having an IOL surgery in both eyes to help with my vision.(They are willing to put my to sleep with deep sedation). It feels a little extreme for a person that has no cataracts and mild vision issues, but I feel like I have attempted everything I can to help with my vision. I do think clear crisp vision would improve my quality of life. Have you come across a similar situation in your practice? Would you suggest a different pathway for my situation?

After seeing 3 ophthalmolgists over a couple years the third one was on the same page as my optometrist's recommendation: monofocal set for intermediate distance, and I had settled on that myself after thinking about the pros and cons and my lifestyle (lots of computer, hobbies). The third Dr also specified the post-op refraction she'd be aiming the two eyes for: -3 for the worse eye (currently -17 from subjective refraction exam) and -2 for the less worse eye (-11 from subjective refraction exam). I have read -2 diopter is a model for intermediate distance but do you know why she would make the worse eye one diopter more? She will also be using the ORA machine during surgery to confirm lenses needed for the -3, -2 combination. I know this Dr. and the multi-practice she's in are very experienced, and the clinic they use is highly rated, I just have difficulty in understanding her answers which seem to be along the lines of "what is your concern, your eyesight will be so much better". I know that, I just want to know the details and the why of her decisions.

I need bi-lateral cataract surgery soon. They want to do the nerve block for both surgeries, but I would much rather have the numbing drops instead. Are there any reasons they can’t do the numbing drops? As far as keeping your eye still during surgery, don’t they use a speculum to help secure the eye? Thank you.

Thanks for your videos! I'm going in for my surgery consult in a few weeks. My goals are good distance vision for playing tennis and decent intermediate vision for the computer (kind of like your hypothetical goals). I've kind of narrowed it down to Vivity lenses because they seem to cover those two distances well. What do you think? I'm 74 yo and I've never worn contacts or had lasik surgery.

Thanks for sharing! An EDOF lens like Vivity should help you accomplish those goals. I would make sure you discuss this with your surgeon to make sure you are a good candidate for this lens. Best of luck!

Can I ask - there are so many developments in the medical field: what would you expect the next leap forward on cataract surgery to be in say 10 years time? How would you expect future cataract surgery to differ from that available in 2023?

Great advice and very informative video, thank you ! May I ask how do you manage patients with ADHD ? Does IV sedation work well ? I'm afraid of moving or not understanding the instructions in time. However, the idea of being under general anesthesia makes me more anxious.

I recently had a standard monofocal IOL implanted in left eye during cataract surgery. My doctor didn’t tell me about the different types of lenses nor offer me a choice. After op my vision isn’t crisp steadily, can be randomly blur. I asked dr why and was told I have mild astigmatism, which was not told to me before op. For my other cataract eye, i am considering the Vivity IQ IOL with or without toric. Is a monofocal on one eye and vivity edof for the other eye a good combination? Or better to have the monofocal IQ for other eye? Is 2 monofocal to correct far distance better or a combination with an EdOF better. I am awaiting second op on other eye and was told I need reading glasses of 2.5 for both eyes for now. Apart from cataract my eyes are healthy for a 67yo

I had lower expectations of my cataract surgery to improve my vision with the lens implants by 1/2 of prescription glasses so going from +6.25 in the left lens to around a +3.00 and the same on the right lens. Well I should have expected much more but I have always been a realist and it was totally beyond what I expect to the point I don't need glasses for distance and mid range and except for reading and that is a +2.25. I was not a candidate for the premium lens according to my doctor and got the implants that insurance pays for. I did get drop less surgery I followed all the pre and post op instructions to the letter all I have to do was three days prior do lid washes three times a day. The instructions said twice a day but it felt so good doing the lid washes I just did the extra three days prior. Then two weeks after the operation on my final eye I started using the lid washes as I like them so much but only once a day.

Great advice. I have mono vision as I am 20/400 in one eye (not correctable due to childhood muscle surgeries). I’m am so nervous just thinking about cataract surgery at some point. I have a +5.25 sph correction in my dominant eye. Any advice on new lens correction for me?

Hi! It's hard to say without examing your eyes and discussing your goals for surgery. I would say that you should wait until you really need the surgery before you move forward, especially on your good eye. Also, when the time comes, make sure you think about your goals for surgery and discuss with your eye care provider. Best of luck!

I have cataract evaluation with my doctor. I have high myopia, -8 , -8.5 ., wear contact 4 hour a day. He had done auto refraction machine ( see the balloon inside the machine ) , topography and the OCT optical coherent tomography. He review the reports said my retina and macular is healthy. He recommended me to have Light adjustable lens implants, and ready to schedule me to do the surgery. My question is I did not have 30 days not wearing contact lens , and doctor just use auto refraction machine to get me vision power. , not the manifest refraction machine to measure my vision. . Is it Ok to do the surgery and have the Light Adjusted Lens implant ?

If you did not have Lasik surgery would you still go for LAL or Vivity. I have to decide between the two for cataracts and I never had surgery on my eyes. Big decision. One doctor wanted to go with LAL and then I got a second opinion and she wanted to go with the Vivity lens. So confusing.

I don't have AMD yet, but I have multiple risk factors for it. Is the LAL safe for me? Not loving the idea of directing uv light into my eye to adjust the lense.

Hi there, I usually don't recommend the light-adjustable lens for those with AMD since we don't know the long-term implications. If you have risk factors and you are interested in the lens, I would discuss this with your surgeon. I hope this helps!

If I get monofocal lens 4 near how far will I be able to see without glasses I stay in the house most of the time will I be able to see everything in the house refrigerator the sink be able to do dishes watch TV my computer laptop and tablet and all that kind of stuff and will I have to have glasses and just as soon as I go out the door

@@annshorey6819 I think you can see clearly beyond 1 meter, but you will need glasses at close and medium distances. If you choose EDOF lenses, this time you can see all distances from 50 cm onwards. You will only need to use glasses for up close (30-40 cm). You should still get detailed information from your doctor.
