Yeah he violated a lot of guidelines here and is putting this patient at risk. 1- he didn’t get a CT of the brain to rule out a bleed. 2- there’s no evidence from when her symptoms exactly started tPA has a 1-4.5 hour window 3- tPA cannot be given until the top blood pressure read is less than 185. Man’s a menace.

Personal stories: a) I knew a big, burley guy once. A little overweight, but mostly a solid rugby/football player in high school. He was diagnosed with high blood pressure. Refused to go on meds. Wanted to find an holistic method. Was a 60s hippy in 2012. Ended up having a catastrophic stroke. Visited him with a friend who had gone so school with him. I remember riding the elevator after seeing the guy; silence. Both of us in shock, that this outgoing, incredibly gregarious guy could no longer speak. b) My brother had cardiovascular issues. (A family thing.) Was on blood pressure meds. Ate extremely well. Ran. Was 65-going-on-50. Was walking to the mailbox at the end of the driveway, had an incident (massive stroke) and was dead before he hit the asphalt. Lesson? Sometimes you can do nothing right, sometimes you can do everything right and your days are still numbered. That's not a nice lesson, so how about this one: Love as much as you can while you're able. 'Dance like there's nobody watching?' How about 'Love like there's no tomorrow. Family, friends...and yourself.'

Urm norman bates was a psychopath with delusional hallucinations..... this character has some kind of autism or aspergers completely different. I would,d trust a doc like this any day because the one person I met who had aspergers was like a computer, he was amazing, remembered everything, seriously intelligent he just has trouble with social interactions and social emotions and expressions. I guess the only analogy I can think of it must be like living as the only human amongst androids..... the behaviour he must try to understand is foreign and not something easily adjusted to let alone understand and they dont understand my behaviour nor emotions either adding to communication difficulties except the raw data ie words spoken or written can be understood but the tone used, facial expressions can't be understood. So robots wouldn't understand sarcasm easily.....

Nooooooooo, DO NOT EVER GIVE TPA WITHOUT A CT SCAN. The CT isn't to locate a clot, it's to make sure that the stroke symptoms aren't from a bleed or other problem like a tumor, so you don't give someone tPA and then cause a fatal herniation or bleed. I know these shows are written for the drama, but this would never and SHOULD never happen. 🤦♀️

This would be like if while watching a show about fire fighters you saw one say to the other, while fighting a fire “we have to put this fire out!” And then started spraying the fire with gasoline 🔥 In the real world that patient could have died without the head CT, and it’s just an odd decision for a show about a “good doctor” to make, just for the drama of it all.
