Knowledge full video but only one question ma'am what to do in patient's after giving nerve block too patient's complaints of pain in cases of irreversible pulpitis. Complete anesthesia is achieved but when doing access opening patient complaints of sharp pain.

Failure to achieve anesthesia especially in irreversible pulpitis can be because of these reasons - 1) The conventional anesthetic nerve blocks do not always provide profound pulpal anesthesia and patients with preexisting hyperalgesia cannot tolerate any noxious input. 2) The nerves in the inflamed tissue have decreased excitability thresholds, so anesthesia becomes ineffective. 3) Sodium channels are increased in an inflamed pulp with irreversible pulpitis , so anesthesia doesn’t work. To treat this type of case, use supplemental techniques like intraosseous injections or periodontal ligament injections if the nerve block fails. 👍🏻
