Thanks for the easy to understand, informative video. What is the strongest compounded testosterone one can get? I have just started using TT 200MG/ML T cream 2 clicks (0.5ml) a day. Is it best to apply .25ml twice a day rather than .5ml once daily. Also do I require a script to get thru you and could you ship to Australia? Many thanks.
@hormonespecialist3 жыл бұрын
Hi. My pleasure! Sorry for the delayed response. The 200mg/ml is about the strongest that can be compounded. It starts getting too pasty and thick at higher concentrations than that. Once a day application is fine for maintaining testosterone and it also allows you to space the application apart from when you have contact with another person, preventing transfer. We do require prescriptions, but cannot ship to Australia. Compounding pharmacies are abundant over in Oz though. I suggest you do some phone shopping for the best price.

as a RPh, is it better to apply the cream in the morning or the evening to simulate the body's natural release?
@hormonespecialist2 жыл бұрын
Hi Fred. The body naturally pulses the release of sex hormones so that receptors do not develop resistance. So morning OR evening is okay. It's best to make that decision based on when you are less likely to transfer your topically applied testosterone to your partner. The decision to apply hormones to the skin should be made by the owner of the skin! Now, having said that, testosterone drives libido and more so at higher levels, so if you want to apply it closer to the time of sexual activity, you'll probably like that. But give yourself 2-3 hours after applying it before rubbing that skin on somebody else's.

What about eod or ed injections
@hormonespecialist4 жыл бұрын
I presume by "eod" and "ed" you mean every other day and every day? I like that dosing schedule, AND it can be dosed sub-q with a shorter needle, so less painful. You use the same weekly dose (commonly around 1ml of the 200mg/ml testosterone cypionate) divided by the number of doses you give in a week. Be sure and get 3/10cc insulin syringes with 29 guage 1/2" needles. If you try and use the short needles, they are smaller around (31 guage, which will be like sucking honey through a straw) and won't work well. I have patients who do daily SQ injections. It just requires some patience pulling the oil up into the smaller syringes. You should check with your doctor if you want to administer your testosterone differently than the label directs. I'm just giving you options to discuss. -Pharmacist Layne

Hi, I have heard topical testosterone raises DHT levels significantly. Is this true, and should I be concerned of balding? Also thank you for the video
@hormonespecialist2 жыл бұрын
Hi! You're welcome. It is true that the topical testosterone route raises DHT levels higher than injectable, especially when applied in the genital area. This is because there is a higher 5 alpha reductase enzyme level in the skin and genitals, which is what converts testosterone to DHT, but here is the hard truth. The likelihood of male baldness is most influenced by genetics. And those predisposed to it genetically can experience hair loss from even normal circulating levels of testosterone, so I think DHT concentrations are a bit of a wild goose chase. Yes, they bind with a higher affinity to androgen receptors than testosterone, but your receptors are likely already saturated, so we're back to genes. Thank your parents. I really don't like blocking DHT (with finasteride or dutasteride) because it's a party killer. DHT is a much more potent stimulator of libido that testosterone and you don't want to be without it. Besides, haven't you heard? Bald is sexy, man! I know, it's mostly the bald guys saying that.

I just started and my doctor has me on 150mg/ml cream and I apply .5 ml twice a day for a total of 150 mg. @hormonespecialist2 жыл бұрын
Thanks for the comment. In my 20 years doing this, I find most guys settle on twice a week. Once a week allows it to drop into too much of a trough and can be a little too high of a peak precipitating acne. All you have to do is break through that threshold once or twice to trigger acne. I find that women can get away with once a week--probably because it's a much smaller dose.

Im curious as to why androgel is applied to the shoulders and stomach but (as people mentioned in comments) you can apply the creams scrotally. Thanks for the video im trying to switch from injection to topical because of headaches and androgel is way too expensive.
@hormonespecialist2 жыл бұрын
Androgel is a commercially made transdermal testosterone preparation in what we call a carbomer gel, which uses alcohol (about 67%) as the delivery vehicle. Alcohol generally does not sit well on tender thin skin like that of the scrotum. It can burn, especially if it gets near the anus. This is why they have you avoid those areas. Additionally, because of it's relatively weak concentration of T, you have to apply 5-10 or more grams of gel to get therapeutic blood levels. That's kind of a lot to smear all over your boys, so they direct you to larger surface areas (no judgment whatsoever there with the adjective). Compounding pharmacies generally concentrate the testosterone in their formulations requiring less volume to deliver the same payload, and use bases compatible with sensitive skin. The scrotum is ideal because it is out of the way (avoids contact transference to others) and its very absorptive. We get good levels when applying there. And... as you mentioned. Compounded testosterone is MUCH cheaper. Compounded testosterone is often covered by insurance. Ask your compounding pharmacy. Thanks for your question, Mr. Chip.

Injections have been the gold standard for so long that people are afraid to try anything new. Yes, you do have to administer every day but in reality, it's not too bad, apply, let it dry for 30 seconds, go on your way. You just can't hit the gym or have sex for 5 hours to avoid transference. The reality of it is that all recent studies have found the creams to be just as effective as the injections so it's a matter of personal preference.
