As a plastic surgeon, Dr. Karam frequently gets asked his opinion on Sculptra, and whether or not it is a quality treatment to undergo. Sculptra has been around for just over 20 years, and was originally used to treat HIV patients who were experiencing fat loss from their treatments and medication. Sculptra proved it could increase volume in the face significantly, and was soon introduced to the world of cosmetic medicine. Sculptra is not like a filler or fat transfer, though, but is more comparable to a collagen fertilizer. When Sculptra is injected, it feeds the fibroblast cells, which stimulates collagen production and creates the effect of more volume in the face! While this may sound appealing, and Sculptra does have its pros, Dr. Karam also has a couple of cons that he wants you to be aware of!

Dr. Karam, you did an amazing job describing the pros and cons of Sculptra. I am a dermatologist who was involved in the regulatory approval of Sculptra back in 2008. One aspect that could add more precision would be to cover the various dilution volumes with Sculptra. A larger volume dilution such as 8 ml is much safer to prevent the formation of nodules and/or papules. I am a big supporter of Sculptra as long as it’s injected by an expert. In fact I favor it over fat injections because the latter are unpredictable and almost always over injected in order to account for resorption, which can easily form lumps when patients gain weight in the future. You are also very correct about the risk of hard fibrous tissue following Sculptra. I find that risk to be lower with larger volume dilution as well. Would love to have a conversation with you sometime because I am also considering a vertical face lift on myself.

@@KT-ml1sy primarily my temples and some around the face. From what I understand it takes a series of them to see results. It’s been over a year since I finished. Unfortunately it just didn’t work for me.

I am not a surgeon however I have researched countless products and I will say that I do procedures on myself. The PLA family of products are phenomenal. The newer dilutions are much safer and when used properly, ensuring that one is consistently swirling the particles preventing them from clumping inside the syringe, the risk of nodules are incredibly low, especially when used in areas of lower movement. Thank you for being part of the team to offer such an incredible product to patients. The over filler effect is achieved from hyaluronic acid products which are extremely hydrophilic and for that reason should only be used strategically and I believe under treatment should be done.

@@ZaraQadir97 nah it’s been almost a year and my treatment completed a long time ago. It’s definitely not still producing collagen. You have to know when to stop and find a good injector.

Sculptra injections leave you with a very nice, subtle re-volumization of the face, because the resulting collagen/scar tissue formation does not attract water like filler. So definitely no overfilled look with Sculptra. I’ve had it for over ten years. Minor complications were small nodules in the chin area, which disappear on their own after roughly 12 months. The eyes are a no-go zone though.

I love Sculptra. Ive used Filler 3xs but not happy that I had to spend money every 3 months. With sculptra, Ive gone 3 sessions so far 6 weeks apart. And now my dermatologist said, I dont need to go back for 3 months and then 6-12 months after that. And Im happy that it wakes up the collagen on my face. I use microneedling for under the eyes and the whole face. Retinols 2-3xs a week. Im 63 and people think Im in my 40's. I wont complain.

What timing! Just went yesterday to consult for mini-lift w/fillers. Nurse said Restalyne is a must - I said "no" but she argued her case & said the lift would fail if filler not used. One thing about Sculptra is that a takes a while to show the results. It may take over 6 months to show results. I had it years ago and I feel like the results are still there. Unlike fillers there is not immediate gratification. But I feel it was worth it.

Thank you so much for this explanation of how Sculptra should be utilized. This video could not have been posted at a better time! I have lost an extreme amount of weight and am happy with the look of my face due to fat loss. However, I do have laxity in the skin on the lower part of my cheeks causing wrinkles on both sides of my face next to my mouth. I’ve been trying to decide what I think is the best way to address this issue. I really don’t want filler since I’m happy with the volume. Instead, I think I need the skin lifted. This video helped me determine that my decision to have a lower face lift is the best option for me. So… Thank you, Dr. Karam!

I really enjoyed this educational and informative video on Sculptra Dr. Karam. I've heard Sculptra talked about on other channels but never as you have explained it. It's wonderful to know the history about Sculptra and what it was approved to be used for. If I had to have anything done it would be a Vertical Restore by You Dr. Karam. Im glad you are telling patients about the benefit of fat transfer. I had two separrate injections 7 months apart 9 years ago. I still have a good amount of fat in my cheeks and face with no discernable hollowing.where it was before. For clarity I did have a full SMAS facelift before I did the fat transfer. Separately within a year I also had a minor lip lift. Very glad I didn't do these other procedures which don't appear equivalent to the natural use of our own fat. My facelift was done in Prague, and my fat transfers here in the US. Thank you for all your videos. Laura

I had 3 sculptra treatments to improve the cheek hollow. Have to say, the results on me are amazing. It changed or have been changing the skin texture and make the skin glowing and plumping. But I agree, there is chance on leaving the “beads” around the eye area. Any cosmetic treatment has it own pros and cons. Be aware of the risks and benefits before taking it.

Normally Sculptra shouldn't be used around the eyes, from what I have understood it is mostly injected in the lateral cheeks.

I can't say enough good things about Sculptra! I'm 34, but due to severe weight loss, my face used to look gaunt and very unhealthy. Fat transfer was not an option, because I don't have enough of it. My dermatologist recommended Sculptra to me and I will forever be grateful to her. It restored the volume in my temples and preauricular area. And the quality of the skin! I don't remember my face looking so good even in my 20s. Great product, which, however, needs experienced hands. Highly, highly recommended!

Sculptra has lifted this 68-yr-old face. My face became very narrow from top to bottom and it has also widened the upper portion of my face in my outer cheekbone area. I'm extremely satisfied with the result.

You will be very happy with the results. When first doing sculptra it takes 3 appointments spaced 4-6 weeks apart. The rule of thumb is 1 vile for each decade of life, so at 70 you will most likely need 7 viles spread out over 3-4 months. It takes approximately 6 months when initially doing sculptra to see your full results. It is not for someone that doesn’t have patience. If you can be patient you will be very pleased with the results. It looks completely natural in every sense and because the improvement is slow & gradual virtually no one will realize you had anything done. People that see you every day will not notice the subtle improvements until one day when you just start getting compliments that you look good, refreshed & not tired anymore. You have to have yearly maintenance of 1-2 viles every 12-18 months to maintain your appearance. In the long run it’s far more economical than fillers every 6-12 months that seem to require more & more syringes as you continue to age. The expense can add up quickly. I’ve been doing sculptra for 4 years now and could not be happier. No more fillers for me!

Dr Karam, so well- and thoroughly explained! And consistent with my own experience way back in 2006. Thank you! If you do a follow up to this vid, I hope you'll illustrate specific before/after procedures, honing in on the areas where Sculptra works and doesn't work as you're describing it. My Sculptra procedure was in the naso-labial folds. It lasted two-years to the week. Natural looking and long lasting. Best of all, it's collagen, naturally produced in the body, so no adverse reactions in most cases...or so I've read. Cheers.

I have always embraced aging naturally, but with good skincare habits. It’s so important. I constantly get asked ( at 84) if I have had procedures. I credit my mothers good genetics and example of aging gracefully and naturally. I don’t criticize those who choose to have procedures, but for me, I would rather see what nature has in store for me. I do use your skin care regimen now, and I am loving the simplicity of it.

I love Sculptra and get it in my temples and pre-aricular areas. It hasn't made me puffy like previous Juverderm did. It really has firmed my face up well and made my dermis much thicker. Another positive is it has made my skin incredible smooth and it looks natural. The only downside for me is that it is not recommended for the marionette lines etc. You need patience with this product though because the results for me were around the 3-4 month stage. Now I just get 1-2 vials every 12-18 months as maintenance.

Thank you Dr. Karam for this. Sculptra was something I’d just heard about maybe a yr or so ago & it sounded very interesting & perhaps something I’d be interested in. I liked the relatively natural aspect of it. However, like many, I am looking for something to firm up my face, especially along my jawline as well as the area around my lower mouth. Both of which drive me absolutely crazy! And while I had a lower facial lift 10 yrs ago now, as kind of a preventative measure, I’ve been truly wanting something to tackle these two problem areas along w/the volume loss in my lips, & my neck/under chin sag. Which I’m realizing really do call for a more radical lift. In other words, a deep plane vertical lift!! Something else I learned from your channel & am grateful for the information. I was way too worried about looking unnatural & pulled when I got my facelift & should’ve listened to the surgeon. Instead I got what I asked for & honestly, it was pretty much a waste of time & money for the amount of discomfort involved. I may have looked refreshed, but it didn’t last more than a year. No one ever guessed that I’d had a lift. Which was great on one hand, & perhaps it forestalled the need for a more aggressive lift, but on the other…. The areas which bother me so much now, are the very same areas which propelled me to get the lift 10 yrs ago. Live & learn. At least now after watching this vid, I have confirmed that Sculptra really isn’t something I want in order to address my problem zones. So thank you.

@@dramirkaram Wouldn't a subtle amount of Sculptra work okay in the Nasolabial folds? I just don't know that I like the idea of fillers not dissolving 100% and potentially still being partly there in 10 years time. As an aside, one aspect that no-one talks about with any type of fillers is whether things like microdermabrasion or massage (where the face gets pulled about) can move them around and or squash/ diminish them?

Thank you everyone who shared their positive results from Sculptra and sharing the areas where you got it. I narrowed down my next treatment to sculptra after researching and then after this video was a little unsure but glad I read your experiences in the comments. Not to diminish Dr AmirKaram AT ALL! Will definitely be watching more of his videos and love the before after pics of his special procedure- forgot name as I’m tying this- vertical lift?

I had Sculptra for a few years until my nurse told me about Lanluma for the face. I know it hasn't been approved yet in the U.S but I can say that for me, I prefer Lanluma. I only ever had a filler around ten years ago and once was enough for me. I have a small face and I am very slim and post menopausal. I'm happy with myself but my face lost so much natural collagen that I was looking gaunt. Lanluma has literally restored that look, without looking overdone. No one notices it other than to say I look well. Never had any problems with any nodules, either with Sculptra or with Lanluma. 100% love it!

Thanks for your perspective, I've been looking for the best treatment for me. Lanluma is meant to last longer than Sculptra (according to the internet) but it seems a lot more expensive to get done though.
