Is spinal stenosis a serious condition?
Spinal stenosis is deadly serious. Lumbar spinal stenosis, such as Jon presented with, always gets worse. Not to scare you but left untreated spinal stenosis will cause paralysis of the legs, uncontrollable pain while standing and walking, numbness in the legs, inability to control your urine, and sexual dysfunction. Yikes.
In the old times before laminectomy surgery the result of spinal stenosis was that to end up paralyzed, in bed, incontinent of urine and with sexual dysfunction until you died of a urinary tract infection. Today we are not going to let that happen to you.
The truth is you cannot tell if you have spinal stenosis based just on how it feels. Clogging of the arteries also causes pain down your legs while walking. Bad veins can make your legs feel heavy. And herniated discs or bone spurs can cause pain down your leg. You may suspect you have spinal stenosis due to numbness, heaviness, or tingling in your feet when you walk that is relieved by bending over; however, you cannot feel spinal stenosis. You know if you have spinal stenosis by getting an MRI.
This is a sagittal cut through Jon’s low back taken with magnetic resonance imaging. In this cut the square blocks are the vertebral bones. The spinal fluid in the spinal canal is white. In the upper third of the picture look in the white spinal canal as you can appreciate “strings” running from top to bottom of the image. Those strings are the spinal nerve roots inside the spinal canal. About mid-way in the image, you see the white area narrow. This is spinal stenosis.
Spinal stenosis is caused by several factors. Discs form the floor of the spinal canal (left arrow), so a bulging disc means your floor is rising. The right arrow shows a ligament under the lamia bone. With age and wear ligament thickens. This means the roof is coming down. The spinal bones are also shifted slightly, trapping the spinal canal in between.
The term ‘stenosis’ refers to narrowing of the spinal canal which holds the spinal cord (cervical and thoracic). Think of the spinal canal as a room: the roof is the yellow ligament, the walls are the facet joints, and the floor is the disc space. Joints normally get bigger with age. (If you do not believe it, and you are over 40, look down at your knuckles.) The yellow ligament thickens with age because of wear and tear. And discs are famous for “bulging” (Herniated Disc). As a result of all these changes, with age and the roof coming down, the walls are moving and, and the floor is rising. It is not a surprise, then, that the spinal canal is getting smaller. Some people do not need to wait, they are born with a very narrow spinal canal. This condition is called congenital spinal stenosis. You know you have it if one of your parents and all your siblings have already had Laminectomy surgery. You can also develop narrowing of the spinal canal due to a shift in the vertebrae. The spine is a rigid ring. If one band moves forward on the other one, it creates a kink in between. This type of movement is called spondylolisthesis.
See the problem? Once it gets started, the processes of spinal narrowing can only get worse. Spinal stenosis is always progressive; it only gets worse. That means there is no stretching, exercise, injection, medicine, magic crème, or supplement that can help. Even sleeping in a pyramid with magnetic walls will not save you. We know because we have done studies.

I have severe spinal stenosis and it's such horrific pain 24/7. My whole spine is hell pain. I was diagnosed 3 years ago, and l am getting worse sadly. It's extremely debilitating and has turned my life upside down. I can't even sleep properly as the pain intensifies in bed and sitting or lying down. I used to be super fit and now l find it hard just to do daily living just in my home. It's a really scary serious medical condition.

I have had spinal stenosis for many years. I had a recent MRI scan which showed the stenosis was very bad. It is so serious now that I am losing the strength in my legs, arms and fingers. I have had several bad falls and I struggle with day-to-day activities. I am waiting to see yet another specialist, however, at 76 years old my age is against me. Waiting for an appointment to see a specialist can take many months with the NHS. I do have bladder issues and I have a leakage problem.

Not all surgeries are bad. I had severe lumbar stenosis at my L4-5 and it was pinching my spinal cord. I had fusion October 26 last year and my legs are no longer constantly going numb. Yeah, I still have it but it’s not all the time. No surgery is guaranteed to relieve your issues 100%, especially if you have degenerative disease of the entire spine. Having cervical stenosis isn’t fun either. Just take care of your body and realize we are not getting any younger, ladies and gentlemen

THIS HELPS ME.... The only thing that I found that helps my spinal stenosis is hanging upside down for even a minute or 2 on an inverter table (Which I purchased for just $99 at walmart). It seems to help a little more to get on facing the table then facing away from the table. The first time I did it, I sort of didn't like it. But, since then I do it several times a day and look forward to it. It's only temporary relief... But, that little relief is so nice! The relief lasts longer if you aren't active right after. If I'm having a bad day of back pain and get on this for a little bit, my pain is reduced dramatically!

I have stenosis, spondylolisthesis, spondylitis, well basically all the spondy ailments of L5 -S1. Have had it for over 20 years now. The last time I had a MRI I was at grade 2 and that was 9 years ago. I can't lay down to sleep, my hips, buttocks, legs, knees and feet are in constant pain. I know it's time for surgery, but I just don't want to be completely out of commission right now.

I understand your concern. But when you can't sleep and you're experiencing neurogenic claudication due to grade two or higher spondylolisthesis, it can indeed leave you feeling like you are already incapacitated. In such cases, lumbar fusion surgery can be a viable option to regain your mobility and alleviate the discomfort. If you're keen on avoiding an extended downtime, the solution is a minimally invasive spinal fusion procedure. I would recommend seeking out a spine surgeon in your vicinity who specializes in minimally invasive surgery. With this approach, you could potentially return to light activities within a week or two, or at most, three weeks.

Dr. - your sincerity in this video is greatly appreciated. May I ask please...my MRI report says I have severe right sided foraminal stenosis at L1-2. The surgeon I recently met with said, "It's not too bad," and scheduled me with a colleague in his practice for facet injections at the L1-2 and L4-5 where I had a hemilaminectomy 27 years ago. Now, an MRI I had 2.5 years ago showed no problem with L1-2. The current MRI report states that the disc is severely degenerated, there is severe right foraminal narrowing, modic changes in both vertebrae, endplate ebernation, large osteophyte complex, bilateral facet arthropathy, moderate thecal sac narrowing and right subarticular recess narrowing. The Dr. I met with said I'm not at risk for cauda equina syndrome. From the get go, before I met with him and had the MRI report in hand back in March, I was figuring a foraminotomy would be the way to go. I am concerned that there is some new paradigm at work in your field that might be putting me at risk. Please Dr. Lieberman...what say you? Thanks.

Newest xray says grade 2, degenerative disc disease, moderate stenosis...basically everything has gotten much worse. I cant sleep now, i cant walk more than 5 minutes now and then both legs are aching. @BestPracticeHealth
@BestPracticeHealth 5 ай бұрын
Amen! The terminology really is confusing. Stenosis means narrowing. But the term is not specific. You can have narrowing of the central spinal canal, the lateral recess, or even the frame itself. To make matters worse, each of these can occur alone, or in combination.

What are your views on decompression therapy such as IDD for stenosis? I have stenosis and a herniated disc so am going to fry this therapy as a surgery alternative
@BestPracticeHealth
@BestPracticeHealth 9 ай бұрын
Decompression therapy can be a valuable treatment option, but not for stenosis or disc disease. Decompression therapy, often used in the form of traction, is typically most effective for conditions related to facet-mediated low back pain. These conditions are often characterized by arthritis visible on X-ray and swollen joints as seen on MRI scans. However, decompression therapy, such as traction, is not suitable for herniated discs or stenosis. These conditions have different underlying causes and mechanisms of pain, which require alternative treatments. For herniated discs, more targeted approaches like physical therapy, epidural steroid injections, or in some cases, surgical interventions, may be considered. Stenosis, on the other hand, is laminectomy surgery to decompress the spinal canal, leading to compression of the spinal cord or nerve roots.

first responders are always the hardest to determine their physical readiness and fitness for duty, as the physical requirements of the job can be so crazy severe. I know for me, given the importance of a clear, mental state, and doing this vital work, if you wonder if your fit for duty, then you’re probably not. It would be better to find a non-physical way to finish your career, such as more of an administrative position. I have had really good experience with functional capacity evaluations. They do have norms for physical performance to compare for almost every job, certainly fireman.

The type of pain you’re describing doesn’t make sense, given the imaging findings in your comment. I think you need a better quarterback. Someone who can explain why you’re having pain, give you a solid diagnosis and then propose a sound treatment plan based on that diagnosis. Often, the best doctor to do that is a pain management physician. If you’re just getting plugged into some random protocol, and your pain management physician is not a thoughtful and thorough doctor, then look for one who has training in physiatry. Those types of doctors generally focus more on the cause of the pain, and rehabilitative treatments, rather than just doing injections to cover them up. That’s a generalization, that is certainly not always true, but I want to make sure you get the best chance of getting the best care get to the bottom of your current problem and solve it.

What is causing the stenosis? If it is a disc herniation, then that will slowly get better, and you can expect to get back to full function. Stenosis due to a narrowing of the spinal canal you were born with only gets worse as you age, and at 25 years old you have a lot of aging to do. If you have that kind of stenosis, then you should look into surgery sooner rather than later.

I am frustrated with my regular Dr. I have 20 years of stenosis….injections…fusion, laminectomy (which has helped) now my last mri shows bulging L5 on a nerve…I am waiting for my appointment with a great surgeon…but in the mean time..in pain, my Dr. will not give me medication ……. I am sleeping on a heating pad..my pain started in August…my appointment is November, the emergency department told me not to come back for pain shots.. the emergency Dr. addressed this with my Dr. about the pain…nothing…

Uh, yes! I'm frustrated just reading this. What is your doctor doing? (Although I do agree with the no medicine part. Narcotics are so dangerous!) It sounds like you have non-specific low back pain that could be radiculopathy, facet, or the waste basked "post laminectomy" pain. The next step is typically diagnostic medial branch block and epidurals to sort out the pain generator. You need a good pain management doctor. Do you have one? I recommend people who are double board certified and who do not use chronic narcotics.

Most of the time when I hear "my doctor said it's in my head" it ends up being either a misread MRI or the pain is coming from the facet joints which radiologists often do not read on their reports. Either way, hypnosis is not the answer.

I was recently diagnosed with lumbar and cerebral spinal stenosis with 6 herniated discs. I’m very afraid to have surgery because I’ve heard nothing but bad stories and results. :( I was diagnosed last year and I’m almost to the point where it’s impossible to walk a mile. It puts me out for the rest of the day and getting up to use the bathroom after that is very stressful and difficult. Some days, I can’t get out of bed or walk at all. There has been times when I’ve lost my bowels in the middle of the night, or if I bend too far down or squat, I will lose my bladder and go on myself. It has become extremely debilitating and I don’t have a lot of support from ‘friends’ or ‘family.’ I feel so alone and this pain is unbearable. 😢

You need surgery. You will only continue to worsen. I have spinal stenosis too and I occasionally have problems with sudden urge to urinate. I thought it was just a bladder issue now I'm very concerned it's caused by my stenosis. I've had small surgeries for sciatica that didn't work and ended up with a spinal fusion of L4 L5, L5 S1 in 2011 that worked. I remained pain free until 2019. I was diagnosed with spinal stenosis of L3 L4. My pain has worsened and I have that sudden urge to urinate causing me to run to the bathroom. I just had a lumbar epidural injection on the 15th of Nov. It began to help and I went a few days with much less pain but it's returning. I'm willing to have surgery. There's several non-invasive surgeries that potentially can be done, but it's different for each individual. I hope you seek out a qualified surgeon. Doing nothing is a guarantee you'll only get worse. Best wishes.

It is miserable when you genuinely have excruciating pain all over your body and have embarrassing moments. I have trained my mind to control the bowel movement and urine control. I used to leak but i would go sit quietly in a comfortable position and just block everything out of your mind and you'll be amazed at how strong the mind is. I'm sixty two years old and have been suffering with spinal stanosus for about ten years and unfortunately in South Africa the public hospitals are useless and never got a bed for me (The Johannesburg Hospital). I would like to put myself out there incase an experimental drug for Spinal stanosus that other people don't have to go through what we are going through now. Your family think your just lazy, I've never been one to be lazy as I was used long hours of work and my work was physically challenging, a tradesmen and Trade Training Instructor and trained two(2) apprentices at a time whilst having to run my own machine and had to be available to repear the other machines in case of breakdowns,I was in charge of my shift and had forteen apprentices and never had an apprentice that I trained , they are all managers or directors now. My apologies for telling my working life history.. Please talk to someone if you go into depression as try and change every negative into a positive. Let your mind be one with your body. I'd love DR. Caan from Johannesburg General Hospital to do the op for me as he is one of the best Nurolgists in South Africa and has a great success rate on spinal operations. Im 39 years old. I just had an mri on my spine because an xray showed moderate to severe degeneration disc height loss and facet hypertrophy. The MRI was ordered by a rheumatologist to clinically correlate it with the xray and blood results I recently got showing high levels of inflammation (c protein) and rhematic factor but he really didn't explain the results of this MRI or he didn't know how to. I'm being referred to a neurosurgeon- ok great (waiting on that) But in the meantime I'm dealing with increasing symptoms as far as my ability to walk sometimes with extreme leg heaviness my ability to pee, lift my kids...sometimes episodes of vertigo at night but that jas only happened 3 times. Overall this seems to get progressively worse as the day goes on and sometimes causes sciatica pain. Anyway my results state a grade 3 spinal canal stenosis with complete effacement of csf and distortion, slight T2 hyperintense signal possibly due to myelomacia or edema, along with grade 2 stenosis is other parts as well.as neural forminal stenoses. What does this essentially mean? Will surgery most likely be involved? Am I working against a clock at all with this? This whole process started3 months ago with the inflamamtion symptoms wrecking my body but that since has been managed with a 6 day course of prenisdone. As far as my medical history- I have none really- relatively healthy, no accidents, surgeries or even a broken bone and reasonably active into sports. Any help or explanation is appreciated. Thanks

Stretching and enhancing your range of motion is a fantastic approach, not only for spinal stenosis but for various other conditions too. Your proactive approach is commendable and will greatly benefit you as you age. May I share my concerns about your current treatment approach? I genuinely care about your well-being, and I'm not convinced about the care you're receiving. Spinal stenosis is progressive, and while managing symptoms is vital, it's equally crucial to monitor the underlying condition's progression. Regular MRI surveillance is essential to track the rate of deterioration. I have reservations about Gabapentin as a long-term solution for sciatica. It's a drug with significant side effects, impacting not just your physical health but also your cognitive and emotional well-being. If MRI confirms severe stenosis and you find it challenging to manage without Gabapentin daily, I strongly urge you to consider a Laminectomy surgery now. Waiting might only mean dealing with it in a less favorable state in the future. Your current age and health are factors to consider in making this decision.

Yes. Lack of range of motion of the thoracic spine due to DDD often leads to accelerated degeneration of the cervical spine. How old are you? Having both neck and thoracic DDD at a young age is a sign you have inherited lousy discs. (Did mom or dad have DDD?). People with weak discs need strong muscles to compensate. Consider investing in a core strengthening program for life, yoga or tai chi are the most popular.

I have this and I don't care its the most painful thing I have ever experienced in my life .but I was still able to get around with pain granted but few months ago I decided to go ahead with spinal simulation device implant after every type of pain injections decided to no longer work.i was so positive and upbeat about having this .the thought of having my pain even reduced was a big thing for me ..few weeks in I actually felt no pain what soever not a single it I thought my life was about to get better. Few weeks later pain returned worse than ever and I could only Stan for about 10 mins before the pain became to much .Well few months on iam now depressed. I can't stand for more than 2 to 3 mins before the pain comes and it's so so severe .but I feel no pain at all when I sit down .so iam now no longer mobile. My neck is now also hurting me and my legs and feet have swollen seriously to 4 times the size. I also seem to have a problems with urine .I can control it but it's leaks shall i say .my groin hurts my lower back and pain down both legs .I keep dropping things I have noticed .I have cried and begged for help and my gp keeps sayinging they will contact the surgeon who Did the implant .they call me but just say I need remapping of this device I still have 10 days left to wait for that appointment. I told doctors I don't know why. But no pain meds are touching I don't understand. Iam on everything and tried everything from antidepressants to steroids to antibiotics to pregabalin to tramadol .and at the moment 8mg of bupernorpine sublingual x4 a day and tapentadol and paracetamol it's not doing nothing. Not even a little . They think i want painkillers like they do with everyone. But I don't. I will take back how I was before .but could it be bupernorpine is stopping all my pain meds working ? Because they also use this drug to help people come off drugs. So iam thinking this could be the reason .but if so then 6 different doctors and surgeons should have known this by now. But instead they have left me in a inhuman degree of pain it's not right .I have pain down my arms . Too iam not asking for a cure I Need someone to help me .even for a day pain free .I was told after this implant someone would be at the end of a phone and would be there for me .but I have been left to defend for myself and its continues to get worse .the worst part is iam in the. UK. And shocked at the treatment of all these doctors the pain is so bad and if nothing is working it must mean I have compression of my spine but confused to why so many doctors would ignore how severe my pain is

I am so sorry for everything you are going through. I have a couple of thoughts. First, spinal cord stimulation is not good for stenosis pain. If you have stenosis, that will need to be dealt with before you can get better. Second, medications don't work and make it impossible to figure out what is happening. Find a doctor who can detox you. Once you are off meds get your imaging together and come on my show so we can discuss what you have and what will work. Where there is life there is hope. Left out a few things: 1. I had slippage between vertebrate causing great pain. PT strengthened my core and solved the problem. 2. I had bone spurs on the lamina. Surgery removed them and solved the problem. 3. The orthopedic surgeon wanted to remove the lamina and fuse the vertebrae. The neurosurgeon just wanted to drill out the bone spurs. I chose the neurosurgeon and that solved the problem. I hope you fully research your issues through doctors, friends and yes even the Internet.

I hear you. But spine surgery can indeed be very effective when it's the appropriate solution for the right patient with the right problem. It's a complex decision, often weighed carefully when other interventions haven't provided relief, especially in cases of severe pain or neurological deficits. While there are no guarantees and surgery isn't a perfect option, in many cases, it becomes the best available choice to improve a patient's quality of life.

ok. this hurts beyond compare, I have ask many of my friends to just run me over with their car, shoot me in the spine,. anything to stop the pain, and it keeps growing worse, seems by the day. exercise makes me hurt so much more, but if I sit still, I can hardly walk about 60 percent of the time, with outlooking like frankenstien on crack, IT HURTS SO MUCH. The Dr.s tell me I have so many problems, but yet they keep saying go exercise, it hurts so much to walk, more and more. does this pain follow you when you die...

I'm so sorry to hear that. No one should suffer like you describe. I hope you get help ASAP. How long has this been going on? Is the pain more in your back or your leg? Have you had an MRI or x-ray? The place to start depends on the answers to these questions. PS. The car and gun thing don't work.

This is insane. My doc thinks nothing is wrong. Have had debilitating leg pain for the last 4 months, mostly stuck in bed. Literally, my mind is blown after watching this. Also having urinary retention,. constipation, I've got 2 known disk bulges and multiple lumbar fractures,and MRI said mild to moderate spinal stenosis. Im 33. Funny how the states has more public knowledge on KZfaq than Canada. Ite pathetic

In my own experience the most common cause of debilitating leg pain without an obvious cause is a missed lateral herniated disc on the MRI. One thing you can do is to call your MRI center and request that the study be re-read by a radiologist who is board certified in neuroradiology.

Good advice. Most things heal themselves. However, there are a few things that just need to be fixed: complete ACL tear, complete rotator cuff tear, Bankhardt lesion, severe stenosis, and severe knee and hip arthritis. IMO.

you are one of the worst people who spoke about this matter. i have stenosis on C4 to C7 and its more than severe. painful yes, i am unable to do many things, but when you say the only way to beat it is to die before it gets worse is such insensitive and you should not be a medical practitioner by any given chance. stenosis has multi-factor origin either discs or joints or ..., but most of it if not causing symptoms should not be touched. if its symptomatic, then you consider non surgical options and only go to surgery when all other options are exhausted.
