Why would you internally rotate the tibia in terminal knee extension? The physiological movement pattern is EXTERNAL rotation with end range extension... To gain the last few degrees, you should distract, extend and mobilise in the opposite direction.

@@johanesterhuizen1842 100% yes! It's a technique I use as a physiotherapist to gain terminal knee extension. I would be careful with patients/clients with torn ACL, because the external rotation of the tibia + extension increase the tension of this ligament. Cheers! 👌

I'd also be curious to know what the purpose behind mobilizing the tib/fib through internal rotation is. Aside from building rotational tolerance, is there a mechanical advantage gained? If so, what cases would require this type of mobilization? I'm intrigued, just a bit unclear to me what the parameters of implementation are.

@@jumpguyty tibial internal rotation goes with knee flexion. Sometime, patients lack internal tibial rotation and feel a "pinching" sensation behind the knee and by restoring that physiological movement, it often makes things better. Not always, cause everyone is different, but it helps.

Would this mob be contraindicated for a 18-20 month out, reconstructed ACL, with a medial meniscus correction? If so, any other mobs you would use to gain more active flexion? Thanks!

Do you lack terminal knee extension? If no, I would not do it. To gain active flexion (which is the opposite movement of what the drill in the video is for...): 1) Stretch your quadriceps/hip flexor (3x1min stretch /day day at least) 2) Mobilise your knee in flexion (ex: quadruped position, try to slowly get your pelvis back, as if you wanted to sit on your heels, come back and repeat +++) 3) Strengthen your knee flexors (hamstrings mainly, gastrocnemius): leg curls, eccentric knee flexion like nordic hamstring curls/towel slides on the floor, knee flexion in glute bridge on swiss ball, etc.

Simon Doyon Thanks for your comments! I presume you represent the Ready State? We should be able to also use a “ gap and smash/mob” with a small towel with this level of ACL recovery? Yes/no?

Just want to know who's and what kind of opinion I getting from? And again, the mobilization I'm referring to belongs to The Ready State! which I thought I was responding to?
