Here you have a suggested format for recording the "Needs and expectations of interested parties". You can change the format to suit your requirements and also please do change the filled up material.

Thanks sir ! I have one doubt, we have one Annexure of objectives in which we change objective yearly now question is should we change issue and version number of Annexure or we can continue with same Annexure no. Thanks sir, Please replay.

Issue No. of the format will change if you are changing the format. If you are reviewing the filled up format and changing the contents then the review remarks will be put at the bottom along with the date of review with signatures of the competent authority.
