It's called Christ Mass Sex Dance, and I can't find it anywhere online. "Because it is so highly edited, James Tenney's 'Blue Suede' is so meaningfully conjunctive with the possibilities of cinema. no this is NOT the first sample. This is the first sample using a prerecorded source Elvis Presley's "Blue Suede Shoes". Pierre Schaeffer and Pierre Henry in 1948 have the honor of being the first to put the first sample on to vinyl

The word 'sample' directly implies digital capture - i.e. to convert an analogue sound source to digital samples (as in 'sampling rate'). I don't know if this is the first, but it was achieved via computer AFAIK so meets the definition. If we take the 'post hip-hop' definition of sample to just mean 'copy', there are countless examples pre-Schaeffer in the form of sound effect and novelty records.

It was created with tape. Digital sampling really got started in 1937 with the invention of pulse code modulation, but digital audio recording didn't start till the Japanese company Denon got it going in the 1960s. But digital sampling was later.
