at question 14, why isn't 'de' the correct answer? 'cause in the sentence it says that 'I often go to the library to read newspapers', which basically shows that the topic is reading at an exact location. isn't 'he' used when indicating a vague location/direction?

Because It's a sentence pattern, meaning the particles are fixed and can't be changed to で.

I have doubt in 13 th question, why we need to select option 3 (た form) instead of option 4

先週 means last week so you should be speaking in past tense and since the sentence isnt finished you can't use the ます form there because only plain forms can be in the middle of sentences

It comes down to it all needing to be one sentence. 食べなかった doesn't make sense here because there's nothing to connect the two halves of the sentence together. That would've made sense on its own if that was the only thing the whole sentence was saying - 'I didn't eat breakfast this morning'. 食べないで is the only choice that makes sense in the exam question because it's the only verb form that both a) exists (食べませんで doesn't exist) and b) sticks the two parts together through its special meaning at the end. ーないで is a verb ending that means 'without doing something'. So you end up with 'I went to work this morning without eating breakfast.'

That question is asking which verb form you put before 前に. It can only be 行く from the answers given to us because 前に always follows the dictionary form of the verb. Every day before I 'go' to school, I take the dog for a walk :)
