As far as who gets accepted into the vaults goes, it depends on the vault. I don't know how much you know about Fallout lore, but Vault Tek performed horrific experiments in most of the vaults. There's one that only had a single resident who was given puppets to entertain himself. Another one left the door cracked open on purpose, giving radiation poisoning to the residents. Another one had a bunch of addicts inside but I haven't run that quest in a while, so I forgot the details. There were control vaults where they DIDN'T experiment on people, but you'd be better off facing the bombs.

The folks who were let into the vaults varied. Some were control vaults, where it was literally just a shelter and nothing more. Most though were home to some experiments, ranging from innocuous to fucked up. It's a neat topic to delve into sometime!
