I think it honestly was meant to hint at ichigo actually being in a similar situation, with a humanoid spirit residing in him that functions at a capacity that most others don’t. That’s why Nemu is Mayuri’s lieutenant even though she doesn’t even have a zanpakto as far as I remember. All I could find are some loose mentions of a lieutenant exam and that Yachiru is the only lieutenant who did not take the exam. He had always been "Kenpachi of Zaraki" since he became captain. It was a title. Before that, he had nothing. He didn't know who he was, so his Zanpakuto didn't either. This is reflected during the flashback scene with Yachiru where she meets him for the first time and he states broadly that she has no name either. He names her Yachiru after Unohana, but that still wasn't her real name, just like Kenpachi wasn't his. When he defeats Unohana, he truly inherits the name "Kenpachi". He is undoubtedly the strongest shinigami. Isane says as much when he reveals that he killed her. She states "You truly inherited the name of Captain Unohana". @@BladeZlayer her shikai is called Sanpo Kenju. upon release two sword beasts appear alongside her. these two beasts seem to mimic Yachirus movements. ie; she swings her sword, the beasts swing their swords in the same way. the enemy doesn't appear to be able to see these sword beasts. each beast is larger than the one before it, the second beast, usually depicted on her right side, being the biggest. fella is pretty big. these guys also don't look the same. one on her left is a round fur covered dude. one on her right is a skeletal beast wearing furs. we don't know much else about these guys sadly.
