I think the reason why kokichi was startled by kaito and shuichi's response when he changed his appearance was because he was always judged and insulted before the killing game started so he's not used to people not being bothered when he changed his appearance into something what others would say scary or ugly.

On the other hand, Shuichi and Kaito don't judge anyone without reason, Kokichi actively collaborated to hinder and annoy others, he also judged and intimidated almost the entire class whenever he could. Just look at how they treat Gonta and compare him to Kokichi, one is a true saint and the other is a troublemaker.
