Ok, out of all the enemies to spawn endlessly, why the grandma? I don’t quite get the importance of her to the prologue.

Not sure, but she has been the source of a portal in the studio. Since she is a worshipper of the lake gods, maybe she got or holds special powers.

Granny is Walt's mother, aka the creator of Red and the Lakeview franchise as a whole. Red was forced into the real world due to Walt and his cult in order to fulfil a prophecy created by the Lake Gods to bring about an apocalypse. Our actions end up setting Red free from the cult where he can finally relax in his cabin and tell the story of how the actor who portrayed him (the dad in episode 1) ended up killing Walt, his creator. However... Walt's mother isn't having any of it and seeks to torment Red for going against Walt and the Lake Gods.

I think I finally get it. The epilogue is a dream. It would explain a whole lot. Why the grandma continuously spawns. Why everything is so weird in the epilogue. Why he’s shown waking up at the end of the epilogue.

It's one of the lake gods. In Lakeview Cabin Collection 1 and Lakeview Valley, they're present at the end of the games.
