In 1993 A Minuteman 1B Intercontinental Ballistic Missile was launched from Vandenberg Air Force Base (now Space Force) carrying a payload for the US Army. The mission is known as RSLP TDT-1 and it's believed that the payload was related to missile defence.
Seconds into flight the operations team observed the rocket turning East, rather than West. This would carry it inland, over US towns and cities.
The flight was terminated by the range safety officer, but not before a lot of shouting was heard over the command loops.
The community of Casmalia would be evacuated following the failure out of an abundance of caution.
This launch is sometimes referred to as the Casmalia Express

I think Mark's job was probably to scrub the launch.

On one of the first business trips with the CEO and owner of an aerospace co. I worked for he was taking this little pill before takeoff. I asked him what it was he said “Xanax” I get nervous when I fly”, I said yeah but we’re engineers we make this stuff. His response was….yeah but we make mistakes………..

I was at ground zero when that thing exploded, along with SP/CC and OPs Officer. Pieces of burning fuel and chunks of missile rained down on us, and we had no cover. Some of us crawled under vehicles. I admit I thought I was going to die that day. An MOS patrol saved the lives of 2 A/V troops who were filming the launch and got separated from their vehicle - he drove his SP SUV through flames to reach and rescue them. The next day Base Game Warden and I went out on ATVs to check out the burnt areas. We saw lots of burned wildlife that didn't escape the flames. I remember on-scene commander worried the fire might take out Casmalia!

The systems engineering professor at my university was working on the Minuteman program when this happened. He had a framed photo of the abort with the caption "Mama said there'd be days like this..."

This is how it feels to have a program rushed to production deployment

I like how the giant silo door need to hit sandbags during tests. At real silos it would just slide off and not be able to be closed again. They knew they wouldn't need the silos again if they were ever really launched.

Somehow this reminds me of the A-4 test after WW2 that ended in Mexico.

Sounds like a military drill to me. Back in the day when we were worried about rollovers due to IED strikes when going over everyone was supposed to yell out “Rollover! Rollover! Rollover!” Same for chemical weapon attacks, “gas! Gas! Gas!”. “Program Red!”
