I have been using Alpha-H Liquid Gold once a week and Tretinoin 0.05% the rest of the week nightly for a couple of years now. Best combo I've found for me and I'm not changing it. Hi from Australia

When I started using g 1% retinol about 6 years ago, I found that AHAs and BHAs aggravated my skin. Once I changed to tretinoin, I started using Paula's Chuce weekly AHA/BHA 10 minute wash off peel, but only in warm weather and only on the night I wasn't using tretinoin.

You could use a salicylic face wash despite being on Tret if your skin tolerates it. That is something I can't say, as it depends on your individual skin.

Hi Dr. anne, thank you very much for the explanation! I would appreciate it if you could answer a question for me: I am 30 years old with some expression lines on my forehead and neck. I used one tube of 0.025% and after my skin got used to it I immediately switched to a dose of 0.05%. It happened to me Only once with the high percentage I had a red burn irritation on my cheek and I immediately washed my face with soap and applied moisturizer and it went away after a few minutes. I believe it happened to me because I put marine hyaluronic acid and pycnogenal before that. My skin didn't react well to it with the tretinoin. Then when I continued applying the sandwich method with no other ingredients it didn't happen. I will soon finish the tube of the 0.05 as well. Someone who also uses tretinoin who is not a doctor told me I was making a mistake because I didn't let the low concentration do its thing (I only finished one tube of 0.025).He also told me that if I continue with the 0.05% I will have nowhere to increase it if I will need to in future (we don't have a 1% prescription in Israel the 0.05% is the highest) I would appreciate it if you could tell me if I really need Go back to 0.025? Or can I be calm and continue using 0.05 for life safely if i will not get irritation? Without worries of having too many wrinkles or the trettinoin will not working well... Another question - in the last week I reduced the days I use tretinoin to three to four days a week instead of every night because on the other nights I started using lactic/mandelic acid and salicylic acid (on separate nights not together on the same night) Am I doing it right? By the way, I want to compliment you that you and Dr. Dray are my favorite doctors on KZfaq! There are many doctors on KZfaq who really see that their goal is marketing products , but you are really not like them, you are honest and trustworthy. First of all: Thank you for your kind words, they really mean a lot to me. For your questions: It is perfectly fine for you to keep using the 0.05% if that agrees with your skin. If it had led to irritation, it would have been a good idea to go back to 0.025%, but you don't need to worry about increasing the strength with age. It is about consistency, not about "the stronger the better", you are perfectly fine using the concentration that agrees with your skin for the rest of your life. Will you get wrinkles? Yes. But you will get them with 0.025% as well as with 0.05%, and increasing the strength will not make them go away. No matter the concentration, it will slow down the process, but not stop it. The higher concentration gives results a little quicker, but over a period of 10+ years, it really doesn't matter much. The only thing were concentration plays a role is if you use it to treat adult acne, here you adapt the concentration based on the results you are seeing after several months of using it. As for reducing the amount of nights you apply it when introducing acids: It sounds like a good way to reduce the risk of irritation, see how your skin reacts and adapt it from there. I have been using 0.025% every other night for years now. That is all my skin can handle and I don't see a point in trying to increase frequency or concentration and risk irritation.

I've been using the Aveeno PHA cleanser in the morning, it's very gentle and I think maybe it helps my vitamin C serum absorb better to exfoliate first?

It surely helps to exfoliate regularly, but I don't think it need to be done right before applying the Vitamin C. If that is what works for you, then great though, keep doing it. PHAs are very mild, so easy to combine with other actives.

I have interest in adding AHAs to my routine more because of the collagen support rather than the exfoliation. However, I always hear that you should not to use AHA during the day, but at night I use tretinoin, every night. So, not sure how to add AHAs without giving up my nightly tretinoin :(

You can use AHAs during the day - the recommendation for using them at night comes from the fact that they make your skin more sensitive to sun. But that lasts more than 24 hours, so in the end it doesn't matter if you use them morning or night, make sure to use your SPF. Tretinoin needs to stay at night because here the molecule itself is sensitive to UV exposure.

I'm using benzoyl peroxide because of pimples that is hard and feels like something inside the skin, although not huge like other pimples type. I do use 3% adapalene gel 3/4 times a week. And benzoyl peroxide 5% for 2 times a week as a leave on short therapy by cleansing it off after 20/30mins. Do I need a separate salicylic acid cleanser as exfoliate? I'm just not sure after this one because BP and adapalene both has some exfoliating properties directly or indirectly (including cell turn off accelerations and killing off the P. becteria. But I found no research papers showing dead cell exfoliating results on those. So that's why I'm confused. Thank you. Btw this video needs to reach more. I wish it also included about benzoyl peroxide 😅.

Are you seeing a doctor with your treatment? If yes, this would be the best person to ask. If you get these products OTC, the best I can say is: It depends. Both have strong irritation potential, and adding a salicylic acid in the mix might be too much. I don't think there are any papers on the exfoliation of dead skin cells with this, but that doesn't necessarily pose a problem - if you don't fee you have a lot of texture and blocked pores, I personally wouldn't risk it.

@@DoctorAnne no I'm on OTC for now. Actually I'm using benzoyl peroxide and adapalene. Those clear out pimples. But then after one to two weeks those again appear. This has happened for a while. But those dries more quickly than before maybe because of active treatments. So I was wondering if I need something or not. Thank you very much for replying even after a rough day of work. People like you're helping us students indirectly. Many of us won't be able to afford going to a dermatologist.

I wonder most about, how do I know I would need exfoliant when also using a retinoid? I mean those of you that use tretinoin it has really strong exfolianting properties already so how is that still not enough thats what I don't get. I am for example on azaelic acid 15% and differin. My skin is all smoth so what there is to exfoliate? But also I am looking at getting rid of PIH and some googling says I should couple glycolic acid on off days when on an retinoid. Is it really beneficial when your skin is smooth already? I have normal to oily skin, olive complexion acne prone, PIH problems.

My first answer is always: Listen to your skin. If it is smooth and you don't feel like you need exfoliation, then don't. I only exfoliate to prevent clogged pores, which is what my skin needs in addition. Tret alone doesn't do it for me. And while exfoliation helps against hyperpigmentation, it is just one step in targeting it, so when you are already using Adapalene and Azelaic Acid, I am not sure extra exfoliation is really beneficial for you.

Hello, doctor! I have been using tretinoin 0.025% every night and azelaic acid 20% every morning for more than 4 weeks. I've noticed that my skin is improving, but there are still lots of whiteheads and blackheads in my nose area. In addition to that, I have some hyperpigmentation in certain spots on my face, and I also have some acne around my hairline. Is it okay if I add an AHA/BHA toner to my skincare routine to help get rid of these problems?

I would wait at least two weeks more, as the irritation with Tret often appears after the 6 weeks mark. In general 4+ weeks aren't a long time with the issues you describe and you should give it around 6 months to judge how it works. Both acne and pigmentation are incredibly stubborn. If you introduce an AHA/BHA toner, start slowly and increase, don't go in every night right from the start.

What if I will just use a soft cloth to exfoliate my skin when purging due to tretinoin? To remove the peeling skin? I will soak my face with hydrating oil first, and when it is easy to come off, I will exfoliate it using a very soft and wet cloth.What will happen to my skin? Will it get dark? Or pigmentation? I'm on my 4th week using Tretinoin .05% doctor.

Every skin is different, but even that gentle exfoliation can be too much for some. You can try how your skin reacts and if you feel it getting irritated, stop it. If you stop at the beginning, it isn't very likely you develop pigmentation from that.

Hi Doc, love this video. Im using Differin cream 1% every 3rd day as my skin is extremely sensitive. Should i be washing my face twice a day or only once? I have very oily skin. When i wake up its a total oil slick. But i heard only washing with water in the morning is ok when on a retinoid.

No rule fits everyone, so if you feel water isn't enough to get rid of the oil, try washing it with a gentle cleanser. The goal is to make your skin feel comfortable.

@DoctorAnne thanks Doc Anne. I been on Differin for 7 months now. My skin still gets flakey and some areas gets irrated. Im still getting cysts. Should I keep on it or maybe try switching to retin A at a low strength? I been on it before a long time ago but never stuck with it because I guess I wasn't using it correctly I.e not buffering it with moisturizer. However im just wondering if im one of those folks who can tolerate retinoids. Even 2% salicylic acid face washes irritate me. My skin is super sensitive. Any recommendations? I'm also acne prone.

@@mayamaawishe1731 Are you able to see a dermatologist? That is usually the best and most cost effective option. Retinol is usually not strong enough to treat acne, it is not a medication.

It will most likely not harm your skin, but the actives will lose their power, so you won't get the same results after it is open for too long. So yes, you can use it until you finish it, but the question is if you get the effect you are after.

@@DoctorAnne The concentration of retinol is quite high at 0.5%. Does this mean that retinol will have the effect of a 0.2-0.3% serum? (for me it wouldn't be a problem, now I use the serum 2 times a week, if theoretically the effect of the Retinol decreases, I can use the serum one day yes one no). Does what I'm saying make sense? 🙈 I had no problems or irritations

I'm not sure if Tretinoin is a perfect exfoliant I just remember I used it when I was a teen and it basically made me shed like a snake 🐍 then I found out that it was a snake haha 🤣 anyways my doctor told me that it was a normal thing that happens when you are using it. They ended up giving me this Adapalene gel that worked wonders for me. but I'm now sensitive to the sun ☀ so I'm bummed 🥲🥀

The retinoid irritation is real, but there are ways to avoid it. Adapalene is less irritating though, so often the better choice.

With both medications you should be using sunscreen, so yes both drugs will sensitize your skin, but that's one of the reasons why you should use sunscreen.:)
