MEDICAL ANIMATION TRANSCRIPT: Laminectomy is the surgical removal of part of the vertebral bone called the lamina. 24 vertebrae make up the vertebral column, or spinal column. Each vertebra has two laminae fused together to form the roof of the spinal canal. The laminae protect the spinal cord in the neck and back regions. In the lower back where the spinal cord ends, the laminae protect the large bundle of nerve roots called the cauda equina as well as the nerve roots exiting the spine. Nerve root compression can be caused by bony spurs from arthritis, a herniated disc compressing the nerve root, or less commonly, a bony tumor of the vertebra. A laminectomy relieves back and leg pain caused by this nerve root compression by removing all or part of the lamina. This immediately alleviates the pressure on the nerve root. If a herniated disc is present, your doctor will often perform a discectomy, which directly removes the source of the compression. A laminectomy can be done on a vertebrae in the lower back called the lumbar vertebrae or those in the neck region called cervical vertebrae. Microlaminectomy and microdiscectomy are procedures performed with the aid of a microscope. When you arrive at the hospital an intravenous line will be started, and a catheter may be placed in your bladder to drain your urine. Just before surgery, you will be given either general, or spinal anesthesia. Spinal anesthesia is injected into a sac that surround your spinal cord and numbs the area where the doctor will be working. With spinal anesthesia you will be awake during the procedure, but you have no feeling in the lower half of your body. A laminectomy takes about one to three hours. You will lie on your stomach, or on your side, for the duration of the procedure. Your surgeon will begin by making a small incision overlying your spine. Retractors are used to pull fat and muscles away from the lamina. Using a special bone drill, your surgeon will cut away the lamina in order to see in to the spinal canal and locate the source of the nerve compression. If a herniated disk is causing the compression, he or she will perform a discectomy by removing the protruding part of the disk. To complete the procedure, the deep tissues and incision in your back we'll be closed with stitches. After your surgery you'll be taken to the recovery room for monitoring. The nursing staff will regularly check vital signs and the strength of the leg on the side of your surgery, to be sure the nerve has not been severely injured. You will be encouraged to get up and walk as soon as you feel comfortable. If a catheter was inserted before your surgery, it will be removed before you leave the hospital. If you've had an uncomplicated discectomy, you will likely leave the hospital the next day. Laminectomies performed for other reasons may require longer hospital stays.

I am so happy you got lucky, and had great surgeons and assistants. Mine was-is a disaster. S1 to L4 5 1/2 hours, pain enough to cause vomiting, cold seats continues.

Four months ago I had anterior and posterior lumbar fusion. The pain pre-surgery is gone but have this horrible pain which from an MRI shows I need this procedure now. Glad to read most comments are positive regarding this procedure. Thanks!

I had this done 30 years ago. L4 and disc removal as well. I would have to say that mine was a total success. The doc said I was a textbook case. One thing you must do is change your life. No running, jumping or pushing on anything. No lifting over 50 lbs although I lift things I shouldn't. My recovery time was 4 weeks but as for as the debilitating pain in my left buttocks and down my left leg was gone the day of the surgery. Word up is that recovery times now can be as little as 2-3 days with the new techniques and surgery tools. For the questions below about a hole in your back I can't feel or notice any difference in my spine.

i had 3 of these done between 2002 and 2004. The amount of scar tissue present has caused constant pain in my lower back ever since. The deep pain in my legs now suggests I may need another procedure but i can only imagine how much worse my back will feel after a 4th. Also, a "small incision" turned into an 11" incision covering my entire lumbar spine on the 3rd surgery. This surgery is a life saver for many people, and if a disc ruptures it's a full on emergency and you'll be glad this is available, but the complications can be life changing. i was 15 in 2002, a lot of doors were closed after having this done.

@@baffled_hawk I do know bones can regrow and reform; it's basically how broken bones heal and limb-lengthening operations work. I however am not sure if bones are capable of growing back to its default form to fill in a gap that huge though.

I am seriously so nervous for mine but I'm up watching the videos and reading your comments and it's giving me hope because I have no quality of life with this intense pain 24-7. No physical therapy relieved it. Getting hurt like this in a bad car accident and then crushing the discs even worse going right to work . I am so relieved to read all your successes and to see there is a new life ahead for me too with this surgery. 💜🙏

I sadly got taken off the surgery list when I went last month for the appointment and thrown back in so much pain to three more months of physical therapy. I was so devastated it was like someone kicked me in the stomach and knocked the wind out of me. The pain is daily and it's destroying the quality of my life and I told my surgeon that. His reply that day was your not gonna get relief from this surgery . So you can see why I am at a loss for words . I actually feel like there is no hope at all for me . I'm praying for you even though it have complete faith you are gonna do amazing.🙏♥️

@@elizabethelizabeth7059 and don't be terrified because your surgeon is sure you need it and even more importantly he got she is sure they can do it 110% correctly and seamlessly. That's everything. I felt chills leaving that appointment last month and I knew it was time for a 2nd opinion. Yale I truly thought was everything. It's not .

I am so happy for those who achieved pain relief from their laminectomy procedure! I have endured 16 years of chronic, 24-7 puke causing pain, 3 surgeries; the last one a 5 1/2 hr. laminectomy, where the surgeon claimed to have spent 4 hours attempting to open the canal. Now my back is a total mess, remining a large cavity of missing bone plates and unable to receive any additional surgeries, according to MRI and multiple doctors. My surgeon was terminated after my surgery, due to other circumstances, not mine. Thankfully I have recently received massive relief of pain from the implant of an FDA approved, Abbott Spinal Stimulator device! Pain is no longer my friend. I am so disappointed that so many people have to suffer due to insurance and poor quality surgeons for this spine topic. Kudos to the Great Surgeons! You are Under-appreciated!

I had a laminectomy in 1999 two herniated disc and pinched nerve, my accident happened in 97 so it was a failed surgery, the nerve was damaged and for the last 25 yrs I had and still have leg pain with multiple infections, physical therapy and pain killers, I got a MRI a month ago and I have two herniated disc in the same place and now arthritis in my lower back, the pain drives me to think about suicide just to stop all the pain, I’m also happy for all the people who’s surgeries were successful, my new pain management doctor wants me to try another injection and think about the spinal cord simulator

Not a form of pain relief for lower back pain, this is relief of impingement of nerves in the lower extremity and lower body. The removed portion allows the nerves to expand posteriorly as there is likely pressure from the disc that is impacting motor function, sensation and causing pain.

@Lorin Sanchez In summary, spine surgery is not a great option to relieve back pain. Spine surgery is an often to relieve nerve pain and sensory issues that are impacting movement by causing numbness, paralysis or pain. A lot of the time it is a matter of creating space (decompression) for the spinal cord and nerve branches to "breathe" if you will and regain motor function in the extremities.

i had this done a year ago and the pain and needles in my legs where relieved. Never a will I ever go to a chiropractor. After 3 weeks of no sleep and 24 hour pain, Dr. Bejjania operated on me. What a relief and thank God for him and his help. Worst pain I have ever had period.

I'm 60. Had laminotomy and foraminotomy 11 weeks ago. Back in the gym about 60 to 80 % of what I was lifting. Don't do squats or deadlifts anymore. Before operation I could only walk 8 to 10 min before feet went numb and I had to sit for a few min to decompress. My operation wasn't full laminectomy. Basically cleaned out extra bone growth and partial laminectomy on L4.

In 2019 I was taken into hospital because I was very unwell, it seems that I had Sepsis, the surgeons could not find where it was, eventually they found it in my back. It was in the form of a absess that had wound its way around my spinal cord, it had also attacked a vertibra and eaten it away, I nearly died, they did not expect me to live until the morning, they cleared the absess out and removed the vertibra. Previously, I had needed 3 hip operations, I needed to get along with one stick, after Sepsis I needed 2 sticks and walk very bent over, I already had curvature of the spine, it also seemed as if my neck was jutting more forward than it was before. When indoors I get around without sticks, but need the sticks outside because I have had several falls in the past so I have lost confidence in walking. Now, I also have two swollen feet with numbness. I am grateful to the hospital for saving my life, but is the back problems I have now and not being able to stand or look up is due to the removal of the vertibra?

I had L4-L6 laminectomy 2021 then L2-L4 laminectomy 2023. Still have leg pain and weakness from waist/hips down to knees. Been going to Accupuncture weekly since August 2023-Jan 2024. Helped a little but still have same symptoms on legs.

Had a laminectomy in May 2021. It was supposed to be an easy operation. I could barely move after a week in the hospital. When I was discharged I could barely move and was back in the hospital again for another week with DVTs and luminary embolisms the same day I was discharged. Now 5 months later I am almost as good as before the operation but still using a walker. Can't ask the surgeon anything since he died a week after surgery.

I am looking at an L4/L5 fusion...I wish it was as simple as a laminectomy. Not always a success and a one year to recovery. (Maybe walking slowly after a week or so but full recovery will take longer) NHS waiting will take me years before I can get surgery. If I can I will go to India. Around £8K and ist class surgeons. (Many of the surgeons in UK are from India) - Right now I can't stand or walk more than 15 mins. I can sit down - that does give me instant relief. Just want to be able to run a mile and walk ten miles in one go would be fantastic.

I've had this procedure done. Oh the fun I had🤣🤣🤣 The spinal tap was to die for😁👍 But seriously, it was a horrible process but in the end I had a pretty good result and significant reduction in pain. If you have this done you really need to push yourself through the pain during recovery. More pain more gain.

This represents the worst pain and suffering I've ever experienced in my 55 years. I still remember the tears coming down my cheeks as I tried to do anything. Even sleeping was next to impossible unless I took pain killers. I would wake up every 3.5 hours as they wore off! The operation was the only thing that worked for me. Thank God and Dr. Mark Thomas whose name I'll never forget. The moment I woke up I said to the nurse "The pain, it's gone". Then i asked her to immediately change the TV from CNN to Fox News! Today, I would ask her to turn the TV OFF but this was 16 years ago.

How long was your recovery time? My dad did this surgery in Jan 2022. This surgery left his leg limp and more immobile than before.

This is so very helpful to read.I thank you, everyone. I will be having surgery on the 26th of January . I am very nervous , and I am seeing a chiropractor right now, which has alleviated some of the pain, but I'm only in week two. I live by myself so I am wondering if any of you that have had the surgery were able to do things on your own? Or did you have to have somebody come in and help looking forward to the answers thank you all so much.

I had this procedure done by Dr. Howard Tung, I live in the southern california area. He told me it could cure my back pain. Some time later another Dr. told me this procedure is almost never done for back pain alone and mostly focuses on alleviating the nerve pain down the backs of the legs (Sciatica). Prior to my surgery I had endured about 30 seconds of this nerve pain once within a year. For some reason Dr. Tung, the day prior to surgery, thought it was constant and ongoing. This operation left me in significantly more pain and much greater pain than when I entered the hospital. I could hardly walk after 3 days in the hospital. I could neither empty my bladder nor bowels for 3 days as the pain associated with pushing was too great. Dr. Tung told me this was an outpatient procedure. He told me a lot of things verbally. His bedside manners were horrendous. During my post op visit he explained that I also had a broken facet he explained "It was probably from your fall". I never had any fall and I never told anyone I had fallen from any height causing me a work related injury. As far as I am concerned this man is a quack who has been somehow milking the system with fraudulent and illegal practices for decades.

Is a radical laminectomy the same. I had this done in '87 with a ruptured disc. I was good until 1998 and I assume it was scar tissue that had been torn. I've been miserable ever since.

I had a spinal laminectomy a few years ago. Everything was fine but a few months ago my pain returned. Currently my lower back has the pain of a slipped disc and even meds does not help much. I have had 3 back surgeries and they said they would not do another one.

What about the missing bone ? Cant you put in a plastic cover back over ? Or a coral calcium growing bone that can reform that part ? I have heard of this, type of regrowing the bone. What about that ?

An MRI showed my lamin verdebrae (sic?) was squeezing my L4 and L5 spinal cord area almost completely. Had the laminectomy 5 weeks ago. Unbearable pain is totally gone. I am 82 years old and able to bend over to weed my garden again. My doc says nerves (spinal cord) down will not need the now gone lamin protection unless I decide to take up sky diving or professional snow ski jumping.

What did you have done ? I had a laminectomy l1-s5 l4-l5 and a discectomy l4-l5 . After a6 weeks felt great but it reherniated Went in for another discectomy and guess what it popped out again .. Don’t know what to do now as I can’t have a disk replacement due to these surgeries and I’m unsure about a fusion at 35

I just wanna ask, In this operation all or a part of lamina will removed; which means that the risk of a bad injury in the spinal cord will increase, Is there a procedure for protecting this region or this will change and limit your life activity for along of your life?

Four months after surgery and it is looking like a failure. Sciatica remains down left leg, actually worse than before surgery, also lost feeling in most of left leg . Laminectomies at L1 - L4, micro-diskectomy at L3 & L4. Before surgery doc was telling me I would be playing golf in three to four months. After surgery he says that he should have done a fusion, shakes his head and walked out. It was beyond a doubt the most painful surgery I have had. At four months out it is difficult to walk more than 50 feet without a walker. Surgeon will not talk to me and the PA while a really nice guy is worthless. So much for playing golf again.

I'm sorry to hear what happened to you. I'm 74 and what I've learned over these 74 years is that doctors can no longer be trusted (they used to be trusted), and they no longer care about their patients. They care about money. I now get all my medical advice from people who comment on youtube's medical videos. If I have a problem I put it into youtube's search box and then read all the longer comments, on all the videos that came up in the search. Doing that has saved me many thousands of dollars, and has solved my medical problems. I even cured a skin cancer I had....all on my own, and it cost me less than a dollar (apple cider vinegar and iodine over a period of 3 weeks). EDIT: I didn't take the ACV and iodine internally. I applied the mixture directly to the cancerous growth.

@@sewingmistakesunlimited It's amazing how useful reading comments is. In fact just recently, my sister who has always trusted doctors, was told to get surgery on a rotator cuff problem. I went on youtube and did research on it, reading people's experiences. Over and over again I saw that people were healing it on their own. So I emailed the best videos on the subject...with the best comments, and she checked them out. She cancelled her surgery which was due a few days away at the time and has been healing very nicely for the past few weeks. She's thanked me a thousand times, well, not a thousand, but a lot. I'm glad to hear that someone does what I do. Thank you for letting me know.

it usually isnt filled as bones heal quite rapidly and regenearete and if it is due to some other issues a small part of hip bone is taken to guide or to fill the region

I am 46 and still race dirt bikes, I overjumped a ramp and caused damage to L4,5 on left side. They want to do this operation on me next week. Dr said I'll be back to normal 6-8 weeks and able to ride again. Reading comments below I'm not so sure about that. What are your all's experiences?

I am 24 and have this issue, I have had it for around 4-5 years, I am an avid football player and enjoy pretty much every sport, while I am not playing now will I be able to return to play many months after surgery and recovery? 😢

After working as a first surgical assist with a fellowship trained orthopedic spine surgeon, I can say that this type of surgery is best done for LEG pain, from nerve root compression. NOT FOR BACK PAIN. It is very deceiving to suggest that surgery for BACK pain, with the exception of fusion, with or without instrumentation, may provide temporary relief. Why, only temporary??? Because after a fusion, the levels above and below the fused segment do "double duty" and will wear out . leading to arthritis (and pain).

Will it relax the muscles? I started with sciatica. Procedures have moved the pain to my side hip and lower back. I am considering this surgery. Opinion?

@@sewingmistakesunlimited If your MRI shows nerve root compression that corresponds with your leg pain symptoms, then it may fit with the suspected cause. A suggestion would be to have a selective nerve root injection, under fluoroscopic guidance. This numbing of a single nerve may predict if your suspected cause is right and also mimic the relief you may enjoy if you did have surgery to decompress the nerve root. It is like having a toothaches. When you numb the one tooth, one would expect all of the toothache pain to temporarily be relieved. If it is not, then you may be numbing the wrong tooth, or have the wrong diagnosis.
