THIS IS AN OLDER VERSION OF THIS SKILL. View how this skill should look when performing for the Florida CNA State Exam. View to proper way to safely position a bedbound patient in the bed without the use of siderails to change the patient's sheets. Thank you for the comment. Side rails are not graded on the CNA exam because they are not present in all settings. It is much more important to learn how to safely turn a patient in bed when siderails are not available for use. You promote safety by scooting the patient toward you, then rolling them into the center of the bed facing away from you. This video demonstrates the proper procedure for safety and is presented according to current Florida testing standards. Thanks and Good Luck!

No Gloves, but not required on state exams, dropped call button, no person helping move or secure resident. no draw sheet. She did not raise bed to the height she can work with comfortably. Excellent job explaining what she was doing and showed much concern for the comfort of the resident/ patient. :-)

For the exam, the candidate will be instructed to change the top sheet, bottom sheet and pillow case - therefore no draw sheet will be used. This is a single-person skill and there will be no assistance available in the exam. Gloves are not required since the patient is fully clothed and there are no body fluids. You may not rely on side rails during the exam for safety - it must be performed safely using the techniques in the video. You may raise the bed if you wish but that is not a graded point - and if you raise it, it must be lowered at the end for safety. Many healthcare settings do not have adjustable beds so this is not required for the test. Thank you for the comment!

I wish I had found these videos the first time I took the exam. They're really good quality and show how you should actually preform the skill when testing. Every other video I've seen uses side rails, and of course two of my skills involved turning the resident on their side. I moved them to the opposite side of the bed before turning them, but they were still too close to the edge and I failed :/ Thanks to you, though, I feel more confident taking it again in a few days :)

Maybe they do the skills different in other states because in New York, she would have failed her state board she didn't check to see if the wheels of the bed is lock she didn't raise the bed to waist height, you don't ask your patients to move towards you because not all the time they can move on their own she didn't move the patient in 3 parts shoulders, hip & ankle, did not ask the patient to cross his arm & leg did not move the dirty linen away from her just flinged it & she didn't wipe the call bell she picked up from the floor.. The video was helpful to some level but she did a great job with communication & telling the patient what she was doing each time

I'm NYC due to take my test next month. My instructor told us not to wear gloves for occupied bed or nail care what was your experience with the test, feeling so confused

Yes your instructor is correct you do not wear gloves for occupied bed & nail care & you also don't cut the patients nail

I agree with you. However, we are unable to provide a complete CNA program encompassing all possible scenarios or variables in a KZfaq setting. Remember, we are offering these for FREE. Please understand these are to be used as a test prep aid, not to replace in-depth CNA instruction. We are demonstrating how these should be performed for the exam. Since siderails are not found in most testing centers, showing them on this skill will confuse the student during the test.

In the state of Florida, this is not a required step. It is covered in our classroom training, but it is not required that you raise the bed to perform any skill. In this limited setting, I chose to convey steps that were critical to the performance of the exam. Other points (such as this), should be covered by your classroom instructor, as these videos are meant to integrate into classroom training programs. Thank you and good luck.

I just want to say thank you. I did my written and clinical skills yesterday and I can tell you guys all I had to go in with was your videos which helped me tremendously. Although I made a little mistake here and there in the skills test, I saw my results today and I passed. Thanks guys. Although its a little different from some things they tell us in class, the key point is the important details.

For the exam, it does not matter which way the stitching is facing (they don't grade on that aspect). All grading points can be found on the Clinical Skills checklist on Prometric's website. However, the sheet should be applied with the stitching facing upward. so that when you turn the bed down, the stitching is folded onto the top sheet, not rubbing across the patient's skin. This is a nice thing to do, but not graded on the exam. Thanks for the question!

After spending the whole night watching 4yourCNA on you tube I took my state Exam on 12/23/2012 and wow I'm thankful for this channel. I'm in NYC and I follow the exact way the Nurse is doing it. I got Occupied Bed, Measurement and Respiration, Change resident’s position to a supported side-lying position. Now I'm waiting for my test results Thanks Nurse Patty!

I am very grateful for posting these cna training videos. They have tremendously helped me to pass both the written and skills test on my first try last July 25, 2012. The skills that I was tested on were: changing the sheets of occupied bed, measuring respiration and changing position. Again thank you very much.

You are correct in that if both siderails are raised, it is considered a restraint. However, for the exam, you will not have siderails to work with because side rails are not present in every setting and you MUST know how to perform this skill safely without siderails. This video demonstrates how to perform this safely without the use of siderails. If facilities allow siderails for safety, some unethical staff can use them for a restraint, so they are often banned from use altogether. Thanks!

The call light is not considered clean because it falls on the floor and is handled by the patient with dirty hands (patients in bed will scratch whatever itches and then press the call light...). It is important to consider patient's hands unclean at all times except for immediately after washing. Therefore, we wash their hands before they do things like eat or brush their teeth, or after unclean things like toileting. Thank you for your question!

No. By tucking the sheet on the end of the bed and making hospital corners, it prevents the sheet corners from touching the floor and becoming contaminated with pathogens. You will loosen the sheet over the toes to prevent constriction, but the sheet must be tucked on the end of the bed. However, you NEVER tuck the sheet under the mattress along the sides of the bed because that WILL cause constriction and restrains the patient. Thanks for the question! Good Luck!

Please remember that these videos are intended as a review for the Florida CNA exam and are NOT intended to represent comprehensive CNA instruction or cover every possible patient scenario. Understanding that siderails are considered a restraint and will not be present in many settings is the point I need to make here and knowing how to safely perform this skill without siderails is an important and TESTABLE part of this skill. As an RN, I am aware of how to care for patients without siderails

Florida has a lot of assisted living facilities, home care agencies and nursing homes. These settings will not have full side rails on the bed to ensure safety. Learning how to perform this skill safely without siderails means that the student is able to ensure safety, no matter what the setting! Thanks!

Using a privacy blanket will keep the patient warm and promote privacy through the skill. You may use the top sheet for this function as well, as long as the patient is comfortable. Good luck.

I was informed 3days before my test that i would be testing and had not practiced or studied for the written or skills portion of the CNA and i would like to thank you for the videos you have sheared.. i will say they helped me a lot in passing my CNA!!!

The testing center will have fitted sheets. This makes the skill much easier. Facilities no longer use flat sheets on the mattress because of the possibility of the sheet moving and the patient contaminating the mattress below. All facilities use fitted sheets now :)

Side rails are considered a restraint and cannot be used without a doctor's order. Staffing shortages and budget cuts mean there will not be excess staff available to stand by the bed to prevent injury while you do the skill. Ensuring the patient is in the middle of the bed when performing skills (either on their back or on their side) will promote safety. This is what is being graded during the exam. Good luck!

Gloves are optional for this skill. Gloves should be worn if the patient has any skin openings (sores, rashes, cuts, etc) or if you will come into contact with body fluids. For the exam, your "patient" for this skill (another CNA student) does not have any open areas on their skin and is not incontinent, so no gloves are necessary. You may wear them if you wish, but it is not required (and a waste of gloves). Hope you did well on your exam and best of luck in your new career!

Yes, patients should be asked to scoot toward you and rolled away from you so that they remain on their side in the center of the bed to ensure safety. Patients for the exam will be able to perform these actions. In actual practice, you may require additional assistance to move a patient (a drawsheet or another co-worker) but the exam is structured so that patients can perform this movement on their own. @amcollard Wearing gloves is important if the sheets may be soiled. If the sheets are not soiled, gloves are not required. You may wear gloves for the state exam if you are unsure. Good luck!

Every state is slightly different. You can see your own state's standards by googling "CNA clinical skills checklist california" and looking for the website that starts with asisvcs. This has your state's clinical skills listed and the checkpoints that will be graded. Good luck!

Many patients confined to bed will be able to roll (think pre-term labor). These videos are designed as an aid to prepare students for the Florida exam - as such the exam will specify on the care plan that the patient is able to move on their own, they just can't get out of bed (what is demonstrated here). Since drawsheets are not used in the exam, featuring a video moving the patient with drawsheets would be confusing for students preparing for the exam. Good luck!

The written skills care plan (tells you about the skill/patient) will be handed to you when the evaluator takes you into the testing room. The care plan is printed and you can read it as often as you would like and it will indicate if the patient is continent, etc. You can see the clinical skills checklist on Prometric's website (search florida cna test) which lists the care plan at the top of each skill. These videos are EXACTLY like the test (based on testing care plan). Good luck!

Thank you for your comment! There are lots of people walking down the hallway (staff, visitors, patients, etc.) and they all carry germs. You do not want any airborne germs to waft in on air currents and land inside the pillowcase where they will encounter a warm, dark, moist (from sweat or drooling in your sleep) environment, which is an ideal breeding ground for pathogens. Keeping the opening away from the door minimizes contamination of the pillow. It is an old nursing principle. :-)

@cnastudent1 While it is important not to "snap" the sheets to prevent bringing skin cells and bacteria up to the level of your face and to roll the soiled sheets to keep them from touching your uniform, you will need to unfold your linen to make the bed appropriately. Therefore, some "flopping" of the sheets may occur. Knowing WHY snapping the sheets is inappropriate gives you a greater understanding of the difference between "snapping" and "flopping"!

had this skill today. its a lot harder when you dont know if you're doing anything right or wrong lol. but i passed. one girl who i took the test with spilled water on another skill and still passed. as long as you tell the instructor your corrections, you're golden

Raising the bed to a comfortable working height is a great option, however you must remember to put it back down at the end of the skill or you will automatically fail for safety reasons (the floor is much further away when the patient tries to get up). In homecare, beds are not able to be adjusted and these videos are used in a variety of settings. Since these are demonstration skills and I am not able to "teach" in this format, we do not show raising or lowering the bed. Hope this helps!

For the test, you are not tested on the use of siderails, which means you must perform the skill safely without relying on the rails. This is because siderails will not be present in every setting (they are illegal in assisted living and most nursing homes) and are considered a restraint. To perform this skill safely without siderails, you would first scoot the patient toward you, then roll them away from you so they are on their side in the center of of the bed. Good luck!

The Florida CNA test does not require the use of the top blanket. The care plan directs you to change the bottom sheet, top sheet and pillowcase. Hospital corners keep the sheet corners off the floor. You will not place a draw sheet, incontinence pad or top blanket on the bed for the test, although many healthcare work settings may require these items. You will perform all of your skills exactly as directed in the care plan you will be given at the beginning of the skills test. Good luck!

Siderails are not always present in every setting, therefore they are not to be relied on to protect the patient's safety during the test. You should scoot the patient toward you before turning them onto their side facing away from you (so they are on their side in the middle of the bed) to ensure safety during the skill. This is the best way to ensure safety in the event you do not have siderails on the bed. Good luck!

Raising the bed is optional (many settings will not have beds that can be raised) so this is not a testing point. Gloves are optional for the exam (this will be indicated in the care plan since the patient is completely dressed and continent) and the use of underpads is not indicate in the care plan. ONLY perform the actions indicated in the care plan - no more and no less - because the care plan is developed by the nurse specifically for that patient.

Checking the ID bracelet is not required for the exam (and is not graded) because many settings do not use ID bracelets (home care, assisted living, long term care) and they are often wrong so they cannot be relied on to accurately identify a patient. This is the same standard in NC. Gloves are optional for the exam, and their use is not graded (because the "patient" is fully clothed and continent and there are no body fluids for exposure). You may use them if you wish, but it's not graded.

The bed remains locked. It is useful to check to be sure the bed is locked when moving a patient in and out of bed, but not a testing requirement when the patient remains in the bed for the entirety of the skill. If the bed is unlocked, it will move easily, a fact that you will notice right away and can correct. Thanks for the comment!

The test may not use siderails because they are considered a restraint and will not be present in all settings. You are responsible for knowing how to promote safety even when siderails are not available to use. Scooting the patient toward you and turning them away from you places them on their side in the middle of the bed, where they are safe. This is the appropriate way to position the patient. Good luck!

PART II: Since our videos are exactly like the test, if we do not use gloves, they are not required. We are not placing a chucks on the bed because the care plan instructs us to only use a fitted sheet, flat sheet and pillowcase. The care plan is the same throughout the state.You will not be able to ask performance questions as you perform your skill, but the Evaluator can answer questions about where supplies are located. The care plan tells you what you need, just read it carefully.

You should adjust the bed during the opening, before you wash your hands. The bed controls are not considered clean (the patient will scratch what itches while in bed, then touch the bed controls) and touching the controls with your clean hands can contaminate them before you begin your skill. Gloves are worn when there is a possible exposure to blood or body fluids or non-intact skin. This patient (for the exam) is continent and has no open skin, so gloves are not necessary. You may wear them if you wish, but for the exam it is not required (as demonstrated in the video). Good luck!

However, if you do raise the bed, you MUST lower it at the end of the skill or you will fail due to safety violations. Siderails are not used on the test because they are not present in all settings and you must know how to perform the skill SAFELY without them. Gloves do not need to be worn if there are no body fluids on the sheets. They are not required for the exam. Thanks.

The only thing I would like to add, that the girls who sis go to CNA schools checked the ID bracelet with the name every time when they performed their skills, and I did not, as It was not in your videos, but I called the patient by name many times. I was the first one who was performing the Clinical skills, so I did not know about the ID bracelet. Anyway, I passed, as the rest was perfect. Thanks again!

@cnastudent1 Our online program will give you a much greater understanding of the infection control principles this video is demonstrating. Raising the bed to a comfortable working height is a great option, however you must remember to put it back down at the end of the skill or you will automatically fail for safety reasons (the floor is much further away when the patient tries to get up). These videos are created for Florida CNA testing standards. Many states are similar to Florida standards and require demonstration of the same skills. The actual performance of these skills will not vary much from state to state, as all are based on nursing best practice standards which are universal. I am not specifically familiar with Texas standards, so I cannot offer guidance on this issue, however, follow your care plan that you are given for the exam and you will be fine. Good Luck!

You are suppose to raise the rail to the side in which the patient will turn to ensure patient safety. It is only considered a restraint if both side rails are raised.

Every state will have slightly different requirements. It is always best to research your state's particular requirements when preparing for the state exam. In reviewing the California standards (regionaltestingcenter org and click on Manual skills checklist) you will see that this video demonstrates all the specific checkpoints, with the exception of siderail usage. You may wish to access this resource for yourself, to further prepare for the exam. Good luck!

My state exams are tomorrow morning, and I am so nervous right now. The biggest thing I think will cause some trouble is with Blood pressure and listening to the heart, because my hearing isn't so good. So, watching this..as well as a few others helps ease the nerves big time.

Having the evaluator stand opposite for safety is fine for the exam. But what are you going to do in a setting where there is no second person available to ensure safety? Having the patient scoot toward you and turn on their side so they are lying in the middle of the bed at all times will ensure their safety, no matter what the environment! I really wish everyone making a comment would read the previous ones. They answer all questions appropriately for there state exam. Every state is different and not once did i see the sheet touch the floor. So stop picking unless your a nurse or higher seriouly. These vids or really helpful keep up the good work and thanks...

Every state will have different requirements. Remember that the goal is patient safety and not every setting has siderails. So, learning how to do this skill safely without siderails will ensure safety in any setting! Thanks!

I do not know where you got your information, but my information comes directly from the Florida Board of Nursing and Prometric testing services. You may view ALL of the testing standards on Prometric's website for yourself. You do NOT have to change the gown for this skill during the exam and side rails are not graded on the exam because you may be working in a setting that does not have side rails (home care, ALFs) and must be able to do this safely without them. These skills are up to date.

For the exam, it is not necessary to raise the bed to a comfortable working height - this step is not graded. Many settings do not have adjustable beds (home care, assisted living, long term care) so this is not a requirement for the skill. It does make it more comfortable for you and may be done if you prefer, but don't forget to lower it at the end of the skill or you will fail (because the floor is too far away and the patient may fall if they try to get up). This seems like a task that is a lot simpler than it sounds. I also love the fact that she placed a blanket on top of him while she removed the dirty linens. This was the first time that I have ever seen that done as a nursing student. In all, I will say that this is an excellent education tool. However, I do have a question. While making an occupied bed, why is it important to have the opening of the pillowcase facing away from the door? I feel better prepared on how to make an occupied bed.

You should not reach over the patient to the other side of the bed because of the risk of losing your balance and falling on top of the patient. You are correct. Good luck!

The Florida CNA exam does not grade on the use of siderails, so using them will not count. Many patients in many settings will not have siderails as they are considered a restraint. Gloves are optional for this skill (according to the care plan provided by the evaluator during testing) and are demonstrated without gloves for better visualization. Good luck!

For the Florida exam, the care plan will instruct the CNA to change the bottom sheet, top sheet and pillowcase. You will not have to put a drawsheet or chux on the bed during the linen change. Best of luck on your exam!
