Noob.. you just have to finish every question mark and you'll find pretty much everything.. in my 3rd play through I finished everything in every map.. in death March difficulty and uninstalled the game I found everything..

520 hours on steam with all the achievements and before that played on a cracked version almost 150 hours, 160 hours on ps5 and platinum. This is the first time i see this quest, this game still amaze me to this day.

im pretty sure that in blood and wine - in a small cave - you can also find someone from far far away - also different language, but then the quests ends

Found that the first time, but then again, I am nosy type who likes to go everywhere. But it is easy to miss indeed, some may consider that small island with a lone tree to be just a static element for aesthetics and not go there.
