My Revocable Living Trust was set up to avoid Probate in 2013. Are there conditions which preserve a $200K stock fund? Nursing Home care started March 2023.

It is unlikely your revocable trust provides any protection since the primary purpose for most revocable trusts is probate avoidance and not asset protection.

Medicare eventually will cover Long Term Care?

Medicare will only cover up to 100 days of long term care. Even then, there is no guarantee you will get the full 100 days depending on various other factors.

Is a person’s ABLE account protected and /or considered as an asset

Generally speaking, ABLE accounts are not considered a countable asset for Medicaid eligibility purposes.

If you are in a skilled nursing facility and are found eligible for Medicaid, the state will divert most or all of the pension income to the nursing facility to assist in covering the cost of your care.

To give a very lawyerly answer, it depends. It depends on the specific way in which the purchase is structured as well as the Medicaid rules of the state you are in.
