Pulmonary arteriovenous malformations are found in patients with liver cirrhosis, probably due to the absence of a hepatic factor.
So, liver Cirrhosis will affect the blood vessels in the lungs by 2 different mechanisms:-
1) Absence of a hepatic factor which leads to arteriovenous malformatins in the lung.
2) Production of Nitric Oxide (NO) which is vasodilatory in effect, which will dilate the arteriovenous shunts in the lungs, increasing the chance of blood passing quickly without sufficient gas exchange, which furtherly worsens the problem.
Ventilation perfusion mismatch will occur (Shunt Physiology)
Diagnose the disease with Contrast ultrasound or technicium albumin. Contrast ultrasound is more accurate, so it is used as a confirmatory test.

today during the talking with my colleagues at the hospital, I have told them that sometimes we have to think about the hepatopulmonary syndrome when a patient may clinically present a dyspnea that disappears when the patient is laying down (platipnea) and i explained to them how a an orthodeoxea may be associated, They told me they never heard of this syndrome. Then I knew that we really learn a lot on youtube especially when we follow some great channels like yours. so again thank you a lot

Fahmi Ramzy First, HYPOXIA, The main problem in Hepatopulmonary syndrome is vasodilatation due to the release of NO from the cirrhosed liver. Normally, the response of hypoxia in any part of the body is vasodilatation (to supply more blood to organs), but this is not the case in the lungs: the normal response to hypoxia in lungs is vasoconstriction (to shift blood from hypoxic areas towards well ventilated healthy areas). Unfortunately, this protective vasoconstriction cannot work in Hepatopulmonary Syndrome owing to the presence of vasodilator materials from the liver. This explains HYPOXIA Second, Platypnea We agreed that vasodilatation will occur, and adding the effect of gravity to that, you will get the most vasodilatation in the blood vessels in the base of the lungs (since blood will pool down by gravity worsening the symptoms), so as a response, the patient will lie down in bed to metigate the effects of hypoxia (and this is called Platypnea/ orthodeoxia).
